Summary: Despite the increase in living donor liver transplants, the outcomes of retransplantation after living donor liver transplant need further investigation. This study compared the waitlist outcomes and survival post-retransplant in recipients of initial living or deceased donor grafts. The results showed similar waitlist mortality and survival rates for both types of transplants.

Summary: The study retrospectively evaluated the outcomes of liver retransplantation and found that overall and graft survival rates were comparable to those of primary liver transplantation, indicating that modern liver retransplantation is a safer option for patients with liver graft failure.

Summary: This study evaluated the outcomes of heart retransplantation and found that the long-term survival rate after retransplantation was lower than after primary heart transplantation. Although the short-term outcomes were similar between the two groups, heart retransplantation should still be considered for select patients to optimize donor organ usage.

Summary: The efficacy of Lung Retransplantation (LRT) has been debated and retrospective analyses on thousands of cases provide insight into predictive patient criteria. However, the lack of a standardized approach remains apparent.

Summary: Lung retransplantation is the only therapeutic option for patients with refractory allograft dysfunction, but its efficacy and ethics are still debated. Careful consideration should be given to patient selection, donor organ selection, surgical approaches, and future considerations to improve outcomes and address organ utilization.

Summary: The time to retransplantation does not seem to affect heart retransplant recipients with a time to retransplantation of more than 1 year, but a shorter time to retransplantation for prior lung recipients is associated with decreased survival.

Summary: A risk model was developed using data from the European Liver Transplantation Registry to predict survival after liver retransplantation. The model included seven predictors of graft survival and created a simplified risk score ranging from 0-10. Different risk groups identified had significantly different 5-year survival rates, with external validation showing closely aligned expected survival rates with observed mortality probabilities.

Summary: Among kidney retransplant recipients without A, B, and DR domain DSA, there were no differences in efficacy failure and safety outcomes at 12 months between those receiving induction therapy with a single 3 mg/kg rATG dose and those receiving the traditional 5 mg/kg rATG.

Summary: This study described and analyzed the outcomes of heart retransplantation (ReHT) in Spain. The results showed that ReHT had higher mortality compared to primary heart transplantation (HT), especially when indicated for acute rejection. However, ReHT beyond 5 years after the initial HT had a similar prognosis to primary HT.

Summary: This study aimed to evaluate the impact of age at the time of returning to dialysis on the association between second kidney transplantation (2KT) and survival. The findings showed that 2KT was associated with better survival compared to remaining on dialysis after the first graft failure. However, this survival benefit decreased with increasing age.

Summary: The proportion and risk of re-KT in older adults have increased over time, but outcomes have improved significantly. These findings support increasing access to re-KT in older adults, although approaches to guide selection and management in those with graft failure need to be explored.

Summary: The study found that there were no significant differences in graft survival, patient survival, and acute rejection after kidney retransplantation between patients with and without a history of PTLD. However, the occurrence of PTLD after kidney retransplantation remained higher in patients with a history of PTLD.

Summary: Patients aged 70 and above achieve satisfactory outcomes following kidney retransplantation, highlighting that age should not preclude this medically complex population from this life-saving procedure. Improvement in functional status and timely retransplantation are the key factors to successful outcomes.

Summary: This study explores the risk factors for delayed graft function (DGF) in recipients undergoing a second kidney transplant, as well as the short-term graft and patient survival of DGF retransplants. The results show that DGF and graft loss due to acute rejection in the first transplant are predictors of reappearing DGF in the second transplant. Dialysis at the time of transplant is identified as the greatest risk factor for DGF in the second transplant. Additionally, there is a lower graft survival rate for retransplants with DGF.

Summary: This study retrospectively analyzed the outcomes of repeat pediatric kidney transplantation (RPKT) among pediatric kidney transplant recipients, focusing on surgical complications. The study found that patients undergoing RPKT face many challenges, but overall the incidence of surgical complications, particularly vascular complications, is low.

Summary: The outcomes of heart-lung transplant are worse than heart transplant and lung transplant alone. The indications for heart-lung transplant have changed over the years, with congenital heart disease and Eisenmenger syndrome being the main indications in recent times. Other indications, such as lung diseases, have virtually disappeared. Heart-lung transplant has high mortality and etiological analysis is important for improving survival.
