Papillary thyroid microcarcinoma: optimal management versus overtreatment

Purpose of review The treatment of small, low-risk papillary thyroid carcinoma has undergone a paradigm shift, with many tumors now initially treated with active surveillance rather than upfront surgery. Further studies on patients enrolled in active surveillance have refined our knowledge of the clinical behavior of papillary thyroid microcarcinomas. Recent findings This article summarizes the major conclusions of landmark trials that launched active surveillance as a viable treatment option for selected patients. We discuss patient factors such as age and tumor size, the assessment of candidates for active surveillance, barriers to acceptance of active surveillance, quality of life issues, and economic considerations. Active Surveillance is a viable first-line treatment option for select papillary microcarcinomas.

Summary: This study investigated protein differential expression in HPV(+) OPSCC samples and found that functional pathways involved in cell death, apoptosis, WNT signaling, and Rho GTPase signaling play important roles in disease recurrence. Downregulation of certain molecular targets is associated with recurrence and may serve as markers for high-risk disease. Immunohistochemistry staining validated some of the mass spectrometry findings.

Summary: Radiofrequency ablation (RFA) is safe and effective for addressing compressive or cosmetic issues caused by benign thyroid nodules. Multiple staged RFA procedures may be necessary in some cases. Consensus guidelines have emerged to define appropriate patient candidates.

Summary: The study confirms that lymphovascular invasion and histologic grade are the strongest predictors of neck lymph node metastasis in cN0 patients with pharynx and larynx cancer. Neck dissection is still necessary for cN0 patients to identify the risk of lymph node metastasis even in the modern era.

Summary: This study investigated the impact of histologic variants and grade on the prognosis of salivary cancer. The results showed that histologic variants had different mortality risks, but these differences were diminished in multivariable analysis. In contrast, worsening grade was strongly associated with deteriorating survival. Integrating grade into stage can improve the predictive ability of the current staging system.

Re-examining predictors of pathologic lymph node positivity in clinically node negative oral cavity cancer

Summary: Elective lymph node dissection (ELND) is commonly performed for early-stage oral cavity squamous cell carcinomas (OCSCC) with clinically negative necks. However, the validity of depth of invasion (DOI) as a predictor of lymph node positivity (pN+) varies in different OC sites. This study found that lymphovascular invasion and histologic grade were strong independent predictors of pN+ in cN0 OCSCC, while DOI was not associated with pN+ in the overall OCSCC population, but showed some predictive value in the oral tongue subset.

Summary: Researchers analyzed whole-genome bisulfite sequencing data on 45 esophageal tumor and nonmalignant samples from both subtypes, revealing distinct DNA methylation features and partially methylated domains (PMDs) associated with transcription repression, chromatin compartments, and somatic mutation rate. They also found that these features co-occur in other cancers originating from the same cell types.
