Summary: Non-variceal upper gastrointestinal bleeding (NVUGIB) is a common and serious medical emergency with high morbidity and mortality. While current guidelines address NVUGIB due to peptic ulcer disease, they do not cover the management of rare and extraordinary rare causes of NVUGIB. It is crucial for all involved clinicians to be familiar with and knowledgeable about these life-threatening conditions, as they often require prompt diagnosis and treatment using a multidisciplinary team approach. The aim of this review is to provide a comprehensive summary of the current evidence regarding the management of such cases.

Summary: A novel hemostatic hybrid hydrogel comprised of hyaluronic acid and polyethylene glycol stabilized by thiourea-catechol coupling and disulfide bonds is reported for endoscopic hemostasis in upper GI hemorrhage. The hybrid hydrogels exhibit enhanced mechanical properties and good blood clotting ability, showing promising results in both rat and pig models. The sustained hemostatic function of the hybrid hydrogels make them a potential candidate for clinical applications in endoscopic hemostasis.

Summary: There is a need for better patient management and physician guidance in treating iron deficiency/iron-deficiency anemia (IDA) in patients who have experienced acute gastrointestinal bleeding (GIB). Published data on this subject are limited and disparate, and there is a lack of information on post-discharge outcomes such as persistent anemia and re-hospitalization. Acute GIB-related ID/IDA is a common condition that is likely underdiagnosed and undertreated. Further research is needed to establish evidence-based guidelines for iron therapies in these patients.

The role of over-the-scope clips (OTSCs) as the first treatment has not been defined. A multicenter, randomized controlled trial showed that compared to standard endoscopic hemostatic treatments, OTSCs can reduce the risk of further bleeding from nonvariceal upper gastrointestinal causes.

Summary: Gastrointestinal endoscopy, one of the highest waste generators in healthcare facilities, produces large-volume non-renewable waste, uses single-use devices, and requires reprocessing or decontamination processes. The increasing use of single-use accessories instead of reusable devices has raised concerns about efficacy and adherence to disinfection protocols. While single-use duodenoscopes have been developed as an alternative, they contribute to amplified waste generation and carbon footprint. A balanced approach is needed to address patient safety and environmental implications, including selective use of disposable duodenoscopes and careful accessory selection during procedures. Collaboration between healthcare professionals and manufacturers is crucial for developing environmentally friendly devices with low carbon footprint.

Summary: A folic acid-conjugated gold nanoparticle has been developed for the treatment of tubulointerstitial fibrosis in chronic kidney disease. This nanoparticle can selectively transport into renal tubules and treat fibrosis more effectively than standard therapy. The size of the nanoparticle and the disease stage determine its delivery to the fibrotic kidney.
