Summary: The purpose of this review is to analyze and study the psychological factors that influence the experience and treatments for diabetic neuropathy (DN) pain. It found that diagnosable mental health conditions and adverse thinking processes have significant impacts on DN pain, and psychotherapeutic modalities can have beneficial effects on pain.

Summary: This study reveals a new mechanism by which psychological stress stimulates cancer progression through the D2 dopamine receptor and activation of the oxygen-independent HIF1a pathway, highlighting the potential of dopamine and its receptor as a therapeutic target, and emphasizing the need to consider psychological stress as a modulator of cancer progression.

Summary: The objective of this study was to investigate the presence of nonbladder sensory abnormalities in participants with overactive bladder syndrome (OAB). The results showed no significant differences in experimental nonbladder pain or auditory sensitivity between OAB cases and controls during quantitative sensory testing (QST). However, there were associations between QST outcomes and clinical pain, suggesting the possibility of centrally mediated sensory amplification in some individuals with OAB.

Summary: The study found that isometric handgrip exercise with or without blood flow restriction can induce exercise-induced hypoalgesia at both local and non-local sites. However, discomfort and changes in systolic blood pressure do not explain this response.

Summary: Several reports indicate an association between psychiatric disorders and altered pain sensitivity. The pain threshold is generally reduced in patients with major depression, while lower pain sensitivity is observed in individuals with schizophrenia. However, limited studies on posttraumatic stress disorder, bipolar disorder, and anxiety disorders prevent clear conclusions. It is important to be aware of these pain perception abnormalities in psychiatric patients to improve their overall well-being.

Summary: The Pain Sensitivity Questionnaire (PSQ) is a reliable tool for assessing pain sensitivity. The Spanish version of the PSQ (PSQ-S) was validated in this study, demonstrating excellent internal consistency and reliability. The PSQ-S showed strong correlations with experimental pain rating parameters and psychological variables. It also successfully distinguished between healthy adults and adults with chronic pain. Therefore, the PSQ-S is a simple alternative to experimental pain procedures for clinical and experimental pain research.

Summary: Depletion of HIRA enhances proliferation and invasion of Fh1-deficient cells by activating MYC and its target genes, leading to increased nucleotide metabolism specifically in Fh1-deficient cells. This finding is instrumental for understanding the mechanisms of tumorigenesis in HLRCC and developing targeted treatments for patients.

Summary: The study investigates the relationships between self-reported temporomandibular disorder (TMD) symptoms, psychological well-being (PWB), psychological distress (PD), and oral health-related quality of life (OHRQoL). Participants with more TMD symptoms tend to have lower scores in PWB, higher levels of PD, and poorer OHRQoL compared to those with fewer or no symptoms.

Summary: The author, originally trained as a gastroenterologist in the UK, switched to lipidology and conducted research on familial hypercholesterolemia during his time as a research fellow in the USA. His studies revealed that FH patients suffer from LDL receptor dysfunction, requiring lipoprotein apheresis treatment. Further research into this genetic metabolic disorder can improve the prognosis of patients.

Summary: A single bout of exercise can reduce pain sensitivity in healthy individuals, but exercise-induced hypoalgesia is often impaired in individuals with chronic pain. Existing literature suggests that training interventions may induce hypoalgesic adaptations, potentially driven by central nervous system and immune system factors, but further research is needed to understand the underlying mechanisms.

Summary: This study aimed to explore the prevalence of clinically significant anxiety and depression in adult patients with chronic orofacial pain (COFP) conditions. The results showed that clinically significant anxiety and depression were commonly observed in patients with COFP, with higher rates than in pain-free participants. The prevalence rates were closely linked to pain severity and the presence of comorbid conditions such as migraines or widespread pain.

Summary: This study compared the level of lubricity and pain reduction of a novel shea lubricant to 2% lidocaine gel during digital rectal examination (DRE). The results showed that the shea lubricant achieved similar level of pain reduction as the lidocaine gel, but had better lubricity and ease of use.

Summary: This article systematically reviews claustrum lesion studies and discusses their functional implications. Claustral lesions are associated with a wide range of symptoms, but do not provide sufficient evidence to support current theories of claustrum function. Rather, lesion studies support the hypothesis that the claustrum regulates cortical excitability. It is suggested that the claustrum is connected to multiple brain networks and performs fundamental and higher cognitive functions.

Summary: The term refractory GERD is not as clear-cut as it may seem, with distinctions needed between different types of symptoms and objective evidence of GERD. Objective evaluation, including endoscopy and esophageal physiologic testing, is essential for understanding mechanisms of symptom generation and true refractory GERD. Attention should also be paid to potential contributors such as supragastric belching and rumination syndrome.

Summary: Distal esophageal spasm (DES) is characterized by premature contractions in the esophagus, often causing dysphagia or non-cardiac chest pain. Treatments for DES can be challenging, with endoscopic options like botulinum toxin and peroral endoscopic myotomy currently being evaluated. Further research is needed to understand the role of contractile vigor and lower esophageal sphincter hypercontractility in DES symptoms.

Summary: Increased SGB, commonly seen in patients with belching as the main symptom, is also observed in patients with refractory reflux symptoms or reflux hypersensitivity despite PPI treatment. Detection of increased SGB through impedance-pHmetry analysis can provide a better understanding of the pathophysiology of symptoms and guide more focused treatment. This study demonstrates that behavioral therapy is superior to no intervention in treating patients with SGB.

Summary: Reflux hypersensitivity (RH) is a phenotype of gastroesophageal reflux disease characterized by typical reflux symptoms and positive reflux-symptom association despite normal acid exposure. The Rome IV criteria and Lyon consensus provided a clearer understanding of the pathophysiology and importance of esophageal hypersensitivity. RH often overlaps with other functional gastrointestinal disorders. Despite identified pathophysiological mechanisms, there is still a need for improved treatment options. This review provides a summary of current knowledge on RH. (Gut Liver, Published online January 2, 2023)

Summary: Supragastric belching is a common behavioral disorder associated with esophageal motility impairment and increased acid exposure. The impact of SGB on reflux is proportional to its severity. Diagnosis of SGB should be considered in patients with excessive belching and refractory reflux symptoms, and behavioral therapy can alleviate acid exposure and improve quality of life.

The implementation of a strict RH definition (both SI and SAP positive) can better discriminate RH from functional heartburn (FH) and should be used in the future.
