Infrapatellar Fat Pad Gene Expression and Protein Production in Patients with and without Osteoarthritis

Osteoarthritis (OA) is one of the most common joint disorders. Evidence suggests that the infrapatellar fat pad (IFP) is directly involved in OA pathology. However, a comparison between OA versus non-OA IFP is still missing. Thus, the aim of this study was to compare IFP molecular, adipocytes and extracellular matrix characteristics of patients affected by OA, and patients undergoing anterior cruciate ligament (ACL) reconstruction. We hypothesized that not only inflammation but also changes in adipocytes and extracellular matrix (ECM) composition might be involved in OA pathogenesis. Fifty-three patients were enrolled. IFP biopsies were obtained, evaluating: (a) lymphocytic infiltration and vascularization; (b) adipocytes area and number; (c) adipo-cytokines and extracellular matrix gene expression levels; (d) IL-6 and VEGF protein production; (e) collagen fibers distribution. OA IFP was more inflamed and vascularized compared to ACL IFP. OA IFP adipocytes were larger and numerically lower (1.3-fold) than ACL IFP adipocytes. An increase of gene expression of typical white adipose tissue genes was observed in OA compared to ACL IFP. Collagen-types distribution was different in the OA IFP group compared to controls, possibly explaining the change of the biomechanical characteristics found in OA IFP. Statistical linear models revealed that the adipocyte area correlated with BMI in the OA group. In conclusion, inflammation and fibrotic changes of OA IFP could represent novel therapeutic targets to counteract OA.

It is important to investigate the interaction between biological tissues and ASs and develop innovative AS designs.

Unveiling the effects of key factors in enhancing gastroesophageal reflux: A fluid-structure analysis before and after laparoscopic sleeve gastrectomy

Summary: This study investigated the mechanism of gastro-oesophageal reflux disease (GERD) using computational models. It found that laparoscopic sleeve gastrectomy could induce de novo GERD and worsen pre-existing GERD. Variables such as the His-angle, antral dimension, and bolus viscosity were found to affect reflux increase.

Body Weight Reduction by Bariatric Surgery Reduces the Plasma Levels of the Novel Orexigenic Gut Hormone Insulin-like Peptide 5 in Patients with Severe Obesity

Summary: This study investigated the basal INSL5 plasma levels in morbidly obese subjects before and after laparoscopic sleeve gastrectomy. The results showed that obese subjects had elevated INSL5 levels before surgery, which were positively correlated with BMI, fat mass, and leptin levels. After surgery, INSL5 levels significantly decreased, and there was no expression of the INSL5 gene in human adipose tissue. These findings suggest that the reduction of INSL5 levels after bariatric surgery may contribute to appetite reduction.

Ultrasound Imaging of Thoracolumbar Fascia Thickness: Chronic Non-Specific Lower Back Pain versus Healthy Subjects; A Sign of a Frozen Back?

Summary: Recent studies have found an association between thoracolumbar fascia thickness and reduced gliding in patients with lower back pain. This study aimed to measure and compare the thickness of thoracolumbar fascia in chronic non-specific lower back pain patients and healthy individuals using ultrasound imaging. The results showed that lower back pain patients had a loss of anisotropy in the thoracolumbar fascia, with a homogeneous increase in thickness and a loss of adaptability in the transverse direction.

Dihydrotestosterone, and Not Testosterone, Enhances the LPS-Induced Inflammatory Cytokine Gene Expression in Human Adipocytes

Summary: In this study, the effects of sex steroids on the expression of inflammatory mediators in human-derived adipocytes were evaluated. The results showed that non-aromatizable androgen DHT and the aromatase inhibitor anastrozole in combination with testosterone significantly enhanced the expression of inflammatory cytokines induced by LPS. These results confirm the involvement of sex hormones in adipose tissue inflammation and suggest a specific role for non-aromatizable androgens as amplificatory sex hormones of the inflammatory response.

Influence of transurethral catheters on urine pressure-flow relationships in males: A computational fluid-dynamics study

An Institutional Standardised Protocol for the Treatment of Acute Displaced Midshaft Clavicle Fractures (ADMCFs): Conservative or Surgical Management for Active Patients?

Summary: This study compared the clinical and radiographic outcomes of operative and non-operative treatment for acute displaced midshaft clavicle fractures (ADMCFs), and verified the effectiveness of the institutional treatment protocol. The results showed that conservative treatment had better clinical outcomes, while return to sports was longer in the surgical group. However, both groups achieved good mid-term clinical results without any signs of non-union.
