Summary: This study evaluated the clinical features, treatment response, and outcomes of liver injury following SARS-CoV-2 vaccination. The liver injury was predominantly hepatocellular and some patients showed features of immune-mediated hepatitis. Corticosteroid therapy may be beneficial for patients with immune-mediated features or severe hepatitis. The overall outcome was generally favorable, but a few patients experienced vaccine-associated liver failure.

Summary: This study showed that combined therapy with tacrolimus and low-dose steroid was noninferior to high-dose steroid for inducing complete remission in adults with minimal change nephrotic syndrome. Additionally, relapse rates were significantly lower in patients maintained on tacrolimus and steroid compared to those on steroid alone, with no clinically-relevant safety differences observed.

Summary: Despite serious complications, such as infection and recurrence of diseases, corticosteroids are still widely used as immunosuppressive therapy for liver transplant recipients. This study compared the safety and efficacy of T-cell specific antibody induction with corticosteroid induction and found that T-cell specific antibody induction significantly reduced the risk of opportunistic infections and metabolic complications.

Summary: Recent experimental investigation suggests that ursodeoxycholic acid (UDCA) may reduce susceptibility to severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection by downregulating angiotensin-converting enzyme 2 (ACE2). This study aimed to determine the potential protective effect of UDCA against SARS-CoV-2 infection in patients with chronic liver disease. The results showed that UDCA therapy was associated with a lower incidence of infection, milder cases, and shorter recovery time.

Ursodeoxycholic acid is effective in PBC treatment but limited in PSC cases, with ongoing research for new drugs.

Summary: A 37-year-old female patient diagnosed with PBC-AIH overlap syndrome and PNH was successfully treated with a combination of prednisolone and ursodeoxycholic acid, showing good recovery at the 24-month follow-up evaluation. While this represents a single case, it expands knowledge on immunological diseases associated with PNH and suggests an immune-mediated pathogenic pathway between PNH and PBC-AIH overlap syndrome.

Lymphoproliferative disorder is rare in those with native livers, but EBV IgG-negative LT recipients receiving EBV IgG-positive donor organs are at risk of early PTLD. More large multicenter studies with longer follow-up are needed to further assess the risk.

Summary: BAR502 and UDCA combination therapy showed better efficacy than individual therapy in treating NAFLD/NASH, by improving liver damage, steatosis, inflammation, and fibrosis. The combination also increased insulin sensitivity and bile acid signaling, modulated gene expression, reduced expression of inflammatory markers, and redirected bile acid metabolism.

Summary: With the increasing long-term survival after liver transplantation, the care for medical problems that arise over time in liver-transplanted patients has become important. The most common causes of death occurring more than 1 year after liver transplantation are unrelated to liver diseases and are facilitated by immunosuppressive treatments. Primary care physicians are increasingly exposed to liver-transplanted patients and should be aware of the common risks and complications faced by these patients to enhance their care. Collaboration with transplant hepatologists is crucial for implementing long-term follow-up care.

Summary: In patients with autoimmune hepatitis, dysfunction of the aryl hydrocarbon receptor pathway, resulting from aberrant inhibition or non-canonical activation, impairs Treg- and Th17 cell-induced upregulation of CD39, a key ectoenzyme for immuno-regulation. Blockade of excessive inhibition or non-canonical activation of the aryl hydrocarbon receptor pathway might represent a novel therapeutic strategy to control inflammation while restoring immune balance in autoimmune hepatitis.
