BackgroundIschemic cardiomyopathy and severe left ventricular dysfunction are well established to represent the main determinants of poor survival and premature death compared with preserved ventricular function. However, the role of myocardial revascularization as a therapeutic alternative is not known to improve the long-term prognosis in this group of patients. This study will investigate whether myocardial revascularization contributes to a better prognosis for patients compared with those treated with drugs alone and followed over the long term.MethodsThe study will include 600 patients with coronary artery disease associated with ischemic cardiomyopathy. The surgical or drug therapy option will be randomized, and the events considered for analysis will be all-cause mortality, nonfatal infarction, unstable angina requiring additional revascularization, and stroke. The events will be analyzed according to the intent-to-treat principle. Patients with multivessel coronary disease and left ventricular ejection fraction measurements of less than 35% will be included. In addition, myocardial ischemia will be documented by myocardial scintigraphy. Markers of myocardial necrosis will be checked at admission and after the procedure.DiscussionThe role of myocardial revascularization (CABG) in the treatment of patients with coronary artery disease and heart failure is not clearly established. The surgical option of revascularizing the myocardium is a procedure designed to reduce the load of myocardial hibernation in patients with heart failure caused by coronary artery disease. On the other hand, the assessment of myocardial viability is frequently used to identify patients with left ventricular ischemic dysfunction in which CABG may add survival benefit. However, the effectiveness of this option is uncertain. The great difficulty in establishing the efficacy of surgical intervention is based on the understanding of viability without ischemia. Thus, this study will include only patients with viable and truly ischemic myocardium to correct this anomaly.Trial registrationEvaluation of a randomized comparison between patients with coronary artery disease associated with ischemic cardiomyopathy submitted to medical or surgical treatment: MASS-VI (HF), ISRCTN77449548, Oct 10th, 2019 (retrospectively registered).

Summary: The ISCHEMIA trial compared the outcomes of initial invasive strategy versus initial conservative strategy in patients with chronic coronary disease and moderate or severe ischemia. The trial found no major difference in most outcomes, but there was lower risk of cardiovascular mortality and higher risk of noncardiovascular mortality with the invasive strategy.

Percutaneous coronary intervention or medical therapy in patients with severe aortic stenosis and coronary artery disease undergoing transcatheter aortic valve implantation: a real-world analysis using data from an international network

Summary: In this study, we compared the long-term cardiovascular outcomes of patients with severe aortic stenosis and coronary artery disease (CAD) who received either medical treatment alone or percutaneous coronary intervention (PCI) coupled with medical therapy before or during transcatheter aortic valve implantation (TAVI). Through the analysis of a large multinational electronic health record network, we found that the 5-year probability of death, acute coronary syndrome, and ischemic stroke did not significantly differ between the two groups. These findings suggest that medical treatment alone may be as effective as PCI combined with TAVI for patients with CAD and severe aortic stenosis.

Summary: The use of diagnostic ultrasound-guided high mechanical index impulses improves myocardial perfusion in acute ST segment elevation myocardial infarction. This study assessed the effect of sonothrombolysis on left ventricular diastolic dysfunction (DD) and left atrial (LA) mechanics. The results showed that sonothrombolysis is associated with better DD grade and LA mechanics, reducing the likelihood of LA remodeling.

Summary: This study performed miRNA profiling of patients before coronary artery bypass grafting (CABG) surgery to identify miRNAs that could predict Vasoplegic syndrome (VS). Six miRNAs were increased and two were decreased in patients who developed VS compared to those who did not. The combination of hsa-miR-30d-5p and hsa-miR-770-5p showed high accuracy in predicting VS.

Summary: This study compared the immediate outcomes of patients undergoing single arterial graft versus multiple arterial grafts coronary artery bypass grafting surgery. The results showed that the multiple arterial grafts group had a higher incidence of pulmonary complications and deep sternal wound infection, and glycosylated hemoglobin had a greater influence on the infectious outcome.

Summary: Coronary artery calcium (CAC) scanning has limited value for risk stratification in symptomatic patients. This study aimed to identify and validate predictors of significant coronary obstruction (SCO) in symptomatic patients without CAC. Results showed that an algorithm to assess the risk of SCO in patients without CAC effectively predicted the risk of significant coronary obstruction.

Postoperative myocardial fibrosis assessment in aortic valvular heart diseases-a cardiovascular magnetic resonance study

Summary: This study aimed to investigate the changes in postoperative ventricular remodeling and the influence of preoperative myocardial fibrosis on the reversal of remodeling in patients with aortic regurgitation (AR) or aortic stenosis (AS). The results showed that after surgery, left ventricle mass index decreased in both groups, while late gadolinium enhancement indices remained stable. Preoperative indexed extracellular volume (iECV) and extracellular volume (ECV) were greater in the AR group. Postoperative iECV decreased in both groups but was still greater in the AR group. Postoperative ECV remained stable in the AR group and increased in the AS group.

Summary: In this study, we investigated the additional reduction of regional stress-induced myocardial ischemia (SIMI) in patients undergoing incomplete coronary artery bypass graft surgery (CABG) when autologous bone marrow-derived cells (BMC) were injected into non-revascularized segments. The results showed that BMC treatment led to additional improvement in regional SIMI after CABG, suggesting that BMC could be a potential adjuvant therapy for patients with coronary artery disease.

Summary: Despite improvements in diagnostic and therapeutic options, ischemic heart disease remains a major cause of death worldwide. Researchers in biotechnology and tissue engineering have developed novel strategies such as stem cells and nanotechnology to reduce cardiovascular events. Advances in bioengineering have also led to new diagnostic techniques and biomarkers for atherosclerosis. This review explores innovative invasive and noninvasive modalities for coronary disease characterization, as well as technological revascularization procedures and pharmacological agents targeting residual cardiovascular risks.

Coronavirus disease-2019 and heart: assessment of troponin and cardiovascular comorbidities as prognostic markers in patients hospitalized with coronavirus disease-2019 in a tertiary center in Brazil

Summary: This study aimed to evaluate the correlation of cardiac troponin T levels with comorbidities and in-hospital outcomes in patients with coronavirus disease-2019 in Brazil. Data from a cohort of 3,596 patients were reviewed and it was found that elevated cardiac troponin T was associated with several comorbidities, biomarkers, and clinical outcomes. However, it was not an independent marker of in-hospital mortality.

Summary: This study evaluated the long-term prognosis of coronary CTA scores for cardiovascular events in symptomatic patients with suspected CAD. The results showed that coronary CTA scores based on plaque burden and characteristics can provide long-term prognostic information, especially the distribution of non-calcified plaques is associated with increased cardiovascular risk for late events.
