Current Pharmacological Treatment of Painful Diabetic Neuropathy: A Narrative Review

C Background and Objectives: Distal symmetrical polyneuropathy (DSPN) is one of the most common chronic complications of diabetes mellitus. Although it is usually characterized by progressive sensory loss, some patients may develop chronic pain. Assessment of DSPN is not difficult, but the biggest challenge is making the correct diagnosis and choosing the right treatment. The treatment of DSPN has three primary objectives: glycemic control, pathogenic mechanisms, and pain management. The aim of this brief narrative review is to summarize the current pharmacological treatment of painful DSPN. It also summarizes knowledge on pathogenesis-oriented therapy, which is generally overlooked in many publications and guidelines. Materials and Methods: The present review reports the relevant information available on DSPN treatment. The search was performed on PubMed, Cochrane, Semantic Scholar, Medline, Scopus, and Cochrane Library databases, including among others the terms distal symmetrical polyneuropathy, neuropathic pain treatment, diabetic neuropathy, diabetes complications, glycaemic control, antidepressants, opioids, and anticonvulsants. Results: First-line drugs include antidepressants (selective serotonin reuptake inhibitors and tricyclic antidepressants) and pregabalin. Second- and third-line drugs include opioids and topical analgesics. While potentially effective in the treatment of neuropathic pain, opioids are not considered to be the first choice because of adverse reactions and addiction concerns. Conclusions: DSPN is a common complication in patients with diabetes, and severely affects the quality of life of these patients. Although multiple therapies are available, the guidelines and recommendations regarding the treatment of diabetic neuropathy have failed to offer a unitary consensus, which often hinders the therapeutic options in clinical practice.

Summary: There is bidirectional relation between COVID-19 and T2DM and effective ways are needed to counter the glucometabolic disturbances during acute COVID-19 illness in T2DM patients. Novel antidiabetic medications are considered candidate drugs for tackling these disturbances.

Summary: The incidence of isolated iliac artery aneurysms is about 2%, while common iliac artery pseudoaneurysms are even rarer. A pseudoaneurysm is a localized hemorrhage rather than a true aneurysm, which affects the entire vascular wall. They are usually asymptomatic and can be detected accidentally. If large and symptomatic, they may cause pressure symptoms and various complications. We report two cases of common iliac artery pseudoaneurysm, one treated with endovascular repair and the other with open surgery.

Clinical and Biological Data in Patients with Pancreatic Cancer vs. Chronic Pancreatitis-A Single Center Comparative Analysis

Summary: The objective of this study was to identify clinical and paraclinical data with statistical significance in the differential diagnosis between chronic pancreatitis and pancreatic cancer. The results showed that female gender, advanced age, elevated aminotransferases, cholestasis markers, and uric acid were associated with a higher probability of pancreatic cancer.

Primary Arterial Hypertension and Drug-Induced Hypertension in Philadelphia-Negative Classical Myeloproliferative Neoplasms: A Systematic Review

Summary: This study systematically reviewed the literature on the impact of primary arterial hypertension (HTN) in myeloproliferative neoplasms (MPNs). The results showed that HTN is the most common comorbidity encountered in MPNs, with effects on thrombosis occurrence and patient survival. Drug-induced HTN also poses challenges in MPNs management. Further research is needed to investigate the characteristics of MPNs patients with HTN, and the contribution of HTN to thrombotic complications, survival, and management in MPNs.

Advances in the Pharmacological Management of Diabetic Nephropathy: A 2022 International Update

Summary: Diabetic nephropathy is the leading cause of end-stage renal disease globally, and its pathogenesis involves increased pressure, activity of the renin-angiotensin system, and oxidative stress. Current treatment options focus on optimizing blood pressure and blood glucose targets, but new drug classes such as GLP-1 RAs, SGLT-2is, and inhibitors of mineralocorticoid receptor-mediated sodium reabsorption show positive effects on kidney function in individuals with type 2 diabetes mellitus. This review aims to provide an update on therapeutic options for preventing or slowing the onset and progression of diabetic nephropathy.

Risk phenotypes of diabetes and association with COVID-19 severity and death: an update of a living systematic review and meta-analysis

Summary: This study aimed to provide a systematic overview of the current evidence on high-risk phenotypes of diabetes associated with COVID-19 severity and death. Observational studies investigating phenotypes in individuals with diabetes and confirmed SARS-CoV-2 infection were included. The findings strengthened the associations between male sex, older age, blood glucose level at admission, chronic insulin use, chronic metformin use, and pre-existing comorbidities and COVID-19-related death, and identified new factors such as obesity, HbA(1c) level, chronic glucagon-like peptide-1 receptor agonist use, and pre-existing heart failure associated with COVID-19-related death.

Red Yeast Rice for the Improvement of Lipid Profiles in Mild-to-Moderate Hypercholesterolemia: A Narrative Review

Summary: Reducing low-density lipoprotein cholesterol (LDL-C) levels is important for cardiovascular health. Red yeast rice (RYR) is a popular dietary supplement that can lower LDL-C levels by about 15-34%. It contains monacolins, such as monacolin K, which is similar to statins in its cholesterol-lowering effect. RYR can also reduce the risk of atherosclerotic cardiovascular disease events by up to 45%. It is well tolerated and can be a treatment option for those with mild-to-moderate hypercholesterolemia who are not eligible for statin therapy or unwilling to take medication.

Summary: Myeloproliferative neoplasms (MPNs) are clonal disorders characterized by excessive production of mature myeloid cells. Classical MPNs, such as polycythemia vera, essential thrombocythemia, and primary myelofibrosis, have a tendency to develop thrombotic complications at unusual sites. The pathogenesis of thrombotic events in MPNs is complex and involves endothelial injury, leukocyte adhesion, genetic mutations, and other factors. In this review, we discuss the epidemiology, pathogenesis, histopathology, risk factors, classification, clinical presentation, diagnosis, and management of Budd-Chiari syndrome in Philadelphia-negative MPNs.

Summary: Inflammatory bowel disease (IBD) is a chronic condition with high disability. One third of IBD patients have abnormal liver biochemical tests and 5% have chronic liver disease. Various liver diseases, including primary sclerosing cholangitis, can be associated with IBD. The progress in IBD treatment has not eliminated the risk of drug-induced liver disease and reactivation of viral hepatitis.
