New insights into pain-related changes in muscle activation revealed by high-density surface electromyography

High-density surface electromyography (HDEMG) is an electrophysiological technique that can be used to quantify the spatial distribution of activity within muscles. When pain-free individuals perform sustained or repetitive tasks, different regions within a muscle become progressively more active; this is thought to reflect a strategy to redistribute the load to different regions, thus limiting localised muscle fatigue. The use of HDEMG has revealed that when people with musculoskeletal pain perform the same tasks, the distribution of activity within the same muscle is usually different, and the same muscle region tends to be active throughout the whole task without progressive activation of different muscle regions. This potentially results in a focal overload of a muscle region, and may contribute to fatigue, localised muscle pain and potentially pain persistence and/or recurrence over time. Interestingly, not all patients with musculoskeletal pain present with this regional alteration in muscle activation, reflecting the heterogeneity of patient presentations. This article will briefly review the technique of HDEMG followed by a review of studies demonstrating spatial redistribution of muscle activity in asymptomatic people during both isometric and dynamic conditions, including functional tasks. Lastly, the article will provide a review of HDEMG studies with a focus on changes in the behaviour of the lumbar erector spine and upper trapezius in people with spinal pain. These studies have revealed subtle changes in the distribution of muscle activity in people with spinal pain, which may have relevance for onset, persistence or recurrence of symptoms and could become a target of novel therapeutic approaches.

Between-day reliability of trunk orientation measured with smartphone sensors during sit-to-stand in asymptomatic individuals

Summary: In this study, trunk orientation during sit-to-stand was measured remotely using participants' own smartphones supervised over videoconferencing. The results showed good reliability of absolute and relative peak trunk orientation in the sagittal plane, with higher consistency for relative measures. These findings support the use of smartphone sensors for assessing changes in trunk orientation and assisting physiotherapists in evaluating movement patterns remotely in individuals with musculoskeletal disorders.

Measuring complexity of muscle force control: Theoretical principles and clinical relevance in musculoskeletal research and practice

Summary: Musculoskeletal conditions are a major cause of disability globally, indicating that current rehabilitation techniques do not effectively target the most influential characteristics for long-term musculoskeletal health. Traditional measurements in neuromuscular control lack the ability to capture the hidden information of a physiological system's status over time. Utilizing measurement approaches that consider the complexity of muscle force control could provide valuable insights for diagnosing and prognosing musculoskeletal diseases and injuries.

Measures of neck muscle strength and their measurement properties in adults with chronic neck pain-a systematic review

Trunk stability in fatiguing frequency-dependent lifting activities

Summary: This study investigates trunk stability during repetitive liftings in relation to risk level, using the Lyapunov's maximum exponent. The results show that trunk stability increases with increasing risk level and as the lifting task progresses over time. There is also a negative correlation between trunk stability and muscle co-activation. These findings suggest that stability measures from kinematic data can be useful in assessing the risk of work-related low back pain disorders and implementing early ergonomic interventions.

Laser-Guided Exercise (LGE) combined with PNE is more effective in reducing pain intensity compared to supervised exercise alone.

Pain chronification impacts whole-brain functional connectivity in women with hip osteoarthritis during pain stimulation

Content validity of the Scoliosis Research Society questionnaire (SRS-22r): A qualitative concept elicitation study

Summary: This study aimed to evaluate the content validity of Scoliosis Research Society-22 revised (SRS-22r) for assessing the health-related quality of life (HRQOL) of adolescents with adolescent idiopathic scoliosis (AIS). In-depth semi-structured interviews revealed that the SRS-22r does not adequately capture important concepts related to the HRQOL of AIS patients. These findings support the need for revising the SRS-22r or developing a new patient-reported outcome measure to evaluate the HRQOL of adolescents with AIS.

Effect of movement-evoked and tonic experimental pain on muscle force production

Measurement properties of a computer adaptive device, the Senscoordination 3D Cervical Trainer, to assess cervical range of motion in people with neck pain

Summary: This study aimed to assess the interrater reliability, concurrent validity, and responsiveness of the Sen-scoordination 3D Cervical Trainer (SCT) in measuring cervical range of motion (ROM). The results showed high correlation between the measures obtained with the SCT and the CROM device, as well as high interrater reliability. The SCT was found to be a valid, reliable, and responsive instrument for measuring cervical ROM.

Reliability and criterion validity of handheld dynamometry for measuring trunk muscle strength in people with and without chronic non-specific low back pain

Summary: This study aimed to determine the within-day and between-day test-retest reliability and criterion validity of a handheld dynamometer (HHD) for evaluating maximum isometric trunk strength in people with chronic low back pain (LBP) and asymptomatic individuals. The results showed that the HHD had good to excellent reliability within-day and between-day for both LBP and asymptomatic participants, and there was a moderate to strong correlation between HHD measurements and isokinetic dynamometer measurements.
