Introduction: Extracellular matrix (ECM) remodeling of the intestinal tissue is important in inflammatory bowel disease (IBD) due to the extensive mucosal remodeling. There are still gaps in our knowledge as to how ECM remodeling is related to intestinal epithelium homeostasis and healing of the intestinal mucosa. Areas covered: The aim of this review is to highlight the importance of the ECM in relation to the pathogenesis of IBD, while addressing basement membrane and interstitial matrix remodeling, and the processes of wound healing of the intestinal tissue in IBD. Expert opinion: In IBD, basement membrane remodeling may reflect the integrity of the intestinal epithelial-cell homeostasis. The interstitial matrix remodeling is associated with deep inflammation such as the transmural inflammation as seen in fistulas and intestinal fibrosis leading to fibrostenotic strictures, in patients with CD. The interplay between wound healing processes and ECM remodeling also affects the tissue homeostasis in IBD. The interstitial matrix, produced by fibroblasts, holds a very different biology as compared to the epithelial basement membrane in IBD. In combination with integration of wound healing, quantifying the interplay between damage and repair to these sub compartments may provide essential information in IBD patient profiling, mucosal healing and disease management.

A Highly Sensitive Biomarker of Type II Collagen C-Terminal Pro-Peptide Associated with Cartilage Formation

Helena Port, Anne-Christine Bay-Jensen, Yi He, Morten A. A. Karsdal, Thorbjorn Gantzel, Christian S. S. Thudium, Signe Holm Nielsen

Summary: The development of a competitive chemiluminescence enzyme-linked immunosorbent assay, CALC2, targeting the type II collagen C-terminal pro-peptide as a marker of cartilage formation was evaluated. The CALC2 level was measured after in vitro cleavage of recombinant type II collagen and treatment of human osteoarthritis cartilage explant model with insulin-like growth factor-1. Serum CALC2 levels were found to be decreased in patients with rheumatoid arthritis and ankylosing spondylitis compared to controls. These results suggest that CALC2 may be a novel biomarker of type II collagen formation, but further studies are needed to confirm these findings.

Endoscopic severity and classification of lesions with pan-enteric capsule endoscopy and ileocolonoscopy in ileocolonic Crohn's disease

Jacob Broder Brodersen, Jens Kjeldsen, Torben Knudsen, Michael Dam Jensen

Summary: This study aims to compare the agreement between pan-enteric capsule endoscopy (CE) and ileocolonoscopy (IC) for determining the severity and classification of lesions in ileocolonic Crohn's disease (CD). The results show a strong correlation between IC and CE in assessing the severity of ileocolonic CD, but fair to moderate agreement in sub-scores.

Structural changes after ankle joint distraction in haemophilic arthropathy: an explorative study investigating biochemical markers and 3D joint space width

Eline D. P. van Bergen, Simon C. Mastbergen, Floris P. J. G. Lafeber, Anne-Christine Bay-Jensen, Sofie F. Madsen, Helena Port, Wouter Foppen, Roger E. G. Schutgens, Mylene P. Jansen, Lize F. D. van Vulpen

Summary: This study aimed to quantitatively evaluate the structural changes after AJD in patients with HAA by using 3D joint space width (JSW) measurements and biochemical markers, and to correlate these findings with clinical pain/function. The results showed that on group level, there was a slight decrease in percentage changes in JSW after 12 months, followed by a non-statistically significant increase after 36 months compared to baseline. Biochemical marker collagen/cartilage formation also showed an initial decrease, followed by a trend towards net formation 12, 24 and 36 months after AJD. On individual patient level, no clear correlations between structural changes and clinical parameters were observed.

Lot-to-Lot Variance in Immunoassays-Causes, Consequences, and Solutions

Yunyun Luo, Martin Pehrsson, Lasse Langholm, Morten Karsdal, Anne-Christine Bay-Jensen, Shu Sun

Summary: Immunoassays have become increasingly important in clinical practice and biomedical research, but they are plagued by lot-to-lot variance, which negatively affects assay accuracy and precision. This article explores the reasons and locations of lot-to-lot variance, and proposes methods to mitigate its effects. The findings highlight the importance of considering lot-to-lot variance in assay development and application.
