Summary: The occurrence of bleeding gastrointestinal angioectasia in elderly patients with degenerative calcific aortic stenosis, known as Heyde's syndrome, is a challenging clinical scenario. Heyde's syndrome is mainly caused by the loss of high-molecular-weight von Willebrand factor (HMW vWF) multimers due to their fragmentation passing through the stenosed aortic valve, resulting in acquired von Willebrand syndrome type IIA. Aortic valve replacement has been shown to be a more effective management approach for cessation of recurrent episodes of gastrointestinal bleeding, and a high index of suspicion is needed when dealing with elderly patients with established aortic stenosis presenting with iron deficiency anemia or unclear gastrointestinal bleeding. Collaboration between different specialties is essential for appropriate management.

Summary: A systematic review was conducted to explore the clinical characteristics and surgical success of Heyde syndrome (HS) patients treated with transcatheter aortic valve implantation (TAVI). The review included 31 case reports of patients with aortic stenosis and gastrointestinal bleeding. TAVI was found to be an effective therapy, with a resolution rate of 88% for HS patients.

The prevalence and management of Heyde's syndrome remain controversial. Our review provides a unique case to highlight the diagnosis and management of Heyde's syndrome, as well as briefly describing the prevalence, etiology, diagnosis, and management of calcific aortic valve disease in general.

Summary: Heyde syndrome is a rare condition characterized by the co-occurrence of aortic stenosis and bleeding gastrointestinal angiodysplasias. This study describes two cases in which patients with persistent Heyde syndrome were successfully treated with systemic bevacizumab, leading to resolution of bleeding and normalization of hemoglobin levels. These findings suggest that targeting angiogenesis may be an effective therapeutic option for Heyde syndrome.

Summary: Heyde's syndrome is a disease where patients with aortic stenosis (AS) suffer from bleeding due to angiodysplasia. In this case, an 80-year-old woman with severe AS presented with melena and anemia. After multiple blood transfusions, the patient underwent jejunal resection and a pathological examination revealed angiodysplasia. Heyde's syndrome was diagnosed as the patient's plasma lacked high-molecular-weight von Willebrand factor (VWF) multimers. One year after the diagnosis, the patient underwent transcatheter aortic valve implantation (TAVI) and the VWF index recovered. This valuable case allowed for the pathological analysis of angiodysplasia associated with Heyde's syndrome.

Summary: This case study describes an 84-year-old woman with a history of ischemic heart disease, paroxysmal atrial fibrillation, and severe aortic stenosis. She was admitted to the hospital for an ST-elevation myocardial infarction caused by in-stent thrombosis and underwent primary percutaneous coronary intervention. After receiving loading doses of aspirin and clopidogrel, she developed rectal bleeding and melena, which were determined to be caused by angiodysplasia in the ileocecal valve. The patient underwent elective transcatheter aortic valve implantation and was discharged with a personalized antithrombotic therapy, with no further bleeding episodes reported in the following weeks.
