The term embolic stroke of undetermined source (ESUS) was introduced in 2014 to describe patients with a nonlacunar ischemic stroke and no convincing etiology. The terms ESUS and cryptogenic stroke are not synonyms, as the latter also includes patients with multiple stroke etiologies or incomplete diagnostic work-up. ESUS involves approximately 17% of all ischemic stroke patients, and these patients are typically younger with mild strokes and an annual rate of stroke recurrence of 4% to 5%. It was hypothesized that oral anticoagulation may decrease the risk of stroke recurrence in ESUS, which was tested in 2 large randomized controlled trials: the NAVIGATE ESUS (Rivaroxaban Versus Aspirin in Secondary Prevention of Stroke and Prevention of Systemic Embolism in Patients With Recent Embolic Stroke of Undetermined Source) and the RE-SPECT ESUS (Dabigatran Etexilate for Secondary Stroke Prevention in Patients With Embolic Stroke of Undetermined Source). The present review discusses the trials of anticoagulation in patients with ESUS, suggests potential explanations for their neutral results, and highlights the rationale that supports ongoing and future research in this population aiming to reduce the associated risk for stroke recurrence. Interleukin-6 inhibitors reduce mortality in coronavirus disease-2019: An individual patient data meta-analysis from randomized controlled trials

Summary: This study found that soluble interleukin-2 receptor (sIL-2R) is a strong laboratory marker for predicting the development of severe COVID-19 outcomes. sIL-2R levels were significantly higher in patients with severe disease compared to those with moderate disease, and the dynamic changes of sIL-2R can reflect disease severity and predict the occurrence of severe respiratory failure and mortality.

Summary: The use of oral anticoagulation therapy in patients with heart failure and sinus rhythm can reduce the occurrence of stroke, but it also increases the risk of major bleeding. Rivaroxaban has a better effect in preventing stroke, but it is also associated with a higher risk of bleeding. There were no significant differences between the use of oral anticoagulants and the control group in terms of myocardial infarction, all-cause mortality, and hospitalization for heart failure.

Sinus rhythm restoration and improved outcomes in patients with acute ischemic stroke and in-hospital paroxysmal atrial fibrillation

Summary: This study found that early restoration of sinus rhythm is associated with improved survival and major adverse cardiovascular events (MACE) in patients with acute ischemic stroke and paroxysmal atrial fibrillation (PAF). Self-termination of PAF and medical conversion to sinus rhythm were both associated with lower mortality, stroke recurrence, and MACE risk.

The review proposes a personalized strategy for cardiac monitoring in CS patients, taking into account the likelihood of AF detection and high-risk PFO features.

Summary: This retrospective study evaluated the safety and outcomes of revascularization treatments in patients with acute ischemic stroke and COVID-19. COVID-19 patients had higher rates of intracranial bleeding complications and worse clinical outcomes after treatment compared to non-COVID-19 patients.

Risk of myocardial infarction and ischemic stroke in individuals with first-diagnosed paroxysmal vs. non-paroxysmal atrial fibrillation under anticoagulation

Summary: This study aimed to investigate whether there is a difference in the risk of cardiovascular events, including acute myocardial infarction and ischemic stroke, between individuals with first-diagnosed paroxysmal AF treated with anticoagulants and those with non-paroxysmal AF. The results showed that individuals with paroxysmal AF had a higher risk of acute myocardial infarction, while there was no significant association between the type of AF and risk of ischemic stroke.

Characteristics of intracranial plaque in patients with non-cardioembolic stroke and intracranial large vessel occlusion

Summary: This study used 3.0 T high-resolution MRI to investigate the characteristics of intracranial plaque proximal to large vessel occlusion (LVO) in stroke patients without major-risk cardioembolic source. The results showed that intracranial plaque proximal to LVO was more common on the ipsilateral side to stroke and had larger plaque burden, higher remodelling index, higher percentage of lipid-rich necrotic core, higher prevalence of discontinuity of plaque surface, fibrous cap rupture, intraplaque haemorrhage and complicated plaque. Logistic analysis revealed that remodelling index and plaque burden were positively associated with ischaemic stroke. In the subgroup with <50% stenotic plaque, higher plaque burden, remodelling index, percentage of lipid-rich necrotic core and presence of complicated plaque were more closely related to stroke.

Summary: The aim of this study was to assess the association between the presence of a right-to-left shunt (RLS) and neurological decompression sickness (NDCS) and asymptomatic brain lesions among otherwise healthy divers. The results showed that high-grade RLS is associated with a greater risk of NDCS, but the association with asymptomatic brain lesions is not statistically significant.
