Clinical Spectrum and Outcome of Patients With Non-ST-Segment Elevation Acute Coronary Syndrome and No Obstructive Coronary Atherosclerosis

Background: Because approximately 10% of patients with no-ST-segment elevation acute coronary syndrome (NSTE-ACS) show no obstructive coronary artery disease (NOCAD) on angiography, we assessed the spectrum of diagnoses and the predictors of outcome of these patients. Methods and Results: We studied 178 patients admitted to a coronary care unit with an initial diagnosis of NSTEACS, based on clinical, ECG and laboratory data, but found to have NOCAD. The final diagnosis in these patients was heterogeneous; true NSTE-ACS (ie, coronary thrombosis on an unstable plaque) was ascertained in 1 patient (0.6%), whereas diagnosis at discharge was microvascular NSTE-ACS in 56.2% of patients, variant angina in 10.1%, myocarditis in 8.9%, takotsubo disease in 7.9%, tachyarrhythmia-related chest pain in 6.7%, and non-cardiac pain in 9.6%. At 24.5-month follow-up, 21 deaths (11.8%) had occurred, 9 (5.1%) from cardiovascular causes, including 2 (1.12%) coronary deaths. By multivariable Cox analysis, age only predicted global (hazard ratio [HR] 1.07 [1.02-1.12]; P=0.006) and cardiovascular (HR 1.08 [1.01-1.16]; P=0.04) mortality; non-coronary vascular disease was the main predictor of cardiovascular death or readmission for cardiovascular disease (HR 3.28 [1.75-6.14]; P<0.001) and coronary death or readmission for angina (HR 3.20 [1.26-8.14]; P=0.014). Conclusions: Patients with an initial diagnosis of NSTE-ACS constitute a heterogeneous population with different final diagnoses. Patients have a rather high rate of fatal events, most of which, however, are not related to coronary causes.

Clinical impact of the extent of jeopardized myocardium in patients undergoing transcatheter aortic valve intervention

Summary: This study investigated the impact of the extent of jeopardized myocardium on clinical outcomes in patients with concomitant coronary artery disease (CAD) and severe aortic stenosis (AS) undergoing transcatheter aortic valve implantation (TAVI). The results showed that patients with less extensive revascularization had better survival free from major adverse cardiovascular and cerebrovascular events (MACCE). Therefore, adequate myocardial revascularization before TAVI is crucial for patients with CAD and severe AS.

Quantitative flow ratio modulated by intracoronary optical coherence tomography for predicting physiological efficacy of percutaneous coronary intervention

Summary: This study evaluates the potential of a novel method for fast computation of Murray law-based quantitative flow ratio (mu QFR) from coregistered optical coherence tomography (OCT) and angiography (OCT-modulated mu QFR, OCT-mu QFR) in predicting the physiological efficacy of percutaneous coronary intervention (PCI). The results show that simulated residual OCT-mu QFR has good correlation, agreement, and diagnostic concordance with actual post-PCI OCT-mu QFR. In OCT-guided procedures, the pressure drop of OCT-mu QFR in the stent is low and can be predicted by pre-PCI vessel/plaque characteristics.

Summary: This study compared the procedural success of different types of transcatheter heart valves (THV) in Trans-AXillary Intervention (TAXI) registry. It was found that both self-expanding (SE) and balloon-expandable (BE) THV can be safely used in TAx-TAVI, but SE THV were associated with higher device success, while BE THV were more often used in challenging anatomical circumstances.

The results showed that most of the cases had symptom relief and a reduction in CMR signs of active inflammation during short-term follow-up.

Summary: This study evaluated the incidence and risk factors of Post-COVID-19 Interstitial Lung Disease (PC-ILD) in 100 patients with moderate to critical COVID-19. It found that 8% of patients showed residual radiological and functional signs consistent with PC-ILD at the 15-month follow-up. Aging, severity of COVID-19, and degree of pulmonary involvement during acute infection were identified as significant risk factors for developing PC-ILD.
