Summary: Esophageal adenocarcinoma (EAC) is increasing rapidly and has a poor prognosis, often preceded by Barrett's esophagus (BE). This review provides an overview of BE, EAC, and the progression from BE to EAC. The definition, diagnosis, epidemiology, and risk factors for both conditions are discussed, with special attention given to areas of debate. The progression from BE to EAC is likely driven by inflammatory pathways, pepsin exposure, upregulation of growth factor pathways, and mitochondrial changes. Surveillance is conducted through serial endoscopic evaluation, with shorter intervals recommended for high-risk features.

Summary: This article reports a battery-free and deformable electronic esophageal stent for wireless stimulation of the lower esophageal sphincter without the need for open surgery. The stent is capable of adapting to the dynamic environment of the esophagus and wirelessly harvesting energy through deep tissue. In vivo experiments using pig models demonstrated that continuous electrical stimulations delivered by the stent significantly increased the pressure of the lower esophageal sphincter.

Summary: Both obesity and esophageal adenocarcinoma (EAC) rates have seen a significant increase in recent years in the United States and Western Europe. Obesity plays a key role in the development of EAC, with increased body mass index correlating with higher risk. Pathologically altered visceral fat in obesity is believed to contribute to this process. Visceral obesity may promote EAC through direct effects on gastroesophageal reflux disease and Barrett's esophagus (BE), as well as through the release of pro-inflammatory adipokines and insulin resistance. Disrupted adipokine production, such as the imbalance of leptin and adiponectin, is implicated in the pathogenesis of BE and EAC. This review discusses the epidemiology, pathophysiology, and molecular alterations associated with obesity in BE and EAC, with a focus on the potential role of adipokines and myokines. The review also highlights the possible crosstalk of adipokines and myokines during exercise and suggests lifestyle interventions, specifically regular physical activity, as a promising strategy for preventing the development of BE and EAC.

Summary: This study comprehensively evaluated the efficacy of various endoscopic and surgical treatments for gastroesophageal reflux disease (GERD). The results showed that endoscopic plication, reinforcement of the lower esophageal sphincter (LES), and surgical fundoplication were effective in reducing the requirement of proton pump inhibitor (PPI) continuation compared to PPI therapy. Radiofrequency energy delivery also tended to reduce the requirement of PPI continuation. All endoscopic and surgical treatments were superior to PPI therapy in terms of GERD-health-related quality of life questionnaire score (GERD-HRQL).

Summary: MicroRNAs (miRNAs) are small endogenous RNA molecules that regulate mRNA targets through RNA interference. They play a crucial role in physiological and pathological processes including cell proliferation, differentiation, apoptosis, and inflammation. This review discusses the potential of miRNAs in overall esophageal disease, focusing on eosinophilic esophagitis (EoE), gastroesophageal reflux disease (GERD), and the progression from Barrett's esophagus (BE) to esophageal adenocarcinoma (EAC). The dysregulation of miRNA expression in these diseases highlights their key role in disease progression and suggests their potential as diagnostic and prognostic biomarkers. Furthermore, targeting miRNAs could be a therapeutic option for esophageal disease.

An APAGE working group developed clinical practice guidelines for management of patients with functional dyspepsia (FD) who have overlaps with other functional gastrointestinal disorders. They identified putative pathophysiology and provided a management algorithm for primary and secondary care clinicians.

Summary: This study aimed to evaluate the prevalence of gastrointestinal symptoms and post-COVID-19 disorders of gut-brain interaction among hospitalized patients with SARS-CoV-2 infection. The results showed that patients with COVID-19 had higher rates of gastrointestinal symptoms on hospital admission and lower rates of constipation and hard stools at 12 months post-infection compared to the control group. Additionally, the rate of irritable bowel syndrome was significantly higher in patients with COVID-19 than in controls.

Summary: By comparing IBS patients from the UK and Malaysia, it was found that UK patients were more likely to consume alcohol, had more frequent symptoms such as pain, bloating, and meal-related issues, higher symptom scores, greater limitation of activities, and a higher likelihood of reporting abnormal anxiety. UK patients also had higher levels of perceived stress, gastrointestinal symptom-specific anxiety, and somatoform symptom-reporting.

Summary: There are misconceptions in Malaysian clinical practice regarding the appropriate diagnostic method and treatment strategies for Helicobacter pylori infection. To address this, an expert panel of nine key opinion leaders in gastroenterology developed a set of consensus statements relevant to the Malaysian healthcare practice. The panel achieved consensus in three areas of H. pylori infection management: indications for testing, diagnosis, and treatment. They proposed 19 consensus statements, which can serve as a guide for physicians across various healthcare settings in Malaysia.

It reveals that inappropriate regimens containing antibiotics with high resistance rates are not uncommon, especially among non-gastroenterologist physicians. Furthermore, there are significant issues with the conventional first-line treatment in countries with high clarithromycin resistance.

The study will follow the JBI methodology for scoping reviews and is expected to start in July/August 2023 and end in June 2024. The review will include quantitative, qualitative, primary and secondary literature, as well as grey literature on decision regret in patients with NCDs. The results will be presented through tables, figures, and narrative summaries.
