Summary: This study investigated the serum miRNA profiles of patients with ocular sarcoidosis and idiopathic uveitis characterized by ocular manifestations of sarcoidosis. The results showed distinct differences in serum miRNA expression between diagnosed ocular sarcoidosis and suspected ocular sarcoidosis patients, but the patterns were highly similar. Common pathways, including WNT signaling and TGF-beta signaling, were found to be involved in both groups. This suggests that these two conditions may share a similar underlying pathology and represent possible variants of the disease.

Summary: Sarcoidosis is a granulomatous disease of unknown origin that affects multiple systems. Diagnosis can be made through histological identification of non-caseous granulomas or a combination of clinical criteria. Treatment is often necessary to alleviate symptoms and prevent permanent organ dysfunction, particularly in cases of cardiac sarcoidosis. FDG-PET/CT and FDG-PET/MR have emerged as important imaging techniques for the diagnosis, staging, and guiding of biopsies in sarcoidosis. These imaging modalities can identify active inflammatory granulomas and serve as prognostic and therapeutic tools. This review highlights the critical roles of hybrid PET imaging in sarcoidosis and discusses potential future developments using other radiotracers and artificial intelligence applications.

Summary: A 62-year-old man with a history of allergic rhinitis and lightheadedness was admitted after an episode of sustained ventricular tachycardia detected by Holter monitor. A chest CT revealed peribronchial nodules, and mediastinal and hilar lymphadenopathy. MRI and FDG PET/CT confirmed the diagnosis of isolated cardiac sarcoidosis of the right ventricle, which is extremely rare.

Summary: This study aimed to determine the usefulness of ocular screening in sarcoidosis in a Finnish university hospital population. The results showed that out of 262 patients with sarcoidosis, only 5 patients (2%) were diagnosed with asymptomatic uveitis, and no other types of ocular sarcoidosis were found. Based on these findings, it was concluded that systematic screening for asymptomatic ocular sarcoidosis does not seem useful in the Finnish population.

Summary: This study aimed to profile specific inflammatory pathways in patients with sarcoidosis and cardiac involvement, finding increased activation of soluble cytokine receptors that may contribute to inflammatory cell maturation and transmigration in both conditions. Increased plasma levels of cytokines, soluble cytokine receptors, matrix metalloproteinases, and endogenous MMP inhibitors were observed in patients, with certain profiles emerging for CS patients, suggesting potential implications for the pathogenesis and treatment of cardiac sarcoidosis.

Summary: In the study, monitoring inflammasome activation in response to UCN-01 was found to differentiate FMF patients from healthy donors and patients with other inflammatory disorders. Combining pyroptosis and IL-1 beta measurement increased the sensitivity and specificity of UCN-01-based assays for accurate FMF diagnosis. The UCN-01-triggered monocyte responses were influenced by MEFV gene dosage and mutations, similarly to clinical phenotypes.

Summary: This study aimed to describe the frequency and type of cardiac complications associated with sarcoidosis in a large cohort of patients with sarcoid uveitis. The prevalence of cardiac sarcoidosis was found to be low among patients with sarcoid uveitis, but those with previously diagnosed systemic sarcoidosis or who developed systemic sarcoidosis during follow-up appeared to be at higher risk.

Summary: This retrospective study aimed to determine the etiologies and prognosis of inflammation of unknown origin (IUO) and the contribution of complementary examinations. The results showed that more than half of the IUO cases remained undiagnosed and non-infectious inflammatory diseases and hematological malignancies were the most common causes. 18-FDG-PET/CT had the highest diagnostic value. Most IUO without final diagnosis persisted, and the role of empirical treatments needs further exploration.

Summary: Glycosylated ferritin (GF) has been found to be a good diagnostic biomarker for adult-onset Still's disease (AOSD), with high diagnostic accuracy and specificity. Setting the GF threshold at 16% can increase the specificity and positive predictive value for AOSD diagnosis. GF is important for ruling out differential diagnoses and classifying AOSD in clinical trials.

Summary: This study used multiple-cause-of-death analysis to investigate the mortality and associated causes of Hemophagocytic lymphohistiocytosis (HLH). Between 2000 and 2016, HLH was mentioned in 2072 death certificates, with 232 cases as the underlying cause of death and 1840 cases as a non-underlying cause of death. The age-standardized mortality rate of HLH was 1.93 per million person-years and showed an increasing trend over the study period. HLH decedents were more likely to have associated CMV infections or hematological diseases compared to the general population.
