COVID-19 is a world health emergency which may inevitably affect the management of a complex autoimmune disease such as systemic sclerosis (SSc). Several SSc patients are frail and, in this pandemic, need a careful protection. The COVID-19 infection might complicate the clinical scenario of interstitial lung disease (ILD) in SSc because it determines a severe pneumonia characterized by radiological features similar to SSc-ILD. The striking CT similarities between the 2 diseases make it difficult to distinguish a worsening of SSc-ILD from COVID-19-ILD superinfection. Moreover, other aspects, like isolation during lock down, may cause a significant psychological stress which will pile up on the already difficult contact with the patients for a routine check-up. Moreover, the drug shortage is a real problem in these times. For these reasons, the rheumatologist in daily clinical practice should carefully differentiate the possible COVID-19 infection in order to optimize the patient management. Therefore, the challenge in everyday life will be to achieve in due time the differential diagnosis as well as the long-term psychological impact.

Summary: The purpose of this study was to test the application of respiratory-triggered ultrashort echo-time (UTE) Spiral VIBE-MRI sequence in the assessment of systemic sclerosis-interstitial lung disease (SSc-ILD) compared with computed tomography (CT). The results showed that the UTE sequence had high reliability in assessing the extent of ILD and GGO.

Lung ultrasound B-lines in systemic sclerosis: cut-off values and methodological indications for interstitial lung disease screening

Summary: Lung ultrasound (LUS) can accurately detect SSc-related ILD by evaluating B-lines and pleural line alterations. A cut-off value of >10 B-lines on the whole chest or >1 B-line on the postero-basal chest can be used for screening SSc-ILD. Assessing only the postero-basal chest seems to be the most effective approach, combining high sensitivity with less time consumption.

Summary: The extent and severity of ILD can predict mortality in SSc-ILD patients on CT, as well as densitometric analysis parameters, fibrotic extent, and reticulation extent. Extensive ILD is also associated with death, need for supplemental oxygen, or lung transplantation. Honeycombing extent is a predictor for respiratory mortality.

Summary: Recent advancements have made it possible to quantitatively assess lung vascular and parenchymal changes in thoracic CT images using automated software tools. In this study, the researchers investigated the vessel parameters of patients with SSc and correlated them with interstitial lung disease (ILD) features. The results showed that the CT vessel parameters increased in parallel with ILD extent and functional impairment, suggesting that they may serve as a biomarker of SSc-ILD severity.

Summary: Systemic sclerosis (SSc) is characterized by internal organ fibrosis, including the heart, leading to high morbidity and mortality. Cardiac magnetic resonance (CMR) can assess myocardial function and tissue characteristics in the same examination. The Lake Louise criteria (LLC) can be used to identify recent myocardial inflammation using CMR. Abnormal values include myocardial over skeletal muscle ratio, early gadolinium enhancement values, and epicardial/intramyocardial late gadolinium enhancement.

Summary: Investigating unmet needs and identifying necessary interventions for patients with rheumatic and musculoskeletal diseases (RMDs) is crucial for ensuring continuity and quality of chronic care. A systematic literature review was conducted to identify nursing interventions for RMD patients undergoing biological therapy. Thirteen articles met the inclusion criteria, and three major nursing interventions were identified: education, patient-centered care, and data collection/nurse monitoring, all of which were associated with improved care satisfaction and treatment adherence among patients.

Application of logistic regression and machine learning methods for idiopathic inflammatory myopathies malignancy prediction

Summary: The study aimed to establish and use a machine learning algorithm to predict possible risk factors for malignancy in IIM patients. Age, ALT<80U/L, and anti-TIF1-gamma were found to be risk factors, while ILD was a protective factor in constructing the prediction model. Compared with other machine learning algorithm models, the logistic regression (LR) model performed well in predicting malignancy in IIM. The established prediction model helps clinicians screen, evaluate, and follow up high-risk IIM patients by constructing a diagnostic nomogram.
