How Accurate Is Oral Implant Installation Using Surgical Guides Printed from a Degradable and Steam-Sterilized Biopolymer?

3D printed surgical guides are used for prosthetically-driven oral implant placement. When manufacturing these guides, information regarding suitable printing techniques and materials as well as the necessity for additional, non-printed stock parts such as metal sleeves is scarce. The aim of the investigation was to determine the accuracy of a surgical workflow for oral implant placement using guides manufactured by means of fused deposition modeling (FDM) from a biodegradable and sterilizable biopolymer filament. Furthermore, the potential benefit of metal sleeve inserts should be assessed. A surgical guide was designed for the installation of two implants in the region of the second premolar (SP) and second molar (SM) in a mandibular typodont model. For two additive manufacturing techniques (stereolithography [SLA]: reference group, FDM: observational group) n = 10 surgical guides, with (S) and without (NS) metal sleeves, were used. This resulted in 4 groups of 10 samples each (SLA-S/NS, FDM-S/NS). Target and real implant positions were superimposed and compared using a dedicated software. Sagittal, transversal, and vertical discrepancies at the level of the implant shoulder, apex and regarding the main axis were determined. MANOVA with posthoc Tukey tests were performed for statistical analyses. Placed implants showed sagittal and transversal discrepancies of <1 mm, vertical discrepancies of <0.6 mm, and axial deviations of <= 3 degrees. In the vertical dimension, no differences between the four groups were measured (p <= 0.054). In the sagittal dimension, SLA groups showed decreased deviations in the implant shoulder region compared to FDM (p <= 0.033), whereas no differences in the transversal dimension between the groups were measured (p <= 0.054). The use of metal sleeves did not affect axial, vertical, and sagittal accuracy, but resulted in increased transversal deviations (p= 0.001). Regarding accuracy, biopolymer-based surgical guides manufactured by means of FDM present similar accuracy than SLA. Cytotoxicity tests are necessary to confirm their biocompatibility in the oral environment.

Short Versus Regular-Length Implants to Rehabilitate Partially Edentulous Mandible: A 2-Year Prospective Split-Mouth Clinical Study

Summary: This study compared the clinical outcomes of short implants and regular-length implants in patients with fixed restorations after 2 years. The results showed that both types of implants had a 100% success and survival rate. There was no statistically significant difference in peri-implant marginal bone level between the two types, but short implants showed higher clinical attachment level and probing depth values.

Cytotoxicity of polymers intended for the extrusion-based additive manufacturing of surgical guides

Summary: Extrusion-based printing enables simplified and economic manufacturing of surgical guides for oral implant placement. The cytotoxicity of two materials, biocopolyester (BE) and polypropylene (PP), intended for printing surgical guides, was evaluated. The results show that all evaluated materials are biocompatible for short-term application, but long-term contact with ME may induce inflammatory and apoptotic responses.

Influence of loading and aging on the fracture strength of an injection-molded two-piece zirconia implant restored with a zirconia abutment

Summary: The objective of this study was to investigate the fracture strength and potential phase transformation of an injection-molded two-piece zirconia implant restored with a zirconia abutment after loading and/or aging. The results showed that dynamic loading and the combination of loading and aging reduced the fracture strength of the implant abutment combination. Hydrothermal treatment caused a shallow transformation zone which had no influence on the fracture strength.

One-Piece Zirconia Oral Implants for the Support of Three-Unit Fixed Dental Prostheses: Three-Year Results from a Prospective Case Series

Summary: The study investigated the clinical and radiological outcome of one-piece zirconia oral implants supporting three-unit fixed dental prostheses after three years. 54 implants were placed in 27 patients through a one-stage surgery and immediate provisionalization. Standardized radiographs were taken at different time points to evaluate bone loss and soft-tissue parameters. The zirconia implants exhibited a cumulative survival rate of 98.1% after three years, with a mean marginal bone loss of 2.16 mm. However, high peri-implantitis incidence and low implant success rate were observed, indicating a possible concern for long-term stability.

Influence of planning software and template design on the accuracy of static computer assisted implant surgery performed using guides fabricated with material extrusion technology: An in vitro study

Summary: This study evaluated the influence of planning software and design of surgical templates on the accuracy and precision of static computer assisted implant surgery. Virtual positioning of two adjacent oral implants using two planning software was performed and surgical guides were fabricated using material extrusion. The installation of implants using these guides showed clinically acceptable deviations.

Elemental analysis of contemporary dental materials regarding potential beryllium content

Summary: This study examined the elemental composition of a wide range of commercially available dental materials, with a focus on beryllium content, and found that beryllium content in all samples was below the limit of quantification, indicating that possible traces of beryllium in dental materials were clinically irrelevant.

Guided flapless apicoectomy of the palatal root of a maxillary molar: a case presentation

Summary: This case report presents a novel minimally invasive fully guided apicoectomy technique for the treatment of a maxillary first molar using a custom-made 3D-printed template. The combination of high-resolution CBCT devices, CAD planning software, and CAM technologies has allowed for improved diagnostic and treatment planning in endodontics. The presented technique demonstrates successful outcomes and has the potential for future applications in preserving infected maxillary molars.

Reconstruction of dental roots for implant planning purposes: a feasibility study
