Comparison of acute and chronic myocardial injury in noncardiac surgical patients

Purpose Perioperative myocardial injury is a predictor of postoperative mortality, but the clinical impact of chronic injury during the perioperative period has not been fully investigated. This study aimed to evaluate chronic myocardial injury during the perioperative period in comparison with normal and acute myocardial injury. Methods Patients with serial cardiac troponin measurements before and within 30 days following noncardiac surgery were divided into three groups: normal, acute injury, and chronic injury groups. Acute and chronic myocardial injuries were stratified according to 2018 recommendations by the International Federation of Clinical Chemistry and Laboratory Medicine's Task Force on Clinical Applications of Bio-Markers. Thirty-day and one-year mortalities after surgery were compared. Results Of the 22,969 patients reviewed, 17,671 (76.9%) were classified into the normal, 5,179 (22.5%) into the acute injury, and 119 (0.5%) into the chronic injury groups. The acute and chronic injury groups had higher 30-day mortalities compared with the normal group (0.8% vs. 8.0%; hazard ratio [HR], 11.00; 95% confidence interval [CI], 9.05-13.37; P < 0.001 and 0.8% vs. 7.6%; HR, 10.55; 95% CI, 5.37-20.72; P < 0.001, respectively). In a direct comparison between the acute and chronic injury groups using an inverse probability of weighting adjustments, the 30-day and one-year mortalities were not significantly different. Conclusion Chronic myocardial injury during the perioperative period may show similar clinical impacts on postoperative mortality compared with acute injury. Further studies are needed.

Association between high preoperative body mass index and mortality after cancer surgery

Summary: Despite the association between obesity and increased mortality in the general population, a paradoxical relationship is observed between obesity and mortality in surgery and some types of cancer. This study found that high BMI was associated with lower mortality after cancer surgery, especially in an Asian population. Overweight and obese individuals also had lower mortality compared to those with a normal BMI. However, this association was not observed in female patients and those undergoing breast and gynecological cancer surgery.

Association between Mortality and Sequential Organ Failure Assessment Score during a Short Stay in the Intensive Care Unit after Non-Cardiac Surgery

Summary: This study aimed to evaluate the association between the Sequential Organ Failure Assessment (SOFA) score and outcomes in patients who stayed in the intensive care unit (ICU) for less than one day after non-cardiac surgery. The results showed that a higher initial SOFA score was associated with increased three-year mortality. Additionally, worsening of the SOFA score after surgery was associated with higher three-year mortality and complications. These findings suggest that the change in SOFA score should be considered at discharge from the ICU.

Improved Early Detection Models of Pharyngocutaneous Fistula after Total Laryngectomy

Summary: In this study, we aimed to develop prediction models for the early detection of pharyngocutaneous fistula (PCF) after total laryngectomy (TL), which could prevent severe complications. We retrospectively analyzed clinical data from patients who underwent TL and found that factors such as fever, white blood cell count, C-reactive protein levels, neutrophil count, and neutrophil-to-lymphocyte ratio were significantly associated with PCF. By combining these factors, our predictive models showed improved diagnostic performance compared to fistulography alone.

Comparison of Early and Late Surgeries after Coronary Stent Implantation in Patients with Normal Preoperative Troponin Level: A Retrospective Study

Summary: Current guidelines recommend delaying noncardiac surgery for 6 months after drug eluting stent implantation. However, a retrospective study found that early surgery within 6 months after coronary stent implantation did not increase the incidence of myocardial injury in patients with normal preoperative hs-cTnI. The study also showed no differences in secondary outcomes between early and late surgery groups.

Survey of Public Attitudes toward the Secondary Use of Public Healthcare Data in Korea

Clinical Outcomes Associated with Degree of Hypernatremia in Neurocritically Ill Patients

Digital Breast Tomosynthesis versus MRI as an Adjunct to Full-Field Digital Mammography for Preoperative Evaluation of Breast Cancer according to Mammographic Density

Summary: This study compared the effectiveness of digital breast tomosynthesis (DBT) and magnetic resonance imaging (MRI) as adjunct methods for the preoperative evaluation of breast cancer. The results showed that regardless of breast density, DBT plus full-field digital mammography (FFDM) had a higher specificity than MRI plus FFDM, and may not result in a significant loss in the diagnosis of additional cancers.

Accuracy of Cloud-Based Speech Recognition Open Application Programming Interface for Medical Terms of Korean

Summary: This study evaluated the accuracy of current cloud-based speech recognition (SR) open APIs for medical term recognition in Korean. The results showed that Naver Clova SR had the highest accuracy among the three APIs tested, followed by Amazon Transcribe and Google Speech-to-Text. The accuracy of longer words did not differ significantly between the APIs.

Risk factors associated with repeated epidural blood patches using autologous blood

Summary: This study investigated the factors associated with repeated use of epidural blood patches (EBPs) for the treatment of intracranial hypotension. The results showed that in patients with spontaneous intracranial hypotension (SIH), both the international normalized ratio (INR) and cerebrospinal fluid (CSF) leakage on myelographies were significantly associated with repeated EBPs. However, these factors did not show a difference in repeated EBPs in patients with iatrogenic injury.
