Summary: Proteinuria is a risk factor for the progression of diabetic nephropathy and an imbalance in podocyte function exacerbates proteinuria. Celastrol, the active ingredient of T. wilfordii, has been effective in treating renal injury in diabetic nephropathy, but its mechanisms are unclear. This study investigated how celastrol prevents podocyte damage in diabetic nephropathy using in vivo and in vitro experiments. Celastrol reduced blood glucose levels, attenuated renal histomorphological injury, and induced anti-inflammatory effects. It upregulated SIRT1 expression and downregulated EZH2, inhibiting the wnt/beta-catenin pathway. SIRT1 repression aggravated podocyte injury, while celastrol protected podocyte injury via SIRT1/EZH2 in the wnt/beta-catenin pathway.

Summary: In this study, the researchers investigated the role of MAP4 phosphorylation in causing proteinuria in diabetic nephropathy (DN). They found that the content of p-MAP4 was increased in the urine samples of diabetic patients and in the kidney tissues of diabetic mice. p-MAP4 triggered proteinuria by inducing epithelial-to-mesenchymal transition and apoptosis in podocytes. The study also showed that mice with MAP4 hyperphosphorylation were more susceptible to a certain diabetes treatment and exhibited more severe DN pathology compared to wild-type mice. In vitro experiments further revealed that high glucose triggered the elevation of p-MAP4 and various pathological changes in podocytes, which were rectified by MAP4 dephosphorylation. The findings suggest that p-MAP4 plays a novel role in causing proteinuria in DN and indicate the therapeutic potential of MAP4 in protecting against proteinuria and related diseases.

Summary: The research found that Pueraria lobata extracts containing a total of 73 identified compounds could ameliorate kidney injury and regulate abnormal glycolipid metabolism. In metabolomics analysis, 19 carbonyl compounds with significant differences were identified between DN mice and normal mice, while 12 metabolites had a tendency to return to normal levels after PL treatment. Overall, Pueraria lobata exerts beneficial effects on DN by regulating abnormal glycolipid metabolism and carbonyl stress, and endogenous carbonyl compounds might serve as potential biomarkers for DN.

Summary: This study investigated the protective effect and mechanism of astragaloside IV (AS-IV) on damage in human glomerular endothelial cells (GEnCs) stimulated by high glucose and high insulin. The results showed that AS-IV had a significant protective effect on the cell barrier of GEnCs. It increased cell energy metabolism and viability, inhibited inflammation and oxidative stress damage, and enhanced tight junction between cells.
