Prognostic Impact of the Neutrophil-to-Lymphocyte Ratio in Borderline Resectable Pancreatic Ductal Adenocarcinoma Treated with Neoadjuvant Chemoradiotherapy Followed by Surgical Resection

Background Increasing evidence suggests that cancer-associated inflammation, as indicated by markers such as the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and modified Glasgow Prognostic Score (mGPS), predicts poor outcomes in pancreatic cancer. In this study, the associations between systemic inflammation markers and survival were examined in borderline resectable pancreatic ductal adenocarcinoma (BR-PDAC) patients who underwent neoadjuvant chemoradiotherapy (NACRT) followed by surgical resection.' Methods From April 2009 to December 2017, 119 patients diagnosed with BR-PDAC and receiving NACRT followed by radical surgery were included in this retrospective study. The associations between the pre- and post-NACRT NLR, PLR, mGPS, and clinicopathological characteristics, as well as their predictive values for survival outcomes, were analyzed. This study was approved by an institutional review board at Yokohama City University (B180600049). Results On multivariate analysis with a Cox's proportional hazards regression model, post-NACRT NLR >= 3 (p = 0.040; hazard ratio, 2.24; 95% CI 1.28-3.91) and lymph node metastasis (p = 0.002; hazard ratio, 2.33; 95% CI 1.36-3.99) were significantly associated with shorter overall survival. The median survival time was 22.0 months for patients with post-NACRT NLR >= 3 and 45.0 months for patients with post-NACRT NLR <3 (p = 0.028). Conclusions The NLR following NACRT might predict survival in BR-PDAC patients. Patients with an elevated post-NACRT NLR or positive lymph node metastasis may be candidates for stronger adjuvant therapies.

Mucin phenotypes and clinicopathological features of colorectal adenocarcinomas: Correlation with colorectal adenocarcinoma with enteroblastic differentiation

Summary: This study classified a large number of colorectal carcinoma (CRC) cases based on mucin phenotypes and investigated their correlation with tumor development, biological behavior, and prognosis. The results showed that small-intestinal type and unclassified type had a poor prognosis in CRCs. Furthermore, colorectal adenocarcinoma with enteroblastic differentiation (CAED) mostly belonged to the small-intestinal type or unclassified type. These findings suggest that the phenotypic classification is useful for predicting the prognosis of CRCs, and CAED might reflect the aggressive behavior and poor prognosis of small-intestinal and unclassified types.

Summary: This study aims to develop a malaria vaccine that provides cross-strain protection. Results showed that inoculation of mice with a heterologous live-vaccine prevented the development of experimental cerebral malaria (ECM) and protected the blood-organ barriers from infection. The immune response induced by the live vaccine included cross-reactive antibodies and IL-10-mediated host protection against ECM.

Summary: This study investigated the utility and safety of subcutaneous implantable cardioverter defibrillators (S-ICDs) in patients <= 18 years old. The results suggested that S-ICDs can effectively prevent sudden cardiac death in the pediatric population with a low incidence of lead complications or device infections. The number of suitable sensing vectors did not change during the patients' growth.

The introduction of various monitoring systems has led to a decrease in PNI incidence, but only the application of high-power, short-duration radiofrequency energy on risk points tagged by a 3-dimensional electro-anatomical mapping (EAM) system completely prevents PNI.

Fluorinated [2]rotaxanes with spirofluorene motifs: a non-symmetric distribution of the ring component along the axle component

Summary: Oxidative dimerization of terminal alkynes (Glaser coupling) triggered by spirofluorene derived macrocyclic phenanthroline-Cu complexes was used to synthesize a series of novel [2]rotaxanes. The size and structural variations among the components and their effects on the dynamic behavior were investigated in detail using H-1 and F-19 NMR spectroscopy. Non-symmetric distribution of the ring component along the axle component was observed in the NMR spectra of rotaxanes with substituted macrocyclic components with low symmetry, and localization of the ring component near the 1,3-diyne moiety was observed in some [2]rotaxanes.

Summary: We present a neonatal case of DSM that was diagnosed prenatally and treated via endovascular intervention in the early postnatal period. The patient presented with a large DSM involving the torcular Herophilion prenatal magnetic resonance imaging (MRI). The neonate underwent endovascular occlusion via the umbilical artery, successfully treating the DSM.

Prevalence, definition, and etiology of cesarean scar defect and treatment of cesarean scar disorder: A narrative review

Summary: This review summarizes the definition, prevalence, and etiology of Cesarean scar defects (CSD), as well as the pathophysiology and treatment of Cesarean scar disorder (CSDi), focusing on surgical therapy and examining the effects and procedures of laparoscopy, hysteroscopy, and transvaginal surgery. The definition of CSD was proposed as an anechoic lesion with a depth of at least 2 mm, while the incidence of CSD depends on the number, procedure, and situation of cesarean sections. Histopathological findings in CSD include fibrosis and adenomyosis, and chronic inflammation in the uterine and pelvic cavities affects fertility in women with CSDi. Although the surgical procedures are not standardized, laparoscopic, hysteroscopic, and transvaginal surgeries have shown effectiveness.
