A Gene-Expression Predictor for Efficacy of Induction Chemotherapy in Locoregionally Advanced Nasopharyngeal Carcinoma

A 6-gene signature has been developed to predict the short-term tumor response to induction chemotherapy (IC) accurately. IC only confers survival benefits on patients in the benefit group identified by the gene signature, while showing no benefits on those in the no-benefit group. The gene signature has been validated in both clinical trial and external independent cohorts, indicating its potential for selecting beneficiaries of IC in locoregionally advanced nasopharyngeal carcinoma patients.
Background: Induction chemotherapy (IC) followed by concurrent chemoradiotherapy is the mainstay treatment for patients with locoregionally advanced nasopharyngeal carcinoma. However, some patients obtain little benefit and experience unnecessary toxicities from IC. We intended to develop a gene-expression signature that can identify beneficiaries of IC. Methods: We screened chemosensitivity-related genes by comparing gene-expression profiles of patients with short-term tumor response or nonresponse to IC (n = 95) using microarray analysis. Chemosensitivity-related genes were quantified by digital expression profiling in a training cohort (n = 342) to obtain a gene signature. We then validated this gene signature in the clinical trial cohort (n = 187) and an external independent cohort (n = 240). Tests of statistical significance are 2-sided. Results: We identified 43 chemosensitivity-related genes associated with the short-term tumor response to IC. In the training cohort, a 6-gene signature was developed that was highly accurate at predicting the short-term tumor response to IC (area under the curve [AUC] = 0.87, sensitivity= 87.5%, specificity = 75.6%). We further found that IC conferred failure-free survival benefits only in patients in the benefit group (hazard ratio [HR] = 0.54, 95% confidence interval [CI] = 0.34 to 0.87; P = .01) and not on those in the no-benefit group (HR = 1.25, 95% CI = 0.62 to 2.51; P = .53). In the clinical trial cohort, the 6-gene signature was also highly accurate at predicting the tumor response (AUC = 0.82, sensitivity= 87.5%, specificity = 71.8%) and indicated failure-free survival benefits. In the external independent cohort, similar results were observed. Conclusions: The 6-gene signature can help select beneficiaries of IC and lay a foundation for a more individualized therapeutic strategy for locoregionally advanced nasopharyngeal carcinoma patients.

Disparities in positive results and dissemination of randomized controlled trials in immuno-oncology

Summary: This study analyzed the disparities in positive results and dissemination patterns of randomized controlled trials (RCTs) in global immuno-oncology (IO). The results showed that positive trials were predominantly sponsored by pharmaceutical companies and focused on immune checkpoint inhibitors (ICIs) and multiple IO therapies. Factors such as recent trial start year, upfront registration, large sample size, strict criteria, and survival endpoints were associated with positive results. Trials from Mainland China had a faster publication timeline of positive results but lacked study diversity or full reporting of negative results compared to US and multinational trials. Positive trials had a shorter publication process, more secondary manuscripts, and better dissemination in high-quality journals compared to negative trials. The study suggests improvements in upfront registration, procedural integrity, and inclusion of rival trials reporting negative results for future IO-related reviews.

Patterns and prognosis of regional recurrence in nasopharyngeal carcinoma after intensity-modulated radiotherapy

Summary: This study analyzed the patterns of lymph node failure and prognosis in patients with regional recurrent nasopharyngeal carcinoma (rNPC) alone after primary intensity-modulated radiotherapy (IMRT). The results showed that in-field failure was the main pattern of regional recurrence, while out-field failure mainly occurred in the parotid gland and level Ib. Patients with regional rNPC alone had a good prognosis after salvage treatment.

Summary: The study aimed to investigate whether sublobectomy is comparable to lobectomy in elderly patients with peripheral solid-dominant NSCLC. Results showed that sublobectomy was associated with shorter operation time, but 5-year overall survival and disease-free survival did not significantly differ between the two groups. Cox regression analyses indicated that consolidation tumor ratio, visceral pleural infiltration, and smoking were independent risk factors for worse overall survival.

Summary: A meeting of experts in November 2021 discussed the performance and cost-effectiveness of EBV-based approaches for screening early stage nasopharyngeal carcinoma. Both serum EBV antibody and plasma EBV DNA testing methods were found to be favorable. Additionally, the use of MRI was recommended to increase the sensitivity of NPC detection without greatly impacting the cost-effectiveness of the screening program.

Chemotherapy-Induced Senescence Reprogramming Promotes Nasopharyngeal Carcinoma Metastasis by circRNA-Mediated PKR Activation

Summary: This study reveals that circWDR37 deficiency limits the proliferation, migration, and invasion of senescent NPC cells induced by cisplatin or gemcitabine. Mechanistically, circWDR37 binds and dimerizes PKR, leading to PKR autophosphorylation and activation. Activated PKR phosphorylates IKKβ, releasing RELA from IκBα and activating NF-κB, thus promoting the transcription of CCND1 and senescence-associated secretory phenotype component genes in a circWDR37-dependent manner. Low circWDR37 levels are associated with favorable survival and chemotherapy response in NPC patients treated with gemcitabine or cisplatin induction chemotherapy.

TEAD4 is a master regulator of high-risk nasopharyngeal carcinoma

Summary: By investigating transcription factor-target interactions, researchers identified TEA domain transcription factor 4 (TEAD4) as a master regulator in high-risk nasopharyngeal carcinoma (NPC). TEAD4 promotes NPC progression by inducing migration, invasion, and cisplatin resistance through its autopalmitoylation. This study also reveals that TEAD4's transcriptional activity is independent of its canonical coactivators YAP/ TAZ and serves as an independent predictor of prognosis and treatment response in NPC.

Summary: The ultrastructural characteristics of the entry and bioaccumulation of Fe3O4 nanoparticles in the small intestine of Cyprinus carpio were studied. The extent of damages increased with the nanoparticle concentration, and only particles with a maximum size of 20 nm could penetrate the intestinal wall.
