Management of Persistent Pneumothorax With Thoracoscopy and Bleb Resection in COVID-19 Patients

Several studies have been published describing the clinical and radiographic findings of coronavirus disease 2019-related pneumonia. Therefore, there is currently a lack of pathologic data on its effects in intubated patients. Pneumothorax may occur rarely and results from a combination of fibrotic parenchyma and prolonged high-pressure ventilation. Chest drainage represents first-line treatment. However, in cases of persistent pneumothorax, thoracoscopy and bleb resection may be feasible options to reduce air leak and improve ventilation. This report describes the cases of 2 patients with coronavirus disease 2019 who were successfully treated with thoracoscopy, bleb resection, and pleurectomy for persistent pneumothorax. Summary: In patients with severe symptomatic AS undergoing TAVR, the newly available BE Myval THV demonstrated comparable early clinical performance and safety to the commonly used SE Evolut R THV. It was associated with a lower rate of permanent pacemaker implantation and more than moderate paravalvular leak within 30 days and 6 months after the procedure.

Transcatheter edge-to-edge mitral valve repair for post-myocardial infarction papillary muscle rupture and acute heart failure: A systematic review

Summary: Papillary muscle rupture (PMR) is a rare but serious complication of acute myocardial infarction (AMI) with high mortality and morbidity. Surgery, the standard treatment, is associated with high perioperative risks due to unstable hemodynamics. However, percutaneous mitral transcatheter edge-to-edge repair (M-TEER) has been found to be safe and effective in reducing mitral regurgitation in post-AMI PMR patients, with low in-hospital mortality and complications.

Summary: Complex ascending and aortic arch surgery requires different cerebral protection strategies, including deep or moderate hypothermia and various cerebral perfusion techniques, to prevent brain damage. The etiology of the damage involves multiple factors, such as cerebral embolism, hypoperfusion, hypoxia, and inflammatory response. This narrative review describes the pathophysiology of cerebral damage during aortic surgery, analyzes the options for brain protection, and discusses the current systems of intraoperative brain monitoring.

Summary: This study explores the opportunities and limitations of using artificial intelligence (AI) and big data in the field of heart transplantation. The results show that AI performs well in predicting prognosis and diagnosing heart transplantation, but there are risks of bias, lack of external validation, and limited applicability. Therefore, more unbiased research and high-quality data are needed to support the use of medical AI in clinical decision-making.
