Iron, ferroptosis, and new insights for prevention in acute kidney injury

Acute kidney injury (AKI) is a very common condition with high morbidity and mortality, which can be seen in 5-7% of all hospitalized patients and in up to 57% of all intensive care unit admissions. Despite recent advances in clinical care, the prevalence of AKI has been shown to increase with virtually no change in mortality. AKI is a complex syndrome occurring in a variety of clinical settings. Early detection is crucial to prevent irreversible loss of renal function. The pathogenesis of AKI is highly multifactorial and complex, including vasoconstriction, reactive oxygen species formation, cell death, abnormal immune modulators and growth factors. Emerging evidence from both human and animal studies suggests that dysregulation of iron metabolism may play a potentially important role in AKI. Therefore, targeting the iron homeostasis may provide a new therapeutic intervention for AKI. New therapeutic strategies including iron chelation therapy, targeting iron metabolism related proteins and direct inhibitors of ferroptosis are imperative to improve the outcomes of patients. Taking into consideration the complexity of AKI, one intervention may not be enough for therapeutic success. Future preclinical studies in animal disease models followed by well-designed clinical trials should be conducted to extend findings from animal AKI models to humans.

Factors affecting change in renal function after contrast-enhanced computed tomography in cancer patients

Summary: This study retrospectively analyzed the changes in kidney function in cancer patients after undergoing 206 contrast-enhanced computed tomography (CECT) procedures. The results showed that CECT had a negative impact on kidney function, but did not affect oncological treatment. The deterioration of kidney function was associated with the initial creatinine and urea levels of the patients.

Resistant hypertension in dialysis

Summary: Hypertension is common in chronic kidney disease patients and is a significant risk factor for cardiovascular disease, end-stage kidney disease, and mortality. The current definitions of resistant hypertension do not apply directly to end-stage kidney disease patients. Confirmation of adherence to therapy and confirmation of uncontrolled blood pressure values through ambulatory or home blood pressure measurement are necessary for the diagnosis of true resistant hypertension. The concept of apparent treatment-resistant hypertension has also been introduced. Further research is needed on drug adherence, blood pressure measurement, and defining resistant hypertension in this population.

Patterns of Cognitive Impairment in Hemodialysis Patients and Related Factors including Depression and Anxiety

Apparently Resistant Hypertension in Polish Hemodialyzed Population: Prevalence and Risk Factors

Summary: This study aimed to assess the prevalence, characteristics, and determinants of apparent treatment-resistant hypertension (aTRH) in a large population of end-stage kidney disease (ESKD) patients undergoing hemodialysis (HD). Prevalence rates of hypertension and aTRH were determined using pre- and post-dialysis blood pressure criteria. The results showed that aTRH in ESKD HD patients appears to be multifactorial, influenced by patient-related factors rather than dialysis-related factors.

Iron status and anemia control are related to peritoneal membrane properties in peritoneally dialyzed patients

Summary: This study investigated the relationship between peritoneal membrane characteristics, anemia, and inflammation in peritoneal dialysis patients. The complex interplay of several biomarkers of iron status and their association with peritoneal transport, inflammation, and residual renal function were examined. The results showed that residual renal function significantly affects anemia, and peritoneal transport is associated with inflammation and markers of iron status.

Free-Circulating Nucleic Acids as Biomarkers in Patients After Solid Organ Transplantation

Summary: This review discusses various types of extracellular DNA (such as cell-free DNA) that circulate in human blood, particularly in inflammatory conditions and organ damage. Cell-free DNA (cfDNA) is considered a valuable additional tool in the post-transplant monitoring of transplant patients, complementing traditional strategies and serving as a risk stratification and prognostic marker. Improved sensitivity and specificity in cfDNA detection are necessary for personalized patient management, and further research is needed in terms of measurement, standardization, and processing.

Prevalence of cognitive impairment in peritoneal dialysis patients and associated factors

Summary: This study aimed to assess the prevalence of cognitive impairment (CI) and associated factors in patients undergoing peritoneal dialysis (PD). The study found that 33% of PD patients had CI, and that patients aged >= 65 years old were more likely to have CI. Memory and verbal fluency were the most affected cognitive domains in PD patients with CI.

Cognitive impairment and anxiety are prevalent in kidney transplant recipients

Summary: This study found that cognitive impairment is common in kidney transplant recipients. Patients with impaired cognition scored significantly lower in all cognitive domains compared to cognitively normal patients. Additionally, better cognitive functioning after transplantation was associated with more years of schooling. Depression and anxiety were also prevalent among some kidney transplant recipients.
