Summary: This study aimed to investigate the impact of pre-existing atrial fibrillation (AF) on adverse outcomes after noncardiac surgery. The results showed that AF was associated with increased risk of mortality, heart failure, and stroke, but decreased risk of myocardial infarction. AF improved the discriminative ability of the revised cardiac risk index.

Summary: Myocardial injury after non-cardiac surgery (MINS) is a common complication that can lead to major adverse cardiovascular events. Through pre-operative whole blood gene expression profiling, we identified pathways related to platelet activity and coagulation that are associated with MINS. Additionally, higher pre-operative THBS1 mRNA expression was found to be a risk factor for subsequent development of MINS and was associated with cardiovascular events.

Summary: HF patients with reduced ejection fraction have increased risks of noncardiopulmonary complications, mortality, and readmission after noncardiac surgery compared to those with preserved ejection fraction. Targeted perioperative care for different HF subtypes may be crucial for improving outcomes in this patient population. Despite no statistically significant difference in cardiopulmonary complications between HF subtypes, interventions to reduce the occurrence rate may still be clinically meaningful.

Summary: This study aims to determine the impact of the COVID-19 outbreak on hospital surgical activity and the occurrence of perioperative COVID-19 in two screening pathways for elective and non-elective surgery. The elective pathway involved surveys and RT-PCR tests for asymptomatic and low-risk patients, while the non-elective pathway conducted preoperative screening during hospital admission. Results showed a significant reduction in surgical activity during the outbreak, with a higher incidence of COVID-19 and mortality in non-elective surgery patients.

Summary: Myocardial injury after non-cardiac surgery (MINS) is a common and serious complication that significantly affects perioperative survival. Despite rare intraoperative anesthesia-related deaths, about 1% of non-cardiac surgery patients die within 30 days after surgery. With a large number of surgeries performed annually, postoperative death is the second leading cause of death in the United States. MINS is defined as an elevation in troponin concentrations within 30 days postoperatively. Preventive measures, systematic surveillance approaches, and treatment standards for MINS are still lacking, but there are modifiable factors that can be considered in clinical practice, such as hemodynamic control, adequate oxygen supply, metabolic homeostasis, perioperative medication use, and anesthesia choices.
