Relationship between serum levels of VIP, but not of CGRP, and cranial autonomic parasympathetic symptoms: A study in chronic migraine patients

Background Cranial autonomic parasympathetic symptoms (CAPS) appear in at least half of migraine patients theoretically as a result of the release of peptides by the trigemino-vascular system (TVS). Cranial pain pathways become sensitised by repeated episodes of TVS activation, leading to migraine chronification. Objective The objective of this article is to correlate the presence of CAPS with serum levels of vasoactive intestinal peptides (VIP) and calcitonin gene-related peptide (CGRP). Patients and methods Patients with chronic migraine (CM) were asked about the presence - during migraine attacks - of five CAPS, which were scored from 0 to 10 by using a quantitative scale. Serum VIP and CGRP levels were determined by ELISA. Results We interviewed 87 CM patients (82 females; mean age 44.710.6 years). Seventeen had no CAPS, while 70 reported at least one CAPS. VIP levels ranged from 20.8 to 668.2pg/ml (mean 154.5 +/- 123.2). There was a significant positive correlation between scores in the CAPS scale and VIP levels (Spearman correlation coefficient=0.227; p=0.035). VIP levels were significantly higher in CM patients by at least one point in the scale vs those with 0 points (p=0.002). Analysing symptoms individually, VIP levels were numerically higher in those patients with symptoms, though they were significantly higher only in those patients with lacrimation vs those without it (p=0.013). There was no significant correlation between CGRP levels and the score in the CAPS scale. Conclusions Serum VIP, but not CGRP, levels seem to reflect the rate of activation of the parasympathetic arm of the TVS in migraine.

About the use of the overlap coefficient in the binary classification context

Summary: This study investigates the feasibility of using the overlap coefficient (OVL) in binary classification problems. The findings indicate that the OVL does not provide additional information and the previously reported advantages are based on the assumption that larger values of the marker are assigned higher probabilities in the classification process. Procedural Safety Comparison Between Transcarotid Artery Revascularization, Carotid Endarterectomy, and Carotid Stenting: Perioperative and 1-Year Rates of Stroke or Death

Reducing the overfitting in the gROC curve estimation

Summary: The generalized receiver-operating characteristic (gROC) curve is used to evaluate diagnostic tests that have different associations with positive results for higher and lower marker values. However, using the same data to select classification subsets and calculate the gROC curve leads to over-optimistic estimates of future performance. This study explores the bias of the empirical gROC curve estimator and proposes cross-validation based algorithms to reduce overfitting, improving the estimation of diagnostic accuracy.

Confidence intervals for the length of the receiver-operating characteristic curve based on a smooth estimator

Summary: A good diagnostic test should exhibit distinct behavior for positive and negative populations, but that is not sufficient to create a good classification system. Defining decision criteria is crucial for solving the complex binary classification problem. The receiver-operating characteristic curve length serves as an index for the optimal discriminatory capacity of a biomarker, but its estimation requires parametric or smoothed models. This study examines a kernel density estimator-based approximation for estimating the length of the receiver-operating characteristic curve and explores its behavior through statistical analysis and Monte Carlo simulations with real-world examples.

Adoption and Diffusion of Transcarotid Artery Revascularization in Contemporary Practice

Confirmatory Mediastinoscopy after Negative Endobronchial Ultrasound-guided Transbronchial Needle Aspiration for Mediastinal Staging of Lung Cancer Systematic Review and Meta-analysis

Summary: The current guidelines recommend invasive staging for non-small cell lung cancer (NSCLC) with endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). However, the indication for confirming a negative EBUS-TBNA result with video-assisted mediastinoscopy (VAM) before resection varies in different guidelines. This study evaluates the evidence for the added value of confirmatory VAM after a negative EBUS-TBNA result in NSCLC mediastinal staging, and finds that confirmatory VAM can reduce the rate of unforeseen N2/3 disease, but with a high number needed to treat (NNT), and should be recommended only for specific cases yet to be defined.
