Fetal heart rate changes on the cardiotocograph trace secondary to maternal COVID-19 infection

Objective: To determine the cardiotocograph (CTG) changes in women with symptomatic COVID-19 infection. Study design: 12 anonymised CTG traces from 2 hospitals in Spain were retrospectively analysed by 2 independent assessors. CTG parameters were studied based on fetal pathophysiological responses to inflammation and hypoxia that would be expected based on the pathogenesis of COVID-19 patients. Correlation was made with perinatal outcomes (Apgar score at 5 min and umbilical cord pH). Results: All fetuses showed an increased baseline FHR > 10 percent compared to the initial recording, in addition to absence of accelerations. 10 out of 12 CTG traces (83.3 percent) demonstrated late or prolonged decelerations and 7 out of 12 fetuses (58.3 percent) showed absence of cycling. Not a single case of sinusoidal pattern was observed. ZigZag pattern was found in 4 CTG traces (33 percent). Excessive uterine activity was observed in all CTG traces where uterine activity was monitored (10 out of 12). Apgar scores at 5 min were normal (>7) and absence of metabolic acidosis was found in the umbilical cord arterial pH (pH > 7.0) in the cases that were available (11 and 9, respectively). Conclusion: Fetuses of COVID-19 patients showed a raised baseline FHR (>10 percent), loss of accelerations, late decelerations, ZigZag pattern and absence of cycling probably due to the effects of maternal pyrexia, maternal inflammatory response and the cytokine storm. However, the perinatal outcomes appear to be favourable. Therefore, healthcare providers should optimise the maternal environment first to rectify the reactive CTG changes instead of performing an urgent operative intervention. (C) 2020 Elsevier B.V. A study on exposure to greenspace during pregnancy and lipid profile in cord blood samples

Summary: This study investigated the association between prenatal exposure to greenspace and lipid levels in cord blood. The results showed that increased greenspace in residential surroundings, higher residential accessibility to green space, and more use of green spaces were associated with decreased cord blood lipid levels. The findings regarding greenspace in larger buffers and the number of plant pots were inconclusive. The study also suggested a potential mediatory role of air pollution.

A machine-learning model (MLM) combining NT-proBNP, uric acid, and sFlt-1/PlGF ratio improves the prediction accuracy for preterm preeclampsia.

Longitudinal Behavior of Left-Ventricular Strain in Fetal Growth Restriction

Summary: This study aimed to evaluate the association between fetal growth restriction (FGR) and cardiac left-ventricle (LV) dysfunction. The global and segmental LV longitudinal strain was measured using 2D speckle tracking echocardiography (2D-STE) software, and the differences in cardiac function were compared between FGR and appropriate-for-gestational-age (AGA) fetuses. The study found that FGR fetuses had lower LV strain values, indicating subclinical cardiac dysfunction. The findings provide more insights into fetal cardiac function in cases of placental dysfunction and highlight the potential use of 2D-STE in monitoring the cardiac function of these fetuses.

PlGF, sFlt-1, NT-proBNP, and uric acid measurements combined with maternal factors improved the prediction of complications.

SARS-CoV-2 Infection and Preeclampsia-How an Infection Can Help Us to Know More about an Obstetric Condition

Summary: Pregnant women with SARS-CoV-2 infection have a higher risk of maternal death, ICU admission, preterm delivery, and stillbirth. The risk of preeclampsia (PE) increases in infected pregnant women, especially in severe cases. COVID-19 and PE are likely linked through various mechanisms such as direct effects of the virus, inflammatory response, placental ischemia, myocardial injury, cytokine storm, and thrombotic microangiopathy. This paper aims to explore the similarities between PE and SARS-CoV-2 infection, using COVID-19 as a valuable study model. It highlights the potential for COVID-19-induced myocardial injury in pregnant women and its connection to increased maternal mortality rate.

Performance of fetal ultrasound and magnetic resonance imaging in predicting birthweight according to the test-to-delivery interval: A cohort study

Induction of labor versus expectant management in patients with idiopathic polyhydramnios

A systematic review on idiopathic intracranial hypertension comorbid with polycystic ovarian syndrome and its consequences
