Single-retainer resin-bonded fixed dental prostheses (RBFDPs) are an excellent minimally invasive treatment option for replacing a single missing incisor, even in cases of congenitally missing teeth. Through minimally invasive preparation and the use of improved positioning splints, tissue defects can be effectively addressed to enhance aesthetic outcomes.
Objective Single-retainer resin-bonded fixed dental prostheses (RBFDPs) are described as an excellent minimally invasive treatment modality for the replacement of a single missing incisor even in cases of congenitally missing teeth that are often associated with hard and soft tissue defects that need to be properly managed to optimize the esthetic outcome. The lack of a retentive form due to the minimally invasive preparation form makes the adhesive bonding procedure for RBFDPs relatively technique-sensitive and might discourage practitioners from offering this treatment modality. Clinical considerations A patient with both maxillary lateral incisors congenitally missing was assessed for eligibility for treatment with RBFDPs. Bilateral horizontal ridge defects were present and treated through ridge augmentation to ensure an ovate pontic design and enhance the esthetic outcome. A minimally invasive preparation within enamel was conducted; the restorations were digitally designed and milled out of (3Y-TZP) zirconia ceramic with labial veneering with feldspathic ceramic for enhanced esthetics. An improved design of positioning splints was used for the adhesive bonding procedure to ensure exact, secure, and flawless insertion of the restorations. Conclusions RBFDPs offer a highly esthetic treatment modality for missing teeth in the anterior area. Tissue defects could be overcome be means of a minor oral surgery, while using improved positioning splints might ensure flawless adhesive bonding and promote the usage of RBFDPs. Clinical Significance Hard and soft tissue defects can be remarkably optimized through a minor ridge augmentation. Improved positioning splints allow an easy and secure positioning as well as visual inspection of the seating in end-position and complete removal of resin cement excess. Implementing the concept of insertion splints might promote RBFDPs for anterior tooth replacement as it helps preventing bonding errors.

Accuracy and dimension stability of scannable versus conventional interocclusal registration materials: An in vitro study

Christine Yazigi, Adham Elsayed, Sebastian Wille, Matthias Kern

Summary: This study evaluates the ability and dimensional stability of different registration materials (conventional versus scannable) for recording maxillary-mandibular relationships. The results show that scannable interocclusal registration materials have significantly lower vertical discrepancies after storage.

The Correlation of Surface Roughness Parameters of Zirconia and Lithium Disilicate with Steatite Wear

Stephan Krueger, Sebastian Wille, Matthias Kern

Summary: This study aimed to determine the influence of various surface roughness parameters of zirconia and lithium disilicate ceramics on the wear of steatite antagonists. The results showed no significant correlations between the investigated roughness parameters and antagonist wear for glazed zirconia, unglazed zirconia, and glazed lithium disilicate specimens.

Clinical reliability of self-adhesive luting resins compared to other adhesive procedures: A systematic review and meta-analysis

Mohammed Ahmed Alghauli, Ahmed Yaseen Alqutaibi, Sebastian Wille, Matthias Kern

Summary: This systematic review aimed to collect and interpret the randomized clinical trials comparing self-adhesive luting resins with other adhesive protocols. The results showed that postoperative sensitivity might not be affected by the adhesive protocol. Overall, selective etch and self-etch adhesives demonstrated better clinical outcomes compared to self-adhesive resins. Rating: 8 out of 10.

Single-retainer all-ceramic resin-bonded fixed dental prostheses: Long-term outcomes in the esthetic zone

Matthias Kern, Rainer Glaser

Summary: This article presents an update on the concept of cantilevered single-retainer all-ceramic resin-bonded fixed dental prostheses (RBFDPs) and its clinical application. Through clinical case observations, it is found that restorations made from veneered 3Y-TZP zirconia ceramic can effectively replace single incisors and maintain excellent esthetic outcomes and healthy soft tissues in the pontic area in the long term. Bonding nonretentive oxide ceramics with phosphate monomer-containing luting resins proves to be durable when adequate methods are used.

Clinical outcomes and influence of material parameters on the behavior and survival rate of thin and ultrathin occlusal veneers: A systematic review

Mohamed Alghauli, Ahmed Yaseen Alqutaibi, Sebastian Wille, Matthias Kern

Summary: This review investigates the clinical observational findings and in-vitro outcomes of thin and ultrathin occlusal veneers using different materials and approaches. It concludes that clinically used 1 mm lithium disilicate overlays and ultrathin one-step no-prep polymer infiltrated ceramic network (PICN) occlusal veneers exhibit high success and survival rates. The analysis of in-vitro studies shows variable survival rates and fracture load values based on materials, thickness, and restorative approaches used.

Summary: This study evaluates the mechanical properties and cost efficiency of direct ink writing (DIW) compared to casting and subtractive manufacturing. The results show that DIW has the potential to replace subtractive processes in dental applications, as it demonstrates promising mechanical properties and cost-effectiveness.

Survival and debonding resistance of posterior cantilever resin-bonded fixed dental prostheses for moderately and severely worn dentition during thermomechanical loading

Mohammed Ahmed Alghauli, Sebastian Wille, Frank Lehmann, Matthias Kern

Summary: The study aimed to investigate the survival, debonding resistance, and failure modes of different occlusal veneer designs used as retainers for posterior cantilever RBFDPs at two tooth wear levels. The results showed that all specimens survived the thermomechanical fatigue loading, and the debonding resistance varied among the different groups. Conservative designs with lingual and proximal bevels were recommended, regardless of the level of tooth wear.

Replacement of Two Missing Adjacent Teeth with Two Single-Retainer Resin-Bonded Fixed Dental Prostheses with Improved Proximal Design

Anne Garling, Reinhard Busch, Matthias Kern

Summary: This study presents a minimally invasive treatment approach for replacing two missing adjacent teeth with two single-retainer resin-bonded fixed dental prostheses (RBFDPs). The RBFDPs were digitally designed and milled from monolithic 3Y-TZP zirconia ceramic using CAD/CAM. The innovative design of the proximal section allowed for different paths of insertion without compromising hygiene.

The Single Midline Implant in the Edentulous Mandible-Current Status of Clinical Trials

Nicole Passia, Matthias Kern

Summary: The concept of a single midline implant in the edentulous mandible has been a topic of controversial discussion. Initial clinical results showed high implant survival rates and significant improvements in oral comfort, function, patient satisfaction, and oral health-related quality of life for edentulous patients. However, these trials had limitations in terms of sample size and follow-up period. Recent clinical investigations with longer-term observation periods have provided more insights. This overview aims to present the current literature and highlight clinical issues.

Summary: The aim of this study was to measure the wear progress of three high performance polymers (HPP) materials as well as that of zirconia after artificial aging and compare it with the well-documented wear of lithium disilicate. Monolithic zirconia ceramic demonstrated lower wear than that reported for enamel after simulated 5-year of clinical service, whereas all other test materials showed higher volume loss after artificial aging.

Summary: This study evaluated the accuracy of 3D-printed surgical guides before and after sterilization in a steam sterilizer. Forty guides were printed from five resins, and a control group was used. The fit and vertical discrepancies were measured, and the guides were sterilized and repositioned for further measurements. The accuracy was affected by material and sterilization, but clinically acceptable when an increased load was applied during repositioning.
