Vasculitis is a challenging disease for paediatricians. Certain vasculitides are quite common in children whereas others are much rarer compared with adults. The most common vasculitides in childhood are IgA-associated vasculitis (Henoch-Schonlein purpura) and Kawasaki disease, which are usually self-limiting vasculitides although children do develop complications as a result. We now have much better knowledge of how to manage these patients and prevent the deleterious complications. This review provides an up-to-date discussion on childhood vasculitides, including diagnosis, treatment and follow-up strategies, together with a comparison with vasculitides in adults. It also discusses the newly defined monogenic vasculitides that often present during early childhood.

Plasma checkpoint protein levels and galectin-9 in juvenile systemic lupus erythematosus

Kubra Yuksel, Erdal Sag, Selcan Demir, Semanur Ozdel, Ummusen Akca Kaya, Erdal Atalay, Muserref Kasap Cuceoglu, Rezan Topaloglu, Yelda Bilginer, Seza Ozen

Summary: The study found Galectin-9 and IL-2R alpha to be good markers for disease activity in childhood SLE, with significant correlations between SLEDAI scores and IL-2R alpha, Gal-9, and PDL1. Additionally, checkpoint protein levels did not differ significantly among different clinical clusters in SLE.

Clinical features, muscle biopsy scores, myositis specific antibody profiles and outcome in juvenile dermatomyositis

Summary: The study analyzed the clinical features, laboratory tests, treatment response, and long-term outcomes of 58 patients with JDM. Dermatological manifestations and muscle weakness were key diagnostic elements, while calcinosis was the most common long-term complication. The presence of TIF1g and NXP2 may indicate a severe disease course.

Genetic disorders with symptoms mimicking rheumatologic diseases: A single-center retrospective study

Ummusen Kaya Akca, Pelin Ozlem Simsek Kiper, Gizem Urel Demir, Erdal Sag, Erdal Atalay, Gulen Eda Utine, Mehmet Alikasifoglu, Koray Boduroglu, Yelda Bilginer, Seza Ozen

Summary: The study examined genetic diagnoses in patients presenting with musculoskeletal complaints to the rheumatology department, finding that a significant proportion of cases were due to genetic disorders. It highlights the importance of considering genetic factors in the differential diagnosis of musculoskeletal symptoms in pediatric rheumatology patients.

Hematological involvement in pediatric systemic lupus erythematosus: A multi-center study

Summary: The study revealed that hematological abnormalities are commonly encountered in pediatric patients with SLE, with anemia being the most common abnormality. The main treatment modalities include corticosteroids and IVIG, with rituximab being preferred for resistant cases. Positive antiphospholipid antibodies and high SLEDAI scores are independently associated with hematological involvement.

Comparison of IVIG resistance predictive models in Kawasaki disease

Ummusen Kaya Akca, Elif Arslanoglu Aydin, Hayrettin Hakan Aykan, Oguzhan Serin, Erdal Sag, Selcan Demir, Erdal Atalay, Muserref Kasap, Ezgi Deniz Batu, Tevfik Karagoz, Yelda Bilginer, Seza Ozen

Summary: The study compared ten different scoring systems to predict IVIG resistance in Turkish children, finding that these systems had low sensitivity and were unable to differentiate IVIG-resistant patients effectively. Factors like increased serum GGT levels and low platelet counts were identified as risk factors for predicting IVIG resistance.

The role of vascular inflammation markers in deficiency of adenosine deaminase 2

Ummusen Kaya Akca, Erdal Sag, Sule Unal, Muserref Kasap Cuceoglu, Yelda Bilginer, Seza Ozen

Summary: This study aimed to assess the role of vascular inflammatory factors in the pathogenesis of deficiency of adenosine deaminase 2 (DADA2) and to compare these markers among DADA2 patients with different phenotypes. The results showed significant differences in markers among DADA2 patients with various features, suggesting potential biomarkers for neuropathy and distinguishing DADA2 patients with PAN-like features from PAN patients.

Summary: The study showed a higher prevalence of Methotrexate intolerance in pediatric patients compared to adults, with a stronger correlation between Miss and VAS scores in patients receiving parenteral MTX. Younger age was identified as an independent risk factor for Methotrexate intolerance.

Selcan Demir, Fatma Bilge Ergen, Onur Taydas, Erdal Sag, Yelda Bilginer, Ustun Aydingoz, Seza Ozen

Summary: The study evaluated the frequency and spectrum of MRI findings of spine involvement in patients with JIA, finding that both ERA and non-ERA patients can have spinal involvement despite clinical symptoms or lack thereof.

Summary: Approximately one-third of children with food-induced anaphylaxis develop tolerance within three years. Risk factors for persistent food-induced anaphylaxis include multiple food anaphylaxis, total IgE levels greater than 100 kU/L, and skin-prick test wheal size larger than 10 mm.

Assessment of systemic and ocular inflammation in juvenile idiopathic arthritis via choroidal vascularity index

Abdullah Agin, Sibel Kadayifcilar, Ata Baytaroglu, Ozge Deliktas, Selcan Demir, Erdal Sag, Yelda Bilginer, Jale Karakaya, Seza Ozen, Bora Eldem

Summary: The study aimed to evaluate the retina and choroid in children with juvenile idiopathic arthritis (JIA) using optical coherence tomography (OCT). The choroid was found to be thicker in JIA patients compared to healthy controls, possibly indicating subclinical choroidal inflammation. Ophthalmologic examination, including choroidal imaging, in a larger cohort may help clarify this aspect.

The difference of the inflammatory milieu in MIS-C and severe COVID-19

Summary: This study investigates the cytokine and chemokine profiles of children with MIS-C and compares them to severe/critical COVID-19 cases and healthy controls. The results suggest that IL-1 and IFN-gamma pathways play a significant role in the pathophysiology of MIS-C. These findings contribute to our understanding of immune responses in children with MIS-C and severe/critical COVID-19.
