Summary: This study found that left atrial (LA) size only affects 1-year recurrence rate after catheter ablation in the highest tertile group, while pulmonary vein (PV) to LA volume ratio (PV/LA%vol) can predict 1-year recurrence rate in all LA size groups. Patients with lower PV/LA%vol have a higher AF recurrence rate, and in patients with smaller LA size, PV/LA%vol is more accurate in predicting AF recurrence rate compared to LA dimension (LAD). Additionally, PV/LA%vol is associated with the PITX2 gene.

Summary: This study investigates the association between lesion size index (LSI) and acute conduction gap formation during high-power (HP) ablation for pulmonary vein isolation (PVI) in atrial fibrillation (AF) patients. Using LSI-guided HP ablation, a LSI value above 4.35 for all PV segments showed the best predictive value in avoiding gap formation for effective first-pass PVI. The optimal LSI cutoff values for predicting gap formation were 4.55 for the anterior wall and 3.95 for the posterior wall.

Summary: For patients undergoing radiofrequency pulmonary vein isolation, the necessity of pulmonary vein angiography was investigated after the use of high-resolution mapping catheters and high-power, short-duration ablation technique. The study found that omitting pulmonary vein angiography had no significant impact on the efficacy, efficiency, and complications of the ablation procedure. Therefore, avoiding pulmonary vein angiography may be useful, especially for young patients and those with chronic renal disease, when using this protocol.

Summary: The ZFHX3 gene is associated with atrial fibrillation, inflammation, and fibrosis. This study found that the gene is also associated with extra-pulmonary vein triggers, left atrial structural remodeling, and poor rhythm outcomes after AF catheter ablation. Extra-pulmonary vein triggers are independently associated with the ZFHX3 polygenic risk score and low left atrial voltage.

Summary: Atrial fibrillation (AF) often requires invasive treatment in the form of ablation, but incomplete ablation of the pulmonary veins (PV) can still cure AF in some patients. This suggests that an antiarrhythmic effect other than electrical isolation plays a role in preventing AF. We propose that modifying the arrhythmogenic substrate in the PV can be a new therapeutic approach for a subgroup of patients with PV reentry.

It was found that PFA did not induce any signs of esophageal injury on cardiac magnetic resonance (CMR) imaging after PVI, indicating a potential improvement in safety for catheter ablation of AF.

Summary: This study aimed to determine if the timing of catheter ablation for atrial fibrillation (AF) affects arrhythmia outcomes. The results showed that there was no significant difference in the atrial arrhythmia free survival at 12 months post-ablation between patients who underwent early ablation and those who underwent delayed ablation. Other secondary outcomes also showed no significant difference. Therefore, delaying AF ablation by 12 months for antiarrhythmic drug management does not reduce the efficacy of ablation.

The results showed similar arrhythmia recurrence rates between the two treatment groups, and no excess of procedural complications or clinical events in the extensive ablation group.

Summary: This study investigated the bleeding risk following endoscopic submucosal dissection (ESD) in surgically altered stomach for early gastric cancer (EGC). The results showed that the bleeding risk did not significantly differ between the surgically altered stomach and whole stomach after ESD. Independent risk factors for bleeding after ESD in surgically altered stomach were ischemic heart disease and P2Y12 receptor antagonist.

Summary: This study aimed to evaluate the feasibility and safety of endoscopic submucosal dissection (ESD) in early esophageal cancer after chemoradiotherapy (CRT) by analyzing the association between submucosal fibrosis and ESD. The results showed that although ESD can be reliably performed without adverse events in early esophageal cancer, it is more difficult for lesions with strong submucosal fibrosis, leading to longer procedure time.

Summary: A 63-year-old man with vomiting and anorexia was found to have a pancreatic mass and a retroperitoneal mass extending to the liver. An esophagogastroduodenoscopy revealed advanced gastric cancer, while a biopsy showed poorly differentiated adenocarcinoma. Biopsies were taken from the pancreatic and retroperitoneal masses, revealing liposarcoma in both cases. The retroperitoneal liposarcoma was identified as the primary lesion, the pancreatic mass as metastasis, and the gastric carcinoma as an independent tumor. EUS-FNA was helpful in differentiating the metastatic tumors from gastric cancer.

Summary: Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) is the preferred method for obtaining pathological tissue samples from gastrointestinal subepithelial lesions (SELs). The diagnostic accuracy of EUS-FNA for SELs has improved with the introduction of fine-needle biopsy needles, forward-viewing echoendoscope, rapid on-site evaluation, and macroscopic on-site evaluation. EUS-FNA can be safely performed to diagnose SELs, even for smaller lesions.

Summary: Endoscopic submucosal dissection (ESD) is the standard treatment for superficial esophageal squamous cell carcinoma (SESCC), but the treatment strategy for SESCC complicated by esophageal varices (EVs) has not been established. This report presents two cases of SESCC in patients with alcoholic cirrhosis complicated by EVs who underwent ESD. EVL and sclerotherapy were performed before ESD, but in one case, severe bleeding resulted in incomplete ESD.
