Significance and prognostic impact of v wave on pulmonary artery pressure in patients with heart failure: beyond the wedge pressure

Background A v wave on pulmonary artery wedge (PAW) pressure sometimes augments and appears on pulmonary artery (PA) pressure wave in patients with heart failure (HF). However, the significance of PA v wave in HF remains to be elucidated. Methods We retrospectively analyzed pressure waveforms in 61 HF patients (left ventricular ejection fraction 35 +/- 15%). On the PAW and PA pressure waveforms, mean pressure as well as peak and amplitude of v waves (ampPAWv and ampPAv, respectively) were measured. Occurrence of worsening HF and cardiac death was recorded for 2 years after the catheterization. Results The ampPAWv did not correlate with ampPAv. When the patients were divided into 4 groups: I (high-ampPAWv/high-ampPAv), II (high-ampPAWv/low-ampPAv), III (low-ampPAWv/high-ampPAv), and IV (low-ampPAWv/low-ampPAv), the prevalence of group III was low (I: 13, II: 17, III: 4, IV: 27). Mean pressures of PAW and PA were similarly elevated in groups I and II. Cardiac index was lowest (I: 2.0 +/- 0.4, II: 2.8 +/- 0.6, III: 2.2 +/- 0.2, IV: 2.4 +/- 0.6 L/min/m(2), ANOVAP < 0.01,P < 0.01 for I vs II) and tricuspid annular plane systolic excursion / systolic PA pressure was impaired (I: 0.27 +/- 0.07, II: 0.48 +/- 0.22, III: 0.59 +/- 0.35, IV: 0.68 +/- 0.35 mm/mmHg, ANOVAP < 0.01) in group I. During the follow-up, 13 events were observed. Kaplan-Meier analysis showed that patients in group I were at highest risk of cardiac events. Conclusions PA v was observed mainly in patients with augmented PAW v wave and decreased cardiac index, suggesting an advanced stage of HF. Moreover, augmented PAv was associated with worse outcome in HF patients.

Quantitative text analysis of the mechanisms of tooth injury: Analysis of accidents in five sports that occurred in 15 years under school control

Summary: This study aimed to qualitatively analyze the occurrence of dental trauma in school settings and extract information on injury mechanisms for prevention. The results showed that baseball, basketball, soccer, volleyball, and rugby were the sports with frequent dental injuries. Males were more often injured in all sports except volleyball, and the number of accidents increased among high school students. Typical accident occurrence mechanisms were identified for each sport using quantitative text analysis.

Assessing the impact of watershed characteristics and management on nutrient concentrations in tropical rivers using a machine learning method

Summary: Excessive loadings of nitrogen and phosphorus, combined with imbalances in silicon, have a detrimental effect on water quality and ecosystems in receiving waters. This study conducted periodic water quality monitoring in rivers and streams on Ishigaki Island, Japan, to identify the factors influencing the concentrations of dissolved inorganic nitrogen (DIN), total phosphorus (TP), and dissolved silicon (DSi), with a focus on catchment characteristics. Using a machine learning algorithm called Random Forest (RF), predictive models were developed to estimate nutrient concentrations based on catchment properties. The results showed that agricultural land use significantly influenced DIN and TP concentrations, while broadleaf forests were the most important factor for DSi. Additionally, the study estimated the contributions of DIN from sugarcane fields and livestock barns to riverine DIN, which accounted for up to 60% of the total in the studied river basins. The findings also indicated that DIN from sugarcane fields is more likely to leach into groundwater and rivers in catchments dominated by calcareous geology. These results and methodology have implications for water quality assessment and management in both inland and coastal waters.

Summary: This study aimed to investigate the association between fragmented QRS (fQRS) and clinical outcomes in patients with cardiac sarcoidosis (CS). The results showed that patients with fQRS had a higher risk of adverse events, particularly late-onset ventricular tachyarrhythmias (VTs). Therefore, fQRS may serve as an independent predictor of adverse events in CS patients.

Canagliflozin independently reduced plasma volume from conventional diuretics in patients with type 2 diabetes and chronic heart failure: a subanalysis of the CANDLE trial

Summary: SGLT2 inhibitors have been found to reduce the risk of heart failure progression and mortality rates. This study shows that SGLT2 inhibitors can chronically reduce estimated plasma volume and improve heart failure prognosis in patients with type 2 diabetes and chronic heart failure, especially in those with preserved ejection fraction. The subanalysis of the CANDLE trial indicates that canagliflozin significantly reduces estimated plasma volume without worsening renal function and lowers serum uric acid without causing major electrolyte abnormalities.

Empagliflozin suppresses mitochondrial reactive oxygen species generation and mitigates the inducibility of atrial fibrillation in diabetic rats

Summary: Recent studies have shown that sodium-glucose co-transporter-2 inhibitors (SGLT2-i) can reduce the risk of atrial fibrillation (AF) in patients with diabetes mellitus (DM) by reducing oxidative stress. This study aimed to further investigate the mechanisms behind this protective effect and found that empagliflozin, a type of SGLT2-i, can suppress mitochondrial ROS generation and mitigate fibrosis in diabetic rats. These findings suggest that empagliflozin may have a cardioprotective effect by reducing mitochondrial ROS through AMPK signaling pathways and potentially preventing the development of AF in patients with type 2 DM.

Venous dilation effect of hot towel (moist and dry heat) versus hot pack for peripheral intravenous catheterization: a quasi-experimental study

Summary: This study compared the venous dilation effect of using a hot towel (moist and dry heat) to a hot pack before applying the tourniquet at an access site for peripheral intravenous catheterization. The results showed that the venous diameter and assessment scores were significantly lower for the moist hot towel than for the other two heat applications. Skin temperature and warmth rating score were also significantly lower for the moist hot towel. Stratum corneum hydration showed a significant increase with the moist hot towel.

Impact of right ventricular stiffness on discordance between hemodynamic parameter and regurgitant volume in patients with pulmonary regurgitation

Summary: This study investigated the relationship between pressure half time (PHT) and pulmonary regurgitation (PR) velocity in patients with congenital heart disease after right ventricular outflow reconstruction. It was found that in some patients, despite smaller PR volume, PHT remained short. These patients had smaller right ventricular diameter and demonstrated restrictive physiology. Additionally, when mid RV diameter and restrictive physiology were considered, the predictive value for PR was significantly improved.
