Summary: In this study, an animal model was used to investigate the effects of Anesthesia, Surgery, and simulated ICU conditions (ASI) on sleep fragmentation, EEG slowing, and circadian disarray in aged mice. The results showed that ASI mice experienced frequent arousals, EEG slowing, and disrupted circadian gene expression, similar to delirious ICU patients. These findings suggest that the mouse model can be used to study the neurobiology of delirium.

Summary: The study evaluated the effect of intensive care unit (ICU) visits on the occurrence of delirium, delirium subtypes, and anxiety levels in ICU patients. The results showed that while visitation bans did not affect the incidence of delirium, they did change the delirium subtype and increase anxiety levels. Therefore, ICU visits are important in managing delirium subtypes and anxiety in ICU patients.

Summary: Delirium in older people is associated with significant morbidity and mortality, making prompt recognition essential. However, this study found that over half of older patients presenting to the emergency department with symptoms of delirium were ordered a CT-head scan, despite infrequent acute findings. Therefore, physicians should be more selective when ordering CT-heads in patients with symptoms of delirium.

Summary: International human rights movements have increased visibility and equality for LGBT+ communities. However, the health outcomes for LGBT+ people are still worse than their non-LGBT+ counterparts. This article focuses on the impact of societal structure and health and social care on the lives of older LGBT+ individuals, highlighting areas such as physical and mental health, end of life care, dementia, housing and care settings, and experiences of transgender individuals. The article discusses existing improvements made by LGBT+ communities and proposes a person-centered care approach to improve inclusivity. It also provides a framework for addressing service challenges and making health and social care services more prepared to meet the needs of older LGBT+ people.

Summary: No tele-health transitional care interventions specifically target unpaid caregivers of persons living with dementia (PLWD) despite their frequent hospitalizations and readmissions. This study evaluates the acceptability and experience of caregivers participating in Tele-Savvy, an evidence-based online psychoeducational intervention for PLWDs' caregivers, after their PLWDs' hospital discharge. The findings provide valuable insights for the development of a new transitional care intervention that meets caregivers' needs and schedule post-discharge.

Summary: This paper presents the expansion and evaluation of the Geri-a-FLOAT program, a virtual curriculum designed to convene geriatrics fellows for learning and peer support. Evaluation results showed high levels of participant satisfaction and sustained impact one year after the curriculum. Summary: Due to the increasing medical complexity of patients, heterogeneity of treatment effects in broad clinical syndromes, and significant competing risks for morbidity and mortality, future clinical trials in the CICU are likely to benefit from innovative approaches to trial design, analysis, and execution, such as incorporating novel trial endpoints, Bayesian analysis strategies, trial networks and clinical registries, adaptive platform trials, and comprehensive research programs. It will be crucial for the authors as a field to articulate priorities for future research and embrace these novel techniques that will ultimately help take better care of patients in the contemporary CICU.
