Cost-Effectiveness of Tucatinib in Human Epidermal Growth Factor Receptor 2-Positive Metastatic Breast Cancer From the US and Chinese Perspectives

Background:The clinical evaluation of HER2CLIMB trial showed a 21. 9-month median overall survival with the triplet regimens of tucatinib, capecitabine, and trastuzumab (TXT) for patients with human epidermal growth factor receptor 2 (HER2) -overexpressing metastatic breast cancer. From the payer's perspective of the United States and China, a cost-effectiveness analysis was conducted to evaluate the costs and benefits of adding tucatinib in this study. Methods:We constructed a Markov model for the economic evaluation of adding tucatinib to trastuzumab plus capecitabine in patients with HER-2 positive metastatic breast cancer in the United States and China. The model was conducted with a 10-year time horizon, and the health status was divided into three states: progression-free survival, progressing disease, and death. The health utility scores were consistent with published literature with similar patient status. The transition probabilities were derived from the survival data of the HER2CLIMB study. The unit prices of medicines were obtained from the West China Hospital, Red Book, and published literature. Outcomes were measured in quality-adjusted life-years (QALYs), and incremental cost-effectiveness ratio, which robustness was evaluated by deterministic and probabilistic sensitivity analyses. Results:Compared with the two-drug regimen of trastuzumab plus capecitabine (TX), the addition of tucatinib increased 0.21 QALY, with an increasing cost of $146,995.05 and $19,022.97 in the United States and China, respectively. The incremental cost-effectiveness ratios (ICERs) for the TXT versus TX was $699,976.43 in the U.S. and $90,585.57 in China, both of which are higher than their respective threshold of willingness to play. Deterministic sensitivity analysis shows that the price of tucatinib is the parameter that has the most significant impact on ICERs, but it does not change the results of the model. Probability sensitivity analysis shows that the probability of cost-effective for TXT is 0 in the base case. Conclusion:In the United States and China, tucatinib combined with trastuzumab and capecitabine is not cost-effective for patients with HER-2 positive metastatic breast cancer.

Summary: In this study, a core collection of Astragalus germplasm resources was established using 10 SSR markers. Among different methods, the CoreFinder-based M strategy was found to be the most suitable approach. The genetic diversity and population structure of the core collection were similar to the initial collection. The construction of a core collection is beneficial for the understanding and utilization of Astragalus, and serves as a valuable reference for constructing core collections of other plants and fungi.

Summary: This study found that ionizing radiation can promote PD-L1 expression in macrophages, and ATM kinase inhibitor further enhances this effect. ATM inhibition leads to DNA damage, which activates the STING/IRF3 signaling pathway, upregulates type I interferon expression in macrophages, and ultimately leads to PD-L1 upregulation. ATM inhibition also increases CD8(+) T cell infiltration induced by IR and promotes anti-tumor efficacy in vivo.

A Generalized Deep Learning Clustering Algorithm Based on Non-Negative Matrix Factorization

Summary: In this article, a Generalized Deep Learning Clustering (GDLC) algorithm based on Non-Negative Matrix Factorization (NMF) is proposed to address the slow convergence and low clustering accuracy in the update process of NMF. A nonlinear constrained NMF (NNMF) algorithm is constructed to achieve sequential updates of the matrix elements guided by the learning rate, and the GDLC algorithm is constructed by transforming gradient values into generalized weights and biases through a nonlinear activation function. The experimental results on eight datasets demonstrate the efficient performance of the GDLC algorithm.

Prevalence of taste and smell dysfunction in mild and asymptomatic COVID-19 patients during Omicron prevalent period in Shanghai, China: a cross-sectional survey study

Summary: This study aimed to investigate the prevalence of taste and/or smell dysfunction in mild and asymptomatic patients with Omicron infection in Shanghai, China. The study found that 44% of patients had chemosensory dysfunction, with 22.7% experiencing both taste and smell dysfunction. In addition, cough, expectoration, fever, and sore throat were common symptoms during Omicron infection.

Research trends of targeted therapy for cholangiocarcinoma from 2003 to 2022: a bibliometric and visual analysis

Summary: In the past 20 years, targeted therapy for cholangiocarcinoma has received attention and the number of papers in this field has significantly increased. This study used bibliometric and visual methods to explore the current status and future directions in cholangiocarcinoma-targeted therapy research. A total of 1057 English papers published from 2003 to 2022 were analyzed. The analysis showed an increasing volume of annual publications, with the USA leading in terms of number of publications and the Mayo Clinic as the dominant institution. The study identified research hotspots and frontiers, suggesting that growth factor receptors and pathogenesis may become future research topics.

High-Accuracy Quantitative Functional Imaging Technique for Retinal Opto-Physiology with Optical Coherence Tomography

Summary: Retinal opto-physiology is the physiological response of the retina to a visible light stimulus, reflecting the function of the retina to a certain extent. Optoretinography (ORG) is a newly developed functional imaging technique that uses optical coherence tomography to precisely quantify the opto-physiological response of the retina. By combining adaptive optics and phase analysis techniques, ORG can achieve high resolution and be used to measure retinal opto-physiological functions at the cellular level. Once standards are established, ORG may have broad applications in ophthalmic research and clinical practice.

This review highlights the important role of USP1 in cancer development and the regulatory mechanisms that control its activity, providing new insights into cancer treatment.
