Summary: Patients with migraine who experience cranial autonomic symptoms (CAS) during attacks may have a higher trigeminovascular activation than those without CAS. This study found that CAS was associated with a better response to anti-CGRP monoclonal antibody treatments.

Summary: The presence of CAS in migraine, usually associated with trigeminal autonomic cephalalgias, has also been reported in adult and pediatric populations. Experimental evidence suggests that increased parasympathetic outflow can enhance the sensitization of nociceptive receptors involved in migraine. Identifying and understanding these symptoms in migraine may be crucial for diagnosis and effective management.

Summary: This review focuses on the prevalence, pathophysiology, and clinical characteristics of cranial autonomic symptoms and neck pain in migraine, highlighting their importance in the differential diagnosis. Cranial autonomic symptoms, such as aural fullness, lacrimation, facial sweating, and conjunctival injection, are associated with more severe and frequent migraine attacks. Neck pain can act as a prodromal symptom or trigger for migraines and is correlated with treatment resistance and disability.

Summary: This study investigated the relationship between cranial autonomic symptoms (CASs) and central sensitization in migraine patients. The results showed that migraine patients with CASs had significantly greater central sensitization, and cranial parasympathetic symptoms were more frequent in centrally sensitized patients.

Summary: This study found that cranial autonomic symptoms (CAS) are common in migraine patients and are associated with increased headache frequency and lateralization of headaches. Voice change and throat swelling may be parasympathetically-mediated CAS and may be associated with aural fullness.

Summary: In this study, the researchers investigated the effects of PACAP and VIP on plasma levels of VIP and their contribution to induced cluster headache attacks. The results showed a significant increase in plasma VIP levels during PACAP infusion in individuals with episodic cluster headache, but not in those with chronic cluster headache. Further studies are needed to understand the role of VIP and the parasympathetic system in cluster headache.

Summary: The study found that plasma levels of PACAP-38 and vasoactive intestinal peptide were higher in pediatric migraine patients compared to controls, while levels of calcitonin gene-related peptide and substance P remained unchanged. No significant difference in neuropeptide levels was found between attack and attack-free periods.

Summary: Our study evaluated the efficacy and tolerability of erenumab in managing chronic migraine in a tertiary care setting. The results showed significant reduction in monthly migraine days and overall improvement in other variables at 3 months. More than half of the patients experienced a clinically meaningful improvement in migraine days. No serious adverse events were reported.

Summary: This study investigates the role of PACAP-38 in modulating cranial-autonomic and trigeminovascular responses related to headaches. The findings suggest that PACAP-38 regulates headache-related cranial-autonomic and trigeminal-vascular responses via peripheral and central components of the cranial parasympathetic projection.

Summary: Eptinezumab administered during a migraine attack resulted in early separation from placebo in terms of headache pain freedom/relief, absence of most bothersome symptom, absence of photophobia, phonophobia, and nausea. This suggests a potential peripheral pharmacological action for CGRP blockade.

Summary: This study investigates the feasibility of using the overlap coefficient (OVL) in binary classification problems. The findings indicate that the OVL does not provide additional information and the previously reported advantages are based on the assumption that larger values of the marker are assigned higher probabilities in the classification process. By examining the ability of white blood cell counts to identify different types of meningitis, the problem is further illustrated.

Summary: The Cox regression model and its associated hazard ratio (HR) are widely used for summarizing the effect of treatments on time to event outcomes. However, interpreting the HR depends on the assumed underlying survival model, which poses challenges. Alternative measures have been proposed to address these concerns. This study examines the properties of a specific marginal Cox regression model and considers its estimation when there is a omitted confounder.

Summary: Combat sports athletes exhibit differences in body weight, body composition, and adiposity. This study found that adiposity levels can influence the agreement between different methods used to estimate body fat. However, the agreement between methods and body fat percentage was not affected by adiposity. Anthropometry was the most accurate method.

Summary: This study compared the effectiveness of transcarotid artery revascularization (TCAR), carotid endarterectomy (CEA), and transfemoral carotid artery stenting (TF-CAS) in patients with carotid artery stenosis. The results showed that TCAR had a higher rate of stroke or death during the perioperative period, but there was no statistically significant difference in stroke or death between the three procedures at 1 year. TCAR may be a safe alternative to CEA and TF-CAS, especially for symptomatic patients.

Summary: The generalized receiver-operating characteristic (gROC) curve is used to evaluate diagnostic tests that have different associations with positive results for higher and lower marker values. However, using the same data to select classification subsets and calculate the gROC curve leads to over-optimistic estimates of future performance. This study explores the bias of the empirical gROC curve estimator and proposes cross-validation based algorithms to reduce overfitting, improving the estimation of diagnostic accuracy.

Summary: A good diagnostic test should exhibit distinct behavior for positive and negative populations, but that is not sufficient to create a good classification system. Defining decision criteria is crucial for solving the complex binary classification problem. The receiver-operating characteristic curve length serves as an index for the optimal discriminatory capacity of a biomarker, but its estimation requires parametric or smoothed models. This study examines a kernel density estimator-based approximation for estimating the length of the receiver-operating characteristic curve and explores its behavior through statistical analysis and Monte Carlo simulations with real-world examples.

Summary: In 2015, TCAR was approved by the FDA as a treatment option for high-risk patients with carotid stenosis. This study shows an increasing trend in TCAR utilization, with TCAR representing nearly 1-in-4 procedures at centers offering it. The results suggest the need for high-quality registry-based analyses to evaluate TCAR's real-world outcomes and durability.

Summary: The current guidelines recommend invasive staging for non-small cell lung cancer (NSCLC) with endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). However, the indication for confirming a negative EBUS-TBNA result with video-assisted mediastinoscopy (VAM) before resection varies in different guidelines. This study evaluates the evidence for the added value of confirmatory VAM after a negative EBUS-TBNA result in NSCLC mediastinal staging, and finds that confirmatory VAM can reduce the rate of unforeseen N2/3 disease, but with a high number needed to treat (NNT), and should be recommended only for specific cases yet to be defined.
