Summary: This study investigates the metabolic differences in different subgroups of newly diagnosed treatment-naive IBD patients. The results indicate alterations in energy and amino acid metabolism in IBD patients, with an increased demand for ketone bodies. The differences in metabolites are more pronounced in Crohn's disease patients compared to ulcerative colitis patients, and in serum compared to urine. Cluster analysis identifies three distinct patient profiles with significant differences in clinical, anthropometric, and metabolomic variables.

High clinical remission rates were observed in pediatric patients refractory to anti-TNF compared to those reported in adults, with few publications on pediatric populations treated with Vedolizumab and with long-term follow-up (52 weeks).

Summary: Inflammatory bowel diseases (IBDs) are chronic and inflammatory conditions affecting the gastrointestinal tract. Despite endoscopy being the gold standard test for assessing IBD, it is invasive and uncomfortable for patients. Therefore, non-invasive biomarkers for IBD diagnosis are urgently needed. This review summarizes proteomics and metabolomics studies in animals and humans, identifying urinary biomarkers for IBD diagnosis. Large-scale multi-omics studies with collaboration between clinicians, researchers, and industry are necessary for the development of sensitive and specific diagnostic biomarkers for personalized medicine.

Different diets, such as exclusive enteral nutrition, specific carbohydrate diet, and low FODMAPs diet, have also been explored for their effects on IBD patients. This review summarizes the current evidence on the relationship between dietary components and IBD, and provides guidance for dietary modifications in managing IBD.

Summary: The aim of this study was to validate the content of IBD-Disk in a Greek cohort of IBD patients. The results showed good correlation and consistency between the scores of IBD-Disk and IBD-DI at baseline and follow-up. Female gender and extraintestinal manifestations were significantly associated with a higher IBD-Disk total score. Therefore, the Greek version of IBD-Disk was proven to be a reliable and valid tool for assessing IBD-related disability.

Colorectal cancer poses the highest risk, which is associated with smoking, exposure to anti-tumour necrosis factor, and combination therapy. Summary: This study aimed to investigate the relationship between MMP-9 and TIMP-1 and UC phenotypes, disease activity index, and inflammatory markers. The results showed that children with UC had significantly higher levels of these markers in their serum and feces compared to the control group. Additionally, serum MMP-9 correlated with the severity of the disease and other markers, while serum TIMP-1 correlated with disease activity, inflammatory markers, and erythrocyte sedimentation rate. Serum and fecal levels of MMP-9 and TIMP-1 can be useful in distinguishing UC patients and evaluating disease phenotypes.

Summary: The coexistence of inflammatory bowel disease (IBD) with pancreatic pathology is rare in children. A retrospective analysis of data from 1538 children diagnosed with IBD in 2014-2021 found that 11.4% of them had pancreatic involvement (PI), with acute pancreatitis (AP) diagnosed in 43.8% and asymptomatic hyperlipasemia (HL) or hyperamylasemia (HA) observed in 50.0%. Routine monitoring of pancreatic enzymes should be performed in patients with IBD, especially after the initiation of azathioprine (AZA) treatment.

Summary: This study conducted a systematic review of the literature on pancreatic disorders associated with IBD in children, finding that acute pancreatitis is the most common pancreatic disease in pediatric IBD, with manifestations ranging from asymptomatic increase in pancreatic enzyme activity to severe pancreatitis. The involvement of the pancreas can be closely related to the activity of IBD, and it is recommended to include pancreatic enzyme level measurement in the workup of IBD.
