Research Progress on the Role of Inflammasomes in Kidney Disease

Inflammasomes are multimeric complexes composed of cytoplasmic sensors, apoptosis-associated speck-like protein containing a caspase activation and recruitment domain (ASC or PYCARD), and procaspase-1 and play roles in regulating caspase-dependent inflammation and cell death. Inflammasomes are assembled by sensing pathogen-associated molecular patterns (PAMPs) and damage-associated molecular patterns (DAMPs) and initiate inflammatory responses by activating caspase-1. Activated caspase-1 promotes the release of the inflammatory cytokines interleukin-1 beta (IL-1 beta) and IL-18 and eventually induces pyroptosis. Inflammasomes are closely related to kidney diseases. In particular, the NLRP3 (NACHT, LRR, and PYD domain-containing protein 3) inflammasome has been shown to cause acute and chronic kidney diseases by regulating canonical and noncanonical mechanisms of inflammation. Small-molecule inhibitors that target NLRP3 and other components of the inflammasome are potential options for the treatment of kidney-related diseases such as diabetic nephropathy. This article will focus on the research progress on inflammasomes and the key pathogenic roles of inflammasomes in the development and progression of kidney diseases and explore the potential of this intracellular inflammation to further prevent or block the development of the kidney disease.

Higher PEEP versus lower PEEP strategies for patients in ICU without acute respiratory distress syndrome: A systematic review and meta-analysis

Comparison of Two Predictive Models of Sepsis in Critically Ill Patients Based on the Combined Use of Inflammatory Markers

Summary: This study developed a nomogram model and a simple risk scoring model to predict sepsis in critically ill patients by combining five commonly available inflammatory markers. Although the prediction performance of the two models is comparable, the simple risk scoring model may be simpler and more practical for clinicians to identify potential sepsis in critically ill patients at an early stage and strategize treatments.

Prediction of 3-year risk of diabetic kidney disease using machine learning based on electronic medical records

Summary: This study constructed a 3-year diabetic kidney disease risk prediction model in patients with type 2 diabetes mellitus using machine learning and electronic medical records. The study found that older patients with high homocysteine levels, poor glycemic control, low serum albumin levels, low estimated glomerular filtration rate, and high bicarbonate levels had an increased risk of developing DKD over the next 3 years.

Role of NOD-Like Receptors in a Miniature Pig Model of Diabetic Renal Injuries

IGFBP5 promotes diabetic kidney disease progression by enhancing PFKFB3-mediated endothelial glycolysis

Summary: Renal inflammation is a critical characteristic of diabetic kidney disease (DKD), but effective treatments are limited. This study found that insulin-like growth factor-binding protein 5 (IGFBP5) is significantly increased in the kidneys of diabetic mice. Elimination of IGFBP5 alleviated kidney inflammation in DKD mice. Mechanistically, IGFBP5 increased glycolysis in endothelial cells through the activation of the transcription factor EGR1 and enhanced expression of PFKFB3, which in turn enhanced renal inflammation.

Efficacy and Safety of Daprodustat Vs rhEPO for Anemia in Patients With Chronic Kidney Disease: A Meta-Analysis and Trial Sequential Analysis

Summary: This meta-analysis investigated the efficacy and safety of daprodustat, a novel HIF-PHI, in the treatment of chronic kidney disease (CKD)-related anemia. The results showed that daprodustat was noninferior to rhEPO in correcting anemia in both nondialysis-dependent and dialysis-dependent CKD patients. It also had a better effect on optimizing iron metabolism in dialysis-dependent CKD patients. However, further research is needed to evaluate the therapeutic effect of daprodustat due to the limited number of included studies.

Association between admission serum phosphate and risk of acute kidney injury in critically ill patients with rhabdomyolysis: A retrospective study based on MIMIC- III

Summary: This study aimed to assess the independent association between serum phosphate levels at ICU admission and the subsequent risk of AKI in hospitalized patients. The results showed that elevated serum phosphate levels were associated with an increased risk of AKI.

Mesenchymal stem cells ameliorate cisplatin-induced acute kidney injury via let-7b-5p

Summary: This study demonstrated that aged mice are more susceptible to AKI injury and identified an important role for the upregulation of the senescence-associated DNA damage/p53 pathway in this susceptibility. MSC treatment can reduce the degree of DNA damage and apoptosis. Furthermore, downregulation of let-7b-5p may lead to increased renal AKI susceptibility, and the presence of let-7b-5p in MSC-derived Exos can alleviate tubular epithelial cell apoptosis by inhibiting p53.

Optimization strategies of mesenchymal stem cell-based therapy for acute kidney injury

Summary: Considering the high prevalence and lack of targeted pharmacological management, new therapeutic approaches for acute kidney injury (AKI) are urgently needed. Mesenchymal stem cells (MSCs) have shown promise in the treatment of AKI, but the clinical translation of MSCs-based therapies is hindered by poor retention and survival rates, as well as impaired paracrine ability. Various strategies, including local administration, three-dimensional culture, and preconditioning, have been proposed to address these challenges and improve the effectiveness of MSCs in AKI models.

Peritoneal Dialysis Care in Mainland China: Nationwide Survey

The study results have important implications for enhancing PD care in mainland China.

Characteristics of hospitalized elderly patients with CKD: a comparison between elderly and non-elderly CKD based on a multicenter cross-sectional study

Summary: A multicenter epidemiological survey was conducted to study chronic kidney disease (CKD) among hospitalized patients, aiming to compare the characteristics of elderly CKD with non-elderly CKD. The study found that diabetic nephropathy and hypertension-related renal impairment were the main causes of CKD among elderly patients, who had a higher burden of comorbidities and worse in-hospital outcomes compared to younger patients. The length of stay and hospitalization costs increased with the number of coexisting diseases in elderly CKD patients.
