Early Natural Killer Cell Reconstitution Predicts Overall Survival in T Cell-Replete Allogeneic Hematopoietic Stem Cell Transplantation

Early immune reconstitution plays a critical role in clinical outcome after allogeneic hematopoietic stem cell transplantation (HSCT). Natural killer (NK) cells are the first lymphocytes to recover after transplantation and are considered powerful effector cells in HSCT. We aimed to evaluate the clinical impact of early NK cell recovery in T cell-replete transplant recipients. Immune reconstitution was studied in 298 adult patients undergoing HSCT for acute myeloid leukemia, acute lymphoblastic leukemia, and myelodysplastic syndrome from 2005 to 2013. In multivariate analysis NK cell numbers on day 30 (NK30) > 150 cells/mu L were independently associated with superior overall survival (hazard ratio,.79; 95% confidence interval,.66 to.95; P=.01). Cumulative incidence analyses showed that patients with NK30 > 150 cells/mu L had significantly less transplant-related mortality (TRM), P =.01. Patients with NK30 > 150 cells/mu L experienced significantly lower numbers of life threatening bacterial infections as well as viral infections, including cytomegalovirus. No association was observed in relation to relapse. These results suggest an independent protective effect of high early NK cell reconstitution on TRM that translates into improved overall survival after T cell-replete HSCT. Ocular graft-versus-host disease and dry eye disease after paediatric haematopoietic stem cell transplantation - incidence and risk factors

Summary: This nationwide single-centre study investigated the incidence and risk factors for ocular graft-versus-host disease (oGVHD) and dry eye disease (DED) in children receiving allogeneic haematopoietic stem cell transplantation (HSCT). The study found a relatively high incidence of oGVHD and DED after HSCT, with the use of Busulfan and malignant disease being identified as risk factors. Younger recipient age was associated with a reduced risk of DED. The study highlights the importance of ophthalmic examinations before and after HSCT to diagnose and treat these complications.

Flow Cytometric Detection of Malignant Blasts in Cerebrospinal Fluid: A Biomarker of Central Nervous System Involvement in Childhood Acute Lymphoblastic Leukemia

Summary: Despite the good prognosis of acute lymphoblastic lymphoma in children and adolescents, involvement of the central nervous system (CNS) remains a therapeutic challenge. The current method for detecting CNS involvement has low sensitivity, while flow cytometric analysis of cerebrospinal fluid (CSF) offers a novel and sensitive approach. Prospective studies have shown that flow cytometry detects CNS involvement at a higher rate than conventional methods and is particularly sensitive in detecting low-level CNS disease. Flow cytometry-detected CNS involvement is linked to a higher risk of relapse and poor outcomes in childhood ALL.

Summary: The study aimed to evaluate the validity of the HCT-CI and pulmonary comorbidity in predicting NRM and OS after HCT. The results showed that both the PCS and HCT-CI had predictive value for OS, while only the PCS was significant for predicting NRM. Pulmonary function was found to be a strong predictor of 2-year OS and NRM after HCT.

Summary: We found impairments in innate immune response pathways at the time of COVID-19 hospitalization that are associated with subsequent disease severity. However, these impairments were transient in those who were discharged from the hospital, indicating functional and cellular immune reconstitution. Additionally, we propose a stimulated immune response signature for predicting the risk of developing severe or critical COVID-19 disease at the time of hospitalization, which needs to be validated in larger cohorts.
