Summary: The COVID-19 pandemic has highlighted the fragility of global health and social care systems, especially in regards to the response towards older individuals. There is a need for substantial reshaping of healthcare systems, shifting the focus towards community/primary care, improving the identification and management of geriatric conditions, and enhancing training for healthcare professionals.

Summary: Suicide in older adults is a major global concern, and with the increasing aging population, there is a predicted surge in suicidal deaths. This paper reviews the risk factors, protective factors, assessment rating scales, and current prevention strategies in the geriatric population. It emphasizes the need for personalized interventions based on identifying modifiable risk factors and strengthening protective factors. The study found that factors such as psychiatric illness, physical illnesses, loneliness, and low social support commonly increase suicidal susceptibility in older adults, while good physical health, cognitive function, religiousness, and social connectedness increase resilience. Current preventive strategies have not effectively reduced suicidal risk, highlighting the need for a multilevel prevention program to mitigate risk. Further research and improved preventive measures are necessary to address the issue.

Summary: Postoperative delirium (POD) is a common complication in geriatric patients with hip fractures, associated with poorer functional recovery, longer hospital stays, and higher mortality rates. Various factors such as advanced age, preoperative cognitive impairment, comorbidities, perioperative polypharmacy, and delayed surgery increase the risk of developing POD. Effective interventions such as perioperative risk prediction, avoidance of certain medications, and orthogeriatric comprehensive care can help prevent POD. Anesthesia technique choice does not seem to significantly impact the incidence of postoperative delirium in geriatric patients with hip fractures. There are limited pharmaceutical measures available for POD treatment, with dexmedetomidine and multimodal analgesia showing potential effectiveness but also considering adverse complications when using antipsychotics. In conclusion, perioperative risk intervention based on orthogeriatric comprehensive care is the most effective strategy for preventing postoperative delirium in geriatric patients with hip fractures.

Summary: The study revealed a decrease in mortality rate among geriatric trauma patients in Al-Ain City, United Arab Emirates, over the past decade. There was a significant increase in falls on the same level and injuries occurring at home. Targeting home environments in injury prevention programs may help reduce geriatric injuries.

Summary: Brain aging is an inevitable aspect of old age, leading to changes in the brain such as shrinking of certain parts and decreased communication between neurons. Previous drug treatments for related ailments have had undesirable side effects, but recent research suggests the potential use of natural compounds. This review explores the potential of black pepper as a substance for managing brain aging.

Summary: Over the past decade, various designs for femoral augmentation have been investigated to prevent geriatric hip fracture. However, the experimental methods used to evaluate these augmentations have not been critically evaluated or compared. In this study, the authors conducted a scoping review of experimental studies on femoral augmentation approaches and identified areas for concordance with the clinical situation. They found limitations in the clinical relevance of modeled fall scenarios and large variance in the concordance of the identified studies. The authors make recommendations for improving the fidelity of experimental methods to better model clinical sideways falls.

Summary: In the DPP trial, older adults who received intensive lifestyle intervention had a 71% reduction in incident diabetes. However, a small number of older adults with prediabetes are participating in the National DPP. The BRIDGE trial aims to compare the effectiveness of in-person and virtual DPP programs in older adults and provide insights into the delivery of evidence-based interventions.

Summary: The study aimed to investigate the predictive value of end-expiratory inferior vena cava diameter and collapsibility index for post-spinal hypotension in older adults undergoing spinal anesthesia. Results showed that expVCI diameter, lactate, and pH values were effective predictors for post-spinal hypotension, indicating the potential utility of noninvasive ultrasound methods in assessing hypotension risk.

Summary: Older adults are more vulnerable to influenza and have different responses to infection and therapeutics compared to younger patients. Influenza vaccination and antiviral therapy are essential for preventing and treating influenza in the elderly. Oseltamivir is the most effective in reducing mortality and complications, while peramivir offers the shortest time for symptom alleviation and baloxavir is well tolerated.

Summary: By assessing the frequency and circumstances of falls in a hospital geriatrics ward, this study identified that falls most often occur in the patient's room while changing position. Factors associated with falls include higher disability level, presence of multiple diseases (specifically Parkinson's disease and diabetes), history of falls in the past year, and diagnosis of orthostatic hypotension. The study also found significant independent associations between in-hospital falls and a history of falls, orthostatic hypotension, Parkinson's disease, and medication use (statins, benzodiazepines, and insulin).

Summary: This review presents current knowledge on the effectiveness and safety of allergen immunotherapy (AIT) in patients over 60 years old with inhalant allergies. Studies have shown the effectiveness of AIT in treating allergic rhinitis in seniors, with evidence from trials using sublingual and injection immunotherapy. Data confirm the high safety of AIT in this age group. Seniors with allergic rhinitis or asthma may benefit from AIT if there are no contraindications, and sustained clinical benefit can be experienced even after completing treatment.

Summary: This study investigated the efficacy and safety of allergen immunotherapy (AIT) in adult patients with atopic dermatitis (AD) and monosensitisation to house dust mites (HDMs). The results showed significant improvement in skin symptoms of AD patients after 12 months of AIT treatment, with decreased EASI scores, % BSA, and IsGA index compared to the placebo group. Therefore, HDM-AIT is effective in improving atopic dermatitis in patients with confirmed monovalent mite allergy.
