Summary: There is an increasing demand for minimally invasive facial rejuvenation procedures that provide fast recovery and satisfying results. Hyaluronic Acid (HA) fillers are useful for restoring facial hollows and providing a mild lifting effect, but they are not effective in increasing dermal thickness and improving skin texture. Combining HA with calcium hydroxyapatite (CaHa) has shown promising results in facial rejuvenation, as HA provides hydration and volume while CaHa stimulates collagen production and provides lifting properties. HArmonyCa (TM) is a hybrid filler that combines HA and CaHa, and studies have shown its safety and efficacy in improving skin volume, lifting, and tightening. This case study demonstrates the successful use of HArmonyCa (TM) in a patient with moderate skin laxity and hollowness in the malar region, achieving satisfactory results in terms of fullness, elasticity, and turgor.

The use of fillers in these indications is associated with mild and temporary adverse events. Important advantages of hyaluronic acid fillers include the availability of hyaluronidase for rapid removal and the ability to revise overcorrection. Further controlled trials are needed to improve the evidence base.

After careful examination, a combined treatment approach was suggested, including intralesional neodymium-YAG laser photocoagulation and complete surgical removal, followed by defect closure with muscle-cutaneous flaps. Follow-up showed satisfactory results without disfigurement or functional impairment.
