Mortality After Repeat Revascularization Following PCI or CABG for Left Main Disease The EXCEL Trial

OBJECTIVES The aim of this study was to investigate the incidence and impact on mortality of repeat revascularization after index percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) for left main coronary artery disease (LMCAD). BACKGROUND The impact on mortality of the need of repeat revascularization following PCI or CABG in patients with unprotected LMCAD is unknown. METHODS All patients with LMCAD and site-assessed low or intermediate SYNTAX (Synergy Between PCI With Taxus and Cardiac Surgery) scores randomized to PCI (n = 948) or CABG (n = 957) in the EXCEL (Evaluation of XIENCE Versus Coronary Artery Bypass Surgery for Effectiveness of Left Main Revascularization) trial were included. Repeat revascularization events were adjudicated by an independent clinical events committee. The effect of repeat revascularization on mortality through 3-year follow-up was examined in time-varying Cox regression models. RESULTS During 3-year follow-up, there were 346 repeat revascularization procedures among 185 patients. PCI was associated with higher rates of any repeat revascularization (12.9% vs. 7.6%; hazard ratio: 1.73; 95% confidence interval: 1.28 to 2.33; p = 0.0003). Need for repeat revascularization was independently associated with increased risk for 3-year all-cause mortality (adjusted hazard ratio: 2.05; 95% confidence interval: 1.13 to 3.70; p = 0.02) and cardiovascular mortality (adjusted hazard ratio: 4.22; 95% confidence interval: 2.10 to 8.48; p < 0.0001) consistently after both PCI and CABG (p(int) = 0.85 for both endpoints). Although target vessel revascularization and target lesion revascularization were both associated with an increased risk for mortality, target vessel non-target lesion revascularization and non-target vessel revascularization were not. CONCLUSIONS In the EXCEL trial, repeat revascularization during follow-up was performed less frequently after CABG than PCI and was associated with increased mortality after both procedures. Reducing the need for repeat revascularization may further improve long-term survival after percutaneous or surgical treatment of LMCAD. (C) 2020 by the American College of Cardiology Foundation.

Summary: This study aims to analyze the relationship between total stent length (TSL) and 10-year target-lesion failure (TLF) in patients with ST-elevation myocardial infarction (STEMI) treated with percutaneous coronary intervention. The results show that there is a direct relationship between TSL and the risk of TLF at 10 years, mainly driven by target lesion revascularization (TLR).

Summary: The aim of this substudy was to examine the influence of patient age on the long-term outcomes of ST-elevation myocardial infarction (STEMI). The study included 1,375 patients with STEMI and categorized them into four age groups. The results showed that patients younger than 55 years, patients aged 55 to 65 years, and patients aged 65 to 75 years had lower risks of patient-oriented composite end point (POCE) compared to those older than 75 years, primarily due to lower rates of all-cause death and cardiac death.

Summary: This study suggests that P2Y12 inhibitor monotherapy after complex percutaneous coronary intervention (PCI) can preserve ischemic protection while limiting bleeding risk. Compared with dual antiplatelet therapy (DAPT), P2Y12 inhibitor monotherapy after 1-month to 3-month DAPT can reduce the major bleeding risk in complex PCI patients.

Summary: Background estimation of regurgitant fraction using videodensitometry (VD-AR) is a new objective grading tool for paravalvular regurgitation (PVR) after transcatheter aortic valve implantation (TAVI). This study found that VD-AR > 17% was independently associated with 3-year mortality. Compared to subjective visual evaluation, VD-AR resulted in a smaller proportion of patients classified as prognostically relevant for PVR.

Summary: This article reports on the use of CCTA to non-invasively risk stratify and guide further management of patients with non-obstructive CAD. In the case presented, CCTA showed lipid-rich plaques in the left main stem and left anterior descending arteries, and the patient remained asymptomatic after treatment. The discussion highlights the value of CCTA as a diagnostic tool for assisting in the diagnosis and treatment of patients with non-obstructive CAD.

Impact of periprocedural major adverse events on 10-year mortality after revascularisation

Summary: This study aimed to evaluate the impact of non-fatal periprocedural major adverse events (PMAE) on 10-year mortality and investigate whether PMAE affect mortality predicted by SYNTAX score II 2020 (SSII-2020) and whether optimal medical therapy (OMT) has a positive effect on the prognosis of patients with non-fatal PMAE. The results show that non-fatal PMAE are more common following CABG than PCI, but their prognostic impact is similar, being significant in the first year and then diminishing out to 10 years. Therefore, patients with non-fatal PMAE may require more careful follow-up and additional preventive treatment in the first year post-procedure.
