Italy is facing its fifth week of crisis due to the Coronavirus disease (COVID-19) outbreak, with affected patients and deaths near to 70,000 and 6,000, respectively,(1)numbers that are increasing every day. Whether government imposition of quarantines, travel bans, and lockdown throughout the country will have effect in the next weeks in limiting the spreading of this disease has still to be seen. Meanwhile, a great spirit of sacrifice is required to health care personnel, and authorities have to manage resource allocation to rapidly increase the number of intensive care beds to assist patients with COVID-19.(2)Although operating rooms and a number of wards have been turned into dedicated intensive units, beds and resources are hardly sufficient to satisfy the needs of so many simultaneously critically ill patients.(3,4)Notably, among infected people, about 10% are health workers, and their number is increasing, also due to the scarcity of efficacious protective measures.

A pilot test was conducted to validate the accuracy of the models, which may help clinicians evaluate whether the visual fixation response can support the MCS diagnosis.

Summary: Patients with the coexistence of cerebrovascular fibromuscular dysplasia (cFMD) and spontaneous cervical artery dissection (sCeAD) have distinct risk factor profiles compared to patients without cFMD. The presence of cFMD and a history of migraines independently predict the midterm risk of sCeAD recurrence.

Summary: This study aimed to assess the levels of circulating endothelial progenitor cells (cEPC) and the release of selected proteins in the plasma and CSF of pediatric MMA patients to clarify the pathogenesis and progression of the disease. The study found increased levels of cEPC in peripheral blood and an upregulation of angiogenic markers in CSF. These findings provide valuable insights into the disease's mechanism and progression.

Summary: This guideline provides recommendations for the management of Moyamoya Angiopathy (MMA) based on systematic literature reviews and expert consensus. For adult patients with haemorrhagic presentation, direct bypass surgery is recommended. For ischaemic adult patients and children, revascularization surgery is suggested. Pre- and post-operative haemodynamic and posterior cerebral artery assessment is also recommended.

Quantification of retinal ganglion cell loss in patients with homonymous visual field defect due to stroke

Summary: This study aimed to quantify the degree of ganglion cell degeneration using SD-OCT in adult patients with post-stroke homonymous visual field defect. The results showed that there were significant differences in pRNFL-AVG, GCC-AVG, GLV, and FLV between stroke patients and controls regardless of stroke type and involved vascular territories. Age and elapsed time from stroke had a significant effect on pRNFL-AVG and GCC-AVG, while visual field defect size was unrelated to SD-OCT measurements.

Iatrogenic cerebral amyloid angiopathy: An illustrative case of a newly introduced disease

Summary: Iatrogenic cerebral amyloid angiopathy (iCAA) is a specific type of cerebral amyloid angiopathy, and its diagnosis is increasing. It is hypothesized that iCAA may occur as a late consequence of past neurosurgical interventions involving dural patch grafts. Although positron emission tomography (PET) scans and beta-amyloid levels in cerebrospinal fluid (CSF) are auxiliary criteria, the definite diagnosis still relies on histopathological examination.

Summary: Moyamoya angiopathy (MMA) is a rare cerebrovascular disease characterized by progressive steno-occlusive lesions and collateral vessel development. Surgical treatment, particularly indirect revascularization, is preferred. By performing proteomic characterization, a potential gene, Filamin A, was identified, suggesting a role in MMA pathogenesis. This study provides insights into the molecular understanding of this disabling intracranial vasculopathy.
