Summary: A biliary stricture is a narrowing in the liver's drainage system that can cause obstruction in the flow of bile, with malignancy being the most common etiology. The goal of care is to diagnose malignancy and restore bile flow to the duodenum, with different approaches depending on the location. Endoscopic ultrasound-guided tissue acquisition is accurate for diagnosing extrahepatic strictures, while perihilar strictures remain challenging to diagnose. Drainage for extrahepatic strictures is safer and less controversial compared to perihilar strictures. This guideline aims to provide evidence-based guidance on diagnosis and drainage for both types of strictures.

This clinical guideline suggests endoscopic ultrasound (EUS) with tissue acquisition for diagnosis of solid nonlipomatous SEL and EUS fine-needle biopsy or aspiration for sampling solid SEL. The guideline also recommends resection of gastric GIST >2 cm and all nongastric GIST due to their malignant potential.
