Summary: This study retrospectively analyzed data from 134 patients who underwent subtotal D2 gastrectomy for locally advanced gastric cancer and compared the short-term and surgical outcomes between laparoscopic and open gastrectomy procedures. The results showed that laparoscopic surgery had shorter surgical time, less blood loss, faster postoperative recovery, and shorter hospital stay compared to open gastrectomy. There were no differences in surgical morbidity and mortality rates, as well as disease-free survival and overall survival rates, between the two groups. Therefore, laparoscopic gastrectomy is a feasible and safe treatment option for locally advanced gastric cancer.

Summary: Limited research has been conducted on the application of laparoscopic gastrectomy (LG) in locally advanced gastric cancer (LAGC) patients following neoadjuvant chemotherapy (NACT). This study aimed to evaluate the surgical and survival outcomes of LG in LAGC patients after NACT. The results showed that LG after NACT had better short-term and long-term outcomes with lower postoperative complication risks. Therefore, LG following NACT should be considered as the priority treatment for LAGC patients.

Summary: The NEONIPIGA study found that perioperative platinum-based chemotherapy is the standard treatment for resectable gastric/gastroesophageal junction (GEJ) adenocarcinoma. However, the efficacy of perioperative chemotherapy in gastric/GEJ adenocarcinoma with deficient mismatch repair (dMMR)/microsatellite instability-high (MSI-H) remains debatable. This study showed that neoadjuvant nivolumab-based therapy is feasible, associated with no unexpected toxicity, and has a high pathological complete response (pCR) rate in patients with dMMR/MSI-H resectable gastric/GEJ adenocarcinoma.
