Trends in adherence to the Mediterranean diet in South Italy: A cross sectional study

Background and aims: Increasing literature data show that adherence to the Mediterranean diet is undergoing profound changes in recent years, albeit with marked differences across nations. In Italy, one of the cradles of the Mediterranean diet, the literature regarding the trend for Mediterranean diet adherence is conflicting. Thus, we aimed to explore the trends of adherence to the Mediterranean diet in a large cohort of participants living in South Italy, over 20 years from 1985-86 to 2005-06. Methods and results: Cross-sectional study with two evaluations, one made in 1985-86 and another in 2005-06; all participants were adults aged 30-70 years of age. The adherence to the Mediterranean diet was evaluated using the score proposed by Panagiotakos et al. This score features values ranging from 0 to 55, higher scores reflecting a greater adherence. The data are reported by age (30-49 vs. 50-69 years). Overall, 2451 subjects were included in 1985-86 and 2375 in 2005-06. A significant reduction was observed in the adherence to the Mediterranean diet (age 30-49 years: 31.82 +/- 4.18 in 1985-86 vs. 29.20 +/- 4.48 in 2005-06, reduction by 8.2%, p < 0.0001; age 50-69: 32.20 +/- 4.09 in 1985-86 vs.30.15 +/- 4.27 in 2005-06, reduction by 6.3%, p < 0.0001). Among all these items, the most dramatic change was observed for olive oil consumption, that decreased by 2.35 points in younger and 0.89 in older people. Conclusion: The adherence to the Mediterranean diet decreased from 1985-86 to 2005-06 in South Italy, particularly in younger people, above all due to a decreased olive oil consumption. Summary: The immune response to COVID-19 is important, but some patients experience persistent symptoms that lead to a global health and economic burden. This study investigates the potential role of IL-6 levels as a marker for long COVID. The analysis of 184 patients admitted to a hospital in Italy revealed that higher IL-6 levels were associated with a doubled risk of long COVID, mobility decline, and PTSD, indicating the significance of IL-6 in disease severity, unfavorable outcomes, and long COVID development.

Summary: The COH-FIT survey is a valid instrument to measure global mental health during the COVID-19 pandemic. The study confirmed the validity of COH-FIT items and the internal reliability of the co-primary outcome, the P-score. The survey has been translated into 30 languages and measures various mental health symptoms and factors.

Mediterranean diet and mitochondria: New findings

Summary: Mitochondria play a critical role in various energy processes and are important in the physiological response to stress, aging, and disease. The Mediterranean diet has been found to affect mitochondrial function, reducing the risk of negative health outcomes. Components of the Mediterranean diet, such as polyphenols, can limit free radicals and have anti-inflammatory effects, improving mitochondrial function. It is necessary to conduct further studies in humans to confirm these findings.

Summary: Frailty is a geriatric syndrome with increasing frequency and dire consequences due to population aging. Nutritional factors, particularly the Mediterranean diet, play a crucial role in its pathogenesis. A systematic review and meta-analysis confirmed a robust association between higher adherence to the Mediterranean diet and reduced incident frailty. Promoting the Mediterranean diet as a high-quality dietary pattern adapted to different regions offers an opportunity to counteract the spread of unhealthy eating and lifestyle models and reduce frailty incidence.

Summary: This study aimed to investigate the association between dynapenic abdominal obesity (DAO) at baseline and the development of multimorbidity over a 10-year follow-up period. The results showed that both abdominal obesity and DAO significantly increased the risk of multimorbidity. DAO was also associated with a higher risk of arthritis and diabetes.

An update on the use of conventional and biological disease-modifying anti-rheumatic drugs in hand osteoarthritis

Summary: Osteoarthritis (OA) is a prevalent condition worldwide, causing pain, disability, reduced social activities, and impaired quality of life. The current therapeutic options are limited and focus mainly on managing symptoms, lacking disease-modifying drugs. Efforts have been made to find potential therapeutic targets for OA, repurposing existing drugs used for inflammatory rheumatic diseases. Unfortunately, the use of conventional and biologic drugs for hand osteoarthritis (HOA) has not been successful, highlighting the need for research on HOA molecular mechanisms and personalized treatment options.
