Summary: This study investigated the effects of opioid rotation (OR) on pain scores, Edmonton Symptom Assessment Scores (ESAS), and morphine equivalent daily dose (MEDD) in cancer patients. The results showed that OR was associated with a significant reduction in pain scores, without significant changes in ESAS or MEDD overall. Only rotations to methadone correlated with both a significant reduction in pain scores and in MEDD.

Summary: This study assessed the trends and factors associated with opioid prescriptions for cancer patients in Songklanagarind Hospital, the largest referral center in Southern Thailand. The study found that age, cancer stage, and bone metastasis were factors associated with the amount of opioid usage. Therefore, promoting opioid prescriptions for pain management through medical education can help doctors overcome opiophobia.

Summary: This study investigates the impact of intravenous lipid emulsions (ILE) on the distribution of buprenorphine, fentanyl, and butorphanol. The results show that ILE has the ability to sequester these drugs, leading to a decrease in their concentration in plasma. Furthermore, ILE also affects the distribution of the drugs in the simulated biophase, resulting in a significant decrease in drug concentration in cells.

Summary: The study aimed to determine the escalation trend of morphine equivalent dose in milligrams (MED)/day after initial utilization. A total of 25,108 lost time claims filed between 1998 and 2007 were followed for 8 years, and the slopes of opioid dose escalation were determined for each initial MED/day group. The results showed that the MED/day increased linearly regardless of the initial MED/day dose.

Summary: Current gaps in knowledge hinder the full implementation of patient-reported outcomes in routine post-operative care. This study evaluated symptoms in patients undergoing open laparotomy using MD Anderson Symptom Inventory (MDASI-PeriOp-GYN). The findings indicate that abdominal bloating, abdominal cramping, and urinary urgency are associated with increased risk of post-operative complications. These results support the integration of patient-reported outcomes assessment in patient-centered post-operative care.

Summary: This study compared the pregnancy outcomes of high-intensity focused ultrasound ablation (HIFU), myomectomy, and uterine artery embolization (UAE) for fertility-sparing patients with uterine fibroids and investigated the possible mechanism of improving pregnancy by HIFU. The results showed that myomectomy had a higher pregnancy rate than HIFU and UAE, while the miscarriage rate of HIFU was similar to myomectomy and UAE. Subgroup analysis suggested that ultrasound-guided HIFU (USgHIFU) may have more ideal pregnancy outcomes than magnetic resonance imaging-guided HIFU. In conclusion, HIFU, especially USgHIFU, may serve as an alternative fertility-sparing modality for patients with uterine fibroids over 40 years old and improve pregnancy rates by elevating uterine blood flow impedance.

Summary: This study aimed to investigate the impact of angiotensin converting enzyme (ACE) I/D polymorphism on the risk of insulin resistance and polycystic ovary syndrome (PCOS). The findings showed a significant association between ACE I/D polymorphism and PCOS risk, especially in Caucasian populations. Interestingly, the ACE I/D polymorphism was only correlated with insulin levels and HOMA-IR in Asian populations.

Summary: A new pulmonary nodule diagnostic model was constructed in this study, which showed high diagnostic efficiency, non-invasiveness, and simplicity in measurement. By combining the 7-AAB panel, AI diagnostic system, and other clinical features, the model demonstrated good diagnostic performance in distinguishing lung nodules, especially those with diameters ≤ 2 cm.
