Estimation of risk for major bleeding in native kidney biopsies in patients with multiple risk factors

Cynthia C. Lim, Ru Yu Tan, Jason C. J. Choo, Hui Zhuan Tan, Irene Mok, Yok Mooi Chin, Chieh Suai Tan

Summary: The study aimed to develop a clinical risk model to quantify bleeding risks in high-risk individuals undergoing kidney biopsy. Results showed that higher hemoglobin and platelet levels were independently associated with reduced major bleeding, and a risk model including multiple risk factors could predict major bleeding post-biopsy.

Influenza Vaccination in Systemic Lupus Erythematosus: Efficacy, Effectiveness, Safety, Utilization, and Barriers

Jackie Jia Lin Sim, Cynthia Ciwei Lim

Summary: Influenza increases morbidity and mortality in patients with systemic lupus erythematosus (SLE) and lupus nephritis, but vaccination is effective in preventing infection. Studies show that vaccination reduces influenza infection in SLE patients, with no significant changes in disease activity scores after vaccination. However, vaccination rates are relatively low, and barriers include lack of doctor recommendation and concerns over vaccine safety and efficacy.

Non-Steroidal Anti-Inflammatory Drugs and Risk of Acute Kidney Injury and Hyperkalemia in Older Adults: A Retrospective Cohort Study and External Validation of a Clinical Risk Model

Cynthia Ciwei Lim, Ngiap Chuan Tan, Edmund Pek Siang Teo, Hanis Bte Abdul Kadir, Jia Liang Kwek, Yong Mong Bee, Andrew Teck Wee Ang, Su Hooi Teo, Manish Kaushik, Chieh Suai Tan, Jason Chon Jun Choo

Summary: NSAID use in older adults can lead to acute renal events, especially with systemic and long-term usage. Risk factors such as diabetes, cardiovascular disease, decreased estimated glomerular filtration rate, and diuretic use also contribute to the incidence of acute kidney injury and/or hyperkalemia. The current risk prediction model for NSAID use in older adults needs further improvement.

COVID-19 vaccination in systemic lupus erythematosus: a systematic review of its effectiveness, immunogenicity, flares and acceptance

Shaun Ye Song Tan, Aaron Mingyang Yee, Jackie Jia Lin Sim, Cynthia Ciwei Lim

Summary: This study aimed to assess the effectiveness, efficacy, acceptance, and safety of COVID-19 vaccination in SLE patients. The study found that post-vaccine COVID-19 infections, severe flares, and adverse events were infrequent in SLE patients, while seropositivity and acceptance were high.

Deep learning algorithms to detect diabetic kidney disease from retinal photographs in multiethnic populations with diabetes

Bjorn Kaijun Betzler, Evelyn Yi Lyn Chee, Feng He, Cynthia Ciwei Lim, Jinyi Ho, Haslina Hamzah, Ngiap Chuan Tan, Gerald Liew, Gareth J. Mckay, Ruth E. Hogg, Ian S. Young, Ching-Yu Cheng, Su Chi Lim, Aaron Y. Lee, Tien Yin Wong, Mong Li Lee, Wynne Hsu, Gavin Siew Wei Tan, Charumathi Sabanayagam

Summary: The objective of this study is to develop a deep learning algorithm to detect diabetic kidney disease from retinal photographs of patients with diabetes, and evaluate its performance in multiethnic populations. Three models were trained and tested on different datasets to assess their accuracy in detecting the disease. The results suggest that the algorithm has the potential to be used as a screening tool for diabetic kidney disease.

A deep learning system for retinal vessel calibre improves cardiovascular risk prediction in Asians with chronic kidney disease

Cynthia Ciwei Lim, Crystal Chong, Gavin Tan, Chieh Suai Tan, Carol Y. Cheung, Tien Y. Wong, Ching Yu Cheng, Charumathi Sabanayagam

Summary: This study found that retinal vessel calibre measurements obtained by a deep learning system were significantly associated with incident cardiovascular disease (CVD) in chronic kidney disease (CKD) patients. The addition of kidney function and retinal vessel calibre parameters improved the prediction of CVD risk among CKD patients.

Early repeat hospitalization for fluid overload in individuals with cardiovascular disease and risks: a retrospective cohort study

Cynthia C. Lim, Dorothy Huang, Zhihua Huang, Li Choo Ng, Ngiap Chuan Tan, Wei Yi Tay, Yong Mong Bee, Andrew Ang, Chieh Suai Tan

Summary: This study evaluated the incidence and risk factors associated with early repeat hospitalization for fluid overload among patients with cardiovascular disease and risks. The results showed that cardiovascular disease, recent hospitalization for fluid overload, recent hospitalization for any cause, and intravenous furosemide use were associated with an increased risk of early repeat hospitalization. Conversely, higher systolic blood pressure on admission and diuretic use at discharge reduced the risk of early hospitalization for fluid overload.

Summary: Among adults with chronic kidney disease (CKD), both systemic and topical non-steroidal anti-inflammatory drugs (NSAIDs) are independently associated with adverse acute kidney outcomes.
