Characteristics of Esophageal Injury in Ablation of Atrial Fibrillation Using a High-Power Short-Duration Setting

Background: The mechanism of esophageal thermal injury (ETI; esophageal mucosal injury and periesophageal nerve injury leading to gastric hypomotility) remains unknown when using a high-power short-duration (HP-SD) setting. This study sought to evaluate the characteristics of esophageal injuries in atrial fibrillation ablation using a HP-SD setting. Methods: After exclusion of 5 patients with their esophagus at the right portion of left atrium and 21 patients with additional ablations such as box isolation and low voltage area ablation in left atrium posterior wall, 271 consecutive patients (62 +/- 10 years, 56 women) who underwent pulmonary vein isolation by radiofrequency catheter ablation were analyzed. In the 101 patients, a HP-SD setting at 45 to 50 W with an Ablation Index module was used (HP-SD group). In the remaining 170 patients before introduction of the HP-SD setting, a conventional power setting of 20 to 30 W with contact force monitoring was used (conventional group). We performed esophagogastroduodenoscopy after pulmonary vein isolation in all patients and investigated the incidence and characteristics of ETI. Results: Although the incidence of ETI was significantly higher in the HP-SD group compared with the conventional group (37% versus 22%, P=0.011), the prevalence of esophageal lesions did not differ between the groups (7% versus 8%). Multivariate logistic regression analysis revealed that the use of the HP-SD setting (odds ratio, 6.09, P0.001), and the parameters that suggest anatomic proximity surrounding the esophagus, were independent predictors of ETI. However, the majority of ETI in the HP-SD group was gastric hypomotility, and the thermal injury was limited to the shallow layer of the periesophageal wall using the HP-SD setting. Conclusions: Although the use of the HP-SD setting was a strong predictor of ETI, it could avoid deeper thermal injuries that reach the esophageal mucosal layer.

Summary: This study investigated the bleeding risk following endoscopic submucosal dissection (ESD) in surgically altered stomach for early gastric cancer (EGC). The results showed that the bleeding risk did not significantly differ between the surgically altered stomach and whole stomach after ESD. Independent risk factors for bleeding after ESD in surgically altered stomach were ischemic heart disease and P2Y12 receptor antagonist.

Summary: This study aimed to evaluate the feasibility and safety of endoscopic submucosal dissection (ESD) in early esophageal cancer after chemoradiotherapy (CRT) by analyzing the association between submucosal fibrosis and ESD. The results showed that although ESD can be reliably performed without adverse events in early esophageal cancer, it is more difficult for lesions with strong submucosal fibrosis, leading to longer procedure time.

Retroperitoneal liposarcoma with pancreatic metastasis and gastric cancer: a case report

Summary: A 63-year-old man with vomiting and anorexia was found to have a pancreatic mass and a retroperitoneal mass extending to the liver. An esophagogastroduodenoscopy revealed advanced gastric cancer, while a biopsy showed poorly differentiated adenocarcinoma. Biopsies were taken from the pancreatic and retroperitoneal masses, revealing liposarcoma in both cases. The retroperitoneal liposarcoma was identified as the primary lesion, the pancreatic mass as metastasis, and the gastric carcinoma as an independent tumor. EUS-FNA was helpful in differentiating the metastatic tumors from gastric cancer.

Endoscopic ultrasound-guided fine-needle aspiration for gastrointestinal subepithelial lesions

Summary: Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) is the preferred method for obtaining pathological tissue samples from gastrointestinal subepithelial lesions (SELs). The diagnostic accuracy of EUS-FNA for SELs has improved with the introduction of fine-needle biopsy needles, forward-viewing echoendoscope, rapid on-site evaluation, and macroscopic on-site evaluation. EUS-FNA can be safely performed to diagnose SELs, even for smaller lesions.

Endoscopic submucosal dissection for superficial esophageal squamous cell carcinoma in patients with cirrhosis and esophageal varices

This report presents two cases of SESCC in patients with alcoholic cirrhosis complicated by EVs who underwent ESD. EVL and sclerotherapy were performed before ESD, but in one case, severe bleeding resulted in incomplete ESD.
