First-in-human robotic supermicrosurgery using a dedicated microsurgical robot for treating breast cancer-related lymphedema: a randomized pilot trial

Advancements in reconstructive microsurgery have evolved into supermicrosurgery; connecting vessels with diameter between 0.3 and 0.8mm for reconstruction of lymphatic flow and vascularized tissue transplantation. Supermicrosurgery is limited by the precision and dexterity of the surgeon's hands. Robot assistance can help overcome these human limitations, thereby enabling a breakthrough in supermicrosurgery. We report the first-in-human study of robot-assisted supermicrosurgery using a dedicated microsurgical robotic platform. A prospective randomized pilot study is conducted comparing robot-assisted and manual supermicrosurgical lymphatico-venous anastomosis (LVA) in treating breast cancer-related lymphedema. We evaluate patient outcome at 1 and 3 months post surgery, duration of the surgery, and quality of the anastomosis. At 3 months, patient outcome improves. Furthermore, a steep decline in duration of time required to complete the anastomosis is observed in the robot-assisted group (33-16min). Here, we report the feasibility of robot-assisted supermicrosurgical anastomosis in LVA, indicating promising results for the future of reconstructive supermicrosurgery.

Rebubbling and graft failure in Descemet membrane endothelial keratoplasty: a prospective Dutch registry study

Summary: This Dutch registry study identified risk factors for rebubbling and early graft failure in DMEK. Rebubbling was associated with higher graft failure, while older recipient age and surgical complications were risk factors for rebubbling. Early graft failure risk factors included transplantation before 2016 and surgical complications. Rebubbling was also associated with significantly lower endothelial cell densities.

Donor Site Satisfaction Following Autologous Fat Transfer for Total Breast Reconstruction

Summary: This study aimed to evaluate donor site satisfaction following autologous fat transfer (AFT) for total breast reconstruction in breast cancer patients. The results showed no significant difference in body satisfaction between AFT and implant-based reconstruction, indicating that large-volume liposuction does not aesthetically affect the utilized donor sites. However, reconstructive surgeons should be aware of possible donor site complications, especially contour irregularities at the thighs, and discuss this with their patients.

Bilateral breast infection following total breast reconstruction with autologous fat transfer (AFT): A case report

Summary: In general, total breast reconstruction with autologous fat transfer has a low complication rate. However, infections remain a common complication. This case report presents a severe bilateral breast infection following the surgery, which was successfully treated with surgery and antibiotics. Early surgical prophylaxis and antibiotic treatment can reduce the incidence of breast infections, and identifying and managing infection risks can be improved through monitoring the fit of devices and implementing video consultations.

Effects of School-Based Health-Promoting Activities on Children's Health: A Pragmatic Real-World Study

Summary: Understanding the health effects of scaled-up school-based interventions is crucial for integrating health in schools. This study examined the effectiveness of the Healthy Primary School of the Future (HPSF) initiative in real-world settings. Due to COVID-19 limitations, implementation of HPSF activities was restricted. Results showed that waist circumference in the medium implementer group significantly increased at one-year and two-year follow-up compared to the low implementer group. No significant effects were observed for other outcomes.

The results show that AFT has higher quality-adjusted life years (QALY) and costs in the first year postoperative. However, due to the absence of additional surgeries, AFT is estimated to be more cost-effective over the 10- and 30-year period.
