Favorable clinical response and drug retention of anti-IL-6 receptor inhibitor in rheumatoid arthritis with high CRP levels: the ANSWER cohort study

Y. Nakayama, M. Hashimoto, R. Watanabe, K. Murakami, K. Murata, M. Tanaka, H. Ito, W. Yamamoto, K. Ebina, K. Hata, Y. Hiramatsu, M. Katayama, Y. Son, H. Amuro, K. Akashi, A. Onishi, R. Hara, K. Yamamoto, K. Ohmura, S. Matsuda, A. Morinobu

Summary: IL-6Ri may be a favorable therapeutic option over TNFi and CTLA4-Ig in RA patients with high CRP levels, as indicated by significantly higher 3-year drug retention rates. However, no significant difference was found in terms of CDAI 12 months after bDMARD initiation among different CRP tertile groups.

Perceptions and behaviours related to COVID-19 in patients with rheumatoid arthritis: a cross-sectional study

Summary: Patients with rheumatoid arthritis showed concerns about COVID-19 during the pandemic, but most believed the risk of infection was low. Those with low health literacy had limited knowledge about COVID-19 and were less likely to change behaviors. Healthcare providers should pay attention to vulnerable individuals and effectively communicate COVID-19 related information.

Clinical profile and outcome of large-vessel giant cell arteritis in Japanese patients: A single-centre retrospective cohort study

Eriho Yamaguchi, Keiichiro Kadoba, Ryu Watanabe, Takeshi Iwasaki, Koji Kitagori, Shuji Akizuki, Kosaku Murakami, Ran Nakashima, Motomu Hashimoto, Masao Tanaka, Akio Morinobu, Hajime Yoshifuji

Summary: This study compared the clinical characteristics and outcomes between patients with giant cell arteritis (GCA) with or without large vessel involvement (LVI). The results showed that GCA without LVI had more active disease, severer vascular damage, and worse survival.

Summary: Specific SSc-related autoantibodies are associated with the morbidity and mortality of SRC. Anti-topoisomerase I, anti-RNAPIII, and anti-U1-RNP are associated with the development of SRC, while anti-topoisomerase I is associated with 1-year mortality of SRC.

Summary: This study reveals that defective TCR signaling alters the gut microbiota, promotes Th17 cell differentiation, and triggers systemic autoimmunity, which is a significant finding for understanding the pathogenesis of SLE.

Management of sarcopenia in patients with rheumatoid arthritis

Mie Torii, Takahiro Itaya, Hiroto Minamino, Masao Katsushima, Yoshihito Fujita, Hiroki Tanaka, Yohei Oshima, Ryu Watanabe, Hiromu Ito, Hidenori Arai, Motomu Hashimoto

Summary: This review summarizes the evidence for the management of sarcopenia in patients with rheumatoid arthritis (RA) in terms of drugs, exercise, and nutrition. The prevalence of sarcopenia in patients with RA is higher than that in the general population, making the treatment and management of sarcopenia clinically important for long-term prognosis. However, there remains a lack of concrete evidence on sarcopenia treatment and management in patients with RA, and more longitudinal and intervention studies are needed in the future.

Baseline erythrocyte sedimentation rate level predicts long-term inhibition of radiographic progression by tocilizumab: the KURAMA cohort

Ryu Watanabe, Kosaku Murakami, Toshimitsu Fujisaki, Hiromu Ito, Koichi Murata, Wataru Yamamoto, Takayuki Fujii, Hideo Onizawa, Akira Onishi, Masao Tanaka, Akio Morinobu, Motomu Hashimoto

Summary: The long-term effects of tocilizumab on joint destruction in rheumatoid arthritis patients were evaluated in this study. The results showed that patients who achieved structural remission were relatively younger, had a higher proportion of anti-citrullinated protein antibody positivity, and lower levels of C-reactive protein and erythrocyte sedimentation rate. Multivariate logistic regression analysis demonstrated that the baseline erythrocyte sedimentation rate level was significantly associated with structural remission.

Rheumatic Immune-Related Adverse Events due to Immune Checkpoint Inhibitors-A 2023 Update

Quang Minh Dang, Ryu Watanabe, Mayu Shiomi, Kazuo Fukumoto, Tomomi W. Nobashi, Tadashi Okano, Shinsuke Yamada, Motomu Hashimoto

Summary: With the increase in elderly patients, malignancies are becoming more common in rheumatoid arthritis (RA) patients and often pose challenges to RA treatment. Immune checkpoint inhibitors (ICIs) have emerged as a promising treatment option for various malignancies by blocking immunological brakes on T lymphocytes. However, ICIs are also associated with numerous immune-related adverse events (irAEs) including hypophysitis, myocarditis, pneumonitis, and colitis. Moreover, ICIs can exacerbate existing autoimmune diseases and cause rheumatic irAEs such as arthritis, myositis, and vasculitis. This review provides a comprehensive summary of the mechanisms, management, and potential therapeutic strategies for rheumatic irAEs.

Ryu Watanabe, Motomu Hashimoto

Summary: This review summarizes the latest knowledge on the pathophysiology of eosinophilic granulomatosis with polyangiitis (EGPA) and focuses on the roles of eosinophils and antineutrophil cytoplasmic antibody (ANCA). Furthermore, it introduces the current treatment recommendations and accumulated evidence for mepolizumab on EGPA. Based on current unmet clinical needs, potential future therapeutic strategies for EGPA are discussed.
