Myositis and myasthenia during nivolumab administration for advanced lung cancer: a case report and review of the literature

The immunotherapy significantly improved survival of non-small cell lung cancer patients, but it may cause immune-related adverse events, which are severe in less than 10% of cases. We report the case of one patient who developed myositis and myasthenia during nivolumab treatment for metastatic lung squamous carcinoma. Moreover, we reviewed literature data in order to identify similar cases in cancer patients treated with immune-checkpoints inhibitors. A 65-year-old patient, who had previously received a first-line platinum-based therapy, developed diplopia and ptosis 4 weeks after the start of nivolumab. Although antibodies associated with myositis, myasthenia gravis and paraneoplastic syndromes were absent, immune-related myositis and myasthenia were diagnosed. Corticosteroids, immunoglobulin and pyridostigmine showed poor efficacy and the patient died 7 weeks after the appearance of the first symptoms. Fifteen similar cases were found in the literature. A close collaboration between different specialists is essential to rapidly identify and treat severe immune-related adverse events.

Summary: This study systematically reviewed the measurement properties of outcome instruments used in large-vessel vasculitis (LVV). Out of the 3534 articles identified, 13 met the criteria, addressing 12 instruments. The Indian Takayasu Clinical Activity Score (ITAS) and the Disease Extent Index-Takayasu (DEI-Tak) showed the most adequate measurement properties for disease activity/damage in TA, while specific instruments for GCA and validated patient-reported outcomes for both GCA and TA were lacking.

Summary: This study identifies a consistent and robust phenomenon called axonal response of mitochondria to demyelination (ARMD) in experimental demyelination models, which is also observed in multiple sclerosis (MS). The increase in mitochondrial content within demyelinated axons is not always accompanied by a proportionate increase in complex IV activity, particularly in highly inflammatory models. The findings highlight the importance of complex IV activity and pave the way for the development of novel neuroprotective therapies.

Diagnosing and treating ANCA-associated vasculitis: an updated review for clinical practice

Summary: ANCA-associated vasculitides (AAV) are rare, primary, systemic necrotizing small-vessel vasculitides. The most common types are granulomatosis with polyangiitis and microscopic polyangiitis. Risk factors for developing AAV include exposure to silica dust, farming, and chronic nasal Staphylococcus aureus infection. Diagnosis of AAV requires further investigation, including ANCA testing, and a structured clinical assessment. Treatment options vary depending on the severity of AAV and the patient's characteristics, with rituximab being favored as an effective option for both induction and maintenance of remission.

Targeting the interleukin-5 pathway in EGPA: evidence, uncertainties and opportunities

Advances in the pharmacotherapeutic management of eosinophilic granulomatosis with polyangiitis

Summary: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare systemic vasculitis with potentially lethal outcomes. Treatment options for EGPA have been adapted from other vasculitides, and monoclonal antibodies targeting various pathways, such as IL5 and B cells, have been investigated. This review provides an overview of published studies on EGPA treatments, including glucocorticoids, conventional immunosuppressants, anti-IL5 pathway agents, and potential future treatments. Expert opinion suggests that with advancements in pharmacotherapeutic management, the prognosis of EGPA has improved, and more targeted and safer treatments are available. However, further research and long-term data are needed to optimize treatment strategies for individual patients.

These guidelines are intended to guide clinical practice and decision-making in EGPA, and are based on a multidisciplinary approach and expert opinion.

Using the 6-min walk test to assess the clinical response to mepolizumab and conventional therapy in severe eosinophilic asthma

Summary: The objective of this study was to investigate the role of the 6-minute walk test (6MWT) in severe asthma. The results showed that patients treated with mepolizumab demonstrated significant improvements in pulmonary function tests and clinical laboratory parameters. Additionally, the 6MWT distance showed significant positive correlations with asthma control test and asthma quality of life questionnaire.

Summary: This study describes the presence of interstitial lung disease (ILD) in antineutrophil cytoplasmic antibodies-associated vasculitides (AAV). It found that non-specific interstitial pneumonia (NSIP) was the most common ILD pattern, observed in c-ANCA positive patients. The study also highlights the early complication of ILD in AAV and emphasizes the importance of early diagnosis by both pulmonologists and rheumatologists.
