Summary: Despite the challenges of the coronavirus pandemic, hypertension research and initiatives in Asia have been active in the past two years. New guidelines and research data contribute to local practices, and efforts to reduce high salt intake are being made. Drug-resistant hypertension and nocturnal hypertension are important research areas in Asia. Digital therapeutics and other digital strategies show promise in reducing the burden of hypertension. Ongoing research activities and initiatives provide optimism for the management of hypertension in Asia.

Summary: Artificial intelligence and machine learning systems have been increasingly used in medicine, including gastroenterology and hepatology, to improve clinical decision-making and healthcare delivery. However, there is a risk that these technologies can introduce or worsen biases and health inequities. The application of AI/ML in gastroenterology and hepatology can impact various aspects such as diagnosis, disease management, and screening. To advance clinical practice and promote health equity, an ethical framework for AI/ML development and application is necessary.

Summary: The NHS Health Check programme has been running for 10 years, with low uptake rates and attendees more likely to be diagnosed with conditions like hypertension and diabetes. Attendees were also more likely to receive treatment, such as statins and antihypertensive medicines, but the majority did not receive any treatment or referral. Less than 10% of those eligible for statins were actually prescribed them, indicating a need for targeted strategies for those at highest risk of cardiovascular disease.

Summary: Portopulmonary hypertension (POPH) is a common complication of liver disease and has a significant impact on patient prognosis. Recent studies have improved our understanding of the role of PAH therapy and liver transplantation in the management of POPH and their impact on overall survival. This review summarizes the available evidence and guidelines for the diagnosis and management of POPH.

Summary: This study retrospectively investigated the clinical outcomes of patients with hepatic hydrothorax and identified factors associated with mortality. The results showed a low 12-month survival rate for patients with hepatic hydrothorax, and current smoking and acute kidney injury were identified as modifiable factors affecting survival.

Summary: This study aimed to evaluate the relationship between the severity of non-alcoholic fatty liver disease (NAFLD) and cardiovascular events (CVEs) among patients with prehypertension or hypertension. The results indicated that the severity of NAFLD is associated with an increased risk of CVEs in patients with prehypertension or hypertension.

Summary: Cardiovascular complications are a significant cause of death among LT patients, with a history of coronary artery disease being linked to increased CV mortality. There is a lack of consensus on optimal screening methods, highlighting the need for individualized assessment. Prospective studies with large sample sizes are necessary to improve risk stratification and survival prediction.

Summary: This study analyzed the association between home and ambulatory blood pressure monitoring and cardiovascular risk in hypertensive outpatients, and found that both home blood pressure and ambulatory blood pressure were independently associated with cardiovascular risk. Home blood pressure had a modest superiority to predict cardiovascular prognosis compared to ambulatory blood pressure.

Summary: Utilization of hepatitis C viremic kidneys is increasing in the US, and this study examined racial disparities in this utilization. The study found that recipient race does not independently determine who receives HCV viremic kidneys, but factors such as waiting time, education (for seronegative recipients), and center racial mix (for seropositive recipients) contribute to the differential distribution of HCV viremic kidneys among races.

Summary: Despite the excellent outcomes and expanding candidate indications for living donor liver transplantation (LDLT), there is still limited uptake of this option in the United States. To address this issue, the American Society of Transplantation organized a virtual consensus conference to identify barriers and recommend strategies. The barriers identified include lack of awareness, acceptance, and engagement among patients, providers, and institutions; data gaps and lack of standardization in candidate and donor selection; and data gaps regarding post-donation outcomes and resource needs. The strategies to address these barriers involve education and engagement, collaborative research, and institutional commitment and resources.

Summary: Biliary atresia (BA) is the leading indication for pediatric liver transplantation worldwide, but it is uncommon for older liver transplant recipients. The impact of recent donor allocation changes, organ shortage, and evolving transplantation practices on the BA liver transplant population is unknown. The study found that liver transplant outcomes in BA patients are excellent, and optimal graft outcomes are associated with living-donor liver transplantation (LDLT) and higher transplant center volume.

Summary: Chronic liver disease (CLD) is a prevalent condition that includes subclinical liver disease which may have an impact on stroke risk, mechanisms, and outcomes. However, there is limited synthesis of data and guidance on the relationship between CLD and stroke. Therefore, a multidisciplinary review is necessary to evaluate the correlation between CLD and stroke and provide treatment considerations.
