Summary: The study found that in the current era, ES-ASD or ES-VSD patients represented a decreasing proportion of all lung or heart-lung transplants compared to other indications. The odds of transplantation for ES-ASD and ES-VSD patients were significantly lower compared to other candidates for lung transplantation. In the early era, ES-ASD patients who underwent heart-lung transplants had equivalent survival rates to those who underwent lung transplants, while ES-VSD patients had superior survival rates.

Summary: This study analyzed the characteristics and outcomes of waitlisted patients for heart transplant in the US between 2018 and 2020, finding that patients using exception status were more likely to have nonischemic heart failure and blood type O. There were significant variations in exception status utilization across different regions and centers, and listing by exception criteria was associated with a higher incidence of heart transplantation compared to listing by standard criteria.

Summary: Heart failure is the leading cause of morbidity and mortality in adult patients with congenital heart disease. The nature of the underlying congenital heart disease affects the timing and severity of heart failure, and ongoing management is necessary. Drug therapy has some effect on left ventricle failure, but there is currently a lack of data on drug therapy for right ventricle and Fontan circulation failure. Cardiac resynchronization therapy is considered as a treatment option, while mechanical circulatory support and transplantation are last resorts in highly selected patients.
