Neuromodulation, including first-generation open-loop devices and second-generation closed-loop devices, is a valuable but poorly understood therapeutic option for patients with drug-refractory epilepsy. The precise therapy a patient receives is contingent on the relationship between the patient's own unique neurophysiology and the custom programming of detection and stimulation parameters. Recent evidence demonstrates that therapeutic efficacy can be achieved through neuromodulation of seizure networks, rather than simple disruption of seizure evolution. Nevertheless, the improvement in outcomes achieved combined with its minimally invasive, nondestructive nature make closed-loop stimulation a promising therapy for additional indications, such as generalized and pediatric epilepsy.

Summary: The 5-SENSE score was developed and validated to predict the focality of the seizure-onset zone in SEEG. The score showed high specificity in predicting patients where SEEG is unlikely to identify a focal seizure-onset zone, helping clinicians reduce unnecessary invasive diagnostic burden on patients and overutilization of limited health care resources. Validation of the score showed a specificity of 76.0% and sensitivity of 52.3%.

Summary: This study investigates the localization of barques across the hippocampal longitudinal axis. By analyzing data from 51 epilepsy patients, the study found that barques predominantly occur in the posterior hippocampus and are related to barque activity in the anterior hippocampus. Statistical parametric mapping confirmed significant differences in barque activity between the anterior and posterior hippocampus. The findings have important implications for the interpretation of intracranial electroencephalogram in epilepsy surgery evaluations involving the hippocampus.

Responsive neurostimulation of the thalamus improves seizure control in idiopathic generalised epilepsy: initial case series

Functional Magnetic Resonance Imaging and Diffusion Tensor Imaging-Tractography in Resective Brain Surgery: Lesion Coverage Strategies and Patient Outcomes

Summary: This study aimed to evaluate the application of DTI-tractography and combined fMRI/DTI-tractography in resective brain surgery. The results showed that patients with DTI-tractography were more likely to experience improvement or preservation of preoperative neurological deficits (PNDs) at 1 month postoperatively, while patients with combined DTI-tractography/fMRI were more likely to have complete resolution of PNDs at 6 months. The use of combined DTI-tractography/fMRI also had a positive effect on the resolution of PNDs in patients with low-grade or high-grade lesions.
